Epril ist ein Arzneimittel aus der Gruppe der sog. ACE-Hemmer (Angiotensin-Converting-Enzym-Hemmer) und dient zur Behandlung des hohen Blutdruckes und der Herzinsuffizienz (Herzmuskelschwäche, ungenügende Herzleistung) und zur Vorbeugung einer Herzinsuffizienz bei Patienten, die ein geschwächtes Herz haben. Seine Wirkung beruht auf der Hemmung von körpereigenen Stoffen, die für den erhöhten Blutdruck verantwortlich sind. Dadurch kann der Blutdruck gesenkt und die Leistung des Herzens verbessert werden.
Ihr Arzt oder Ihre Ärztin hat Ihnen Epril verschrieben, um Ihren hohen Blutdruck oder Ihre Herzinsuffizienz zu behandeln. Das Arzneimittel kann Ihnen auch vom Arzt oder der Ärztin verordnet werden, um der Entwicklung einer symptomatischen Herzinsuffizienz vorzubeugen.
Epril darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden.
Nehmen Sie Epril nicht ein,
Wenn Sie nicht sicher sind, ob Sie Epril einnehmen dürfen, wenden Sie sich an Ihren Arzt oder Ihre Ärztin.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen.
Informieren Sie Ihren Arzt oder Ihre Ärztin über alle medizinischen Probleme, die Sie haben oder früher gehabt haben, insbesondere über Allergien.
Sagen Sie Ihrem Arzt oder Ihrer Ärztin, wenn Sie ein Herzproblem haben, sich einem Blutreinigungsverfahren (Dialyse) unterziehen oder mit harntreibenden Mitteln (Diuretika) behandelt werden oder wenn Sie kürzlich an übermässigem Erbrechen oder Durchfall gelitten haben. Teilen Sie ihm oder ihr auch mit, wenn Sie eine salzarme Diät befolgen, Kaliumzusätze, kaliumsparende Mittel oder Kalium enthaltende Diätsalze einnehmen, oder wenn Sie Diabetes oder Nierenprobleme haben, da dies zu erhöhten Kaliumkonzentrationen im Blut führen kann, die ernsthaft sein können. In diesen Fällen kann Ihr Arzt oder Ihre Ärztin eine Anpassung der Dosierung für notwendig erachten oder Ihre Kaliumwerte im Blut überprüfen. Falls Sie Diabetes haben und orale Antidiabetika (Arzneimittel zur Behandlung der Zuckerkrankheit) oder Insulin einnehmen, dann sollten Sie Ihr Blut auf tiefe Blutzuckerwerte hin überwachen, insbesondere während der ersten Monate der Behandlung mit Epril.
Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie jemals eine allergische Reaktion mit Schwellungen im Gesicht, an den Lippen, Zunge und/oder im Rachen (mit Schluck- oder Atembeschwerden) gehabt haben.
Teilen Sie Ihrem Arzt oder Ihrer Ärztin mit, wenn Sie an tiefem Blutdruck leiden (Sie können dies als Schwäche oder Schwindel besonders beim Stehen bemerken).
Vor einer Operation oder Narkose (auch beim Zahnarzt) sollten Sie Ihren Arzt oder Zahnarzt bzw. Ihre Ärztin oder Zahnärztin über die Einnahme von Epril informieren, da während der Narkose ein plötzlicher Abfall des Blutdrucks auftreten kann.
Wenn Sie denken, dass Sie schwanger sein könnten (oder werden könnten), müssen Sie Ihren Arzt bzw. Ihre Ärztin informieren. Es wird nicht empfohlen, Epril während der frühen Schwangerschaft einzunehmen, da Epril das Baby nach dem 3. Schwangerschaftsmonat schwerwiegend schädigen kann (siehe Kapitel «Darf Epril während einer Schwangerschaft oder in der Stillzeit eingenommen werden?»).
Wenn Sie sich einer Desensibilisierungstherapie gegen Bienen- oder Wespenstiche unterziehen wollen, informieren Sie vorher Ihren Arzt oder Ihre Ärztin, da Ihre Therapie mit Epril geändert werden muss. Selten kann es nach Insektenstichen zu allergischen Reaktionen kommen.
Im Allgemeinen kann Epril mit anderen Arzneimitteln eingenommen werden. Es ist aber wichtig, Ihren Arzt oder Ihre Ärztin über andere Arzneimittel zu informieren, die Sie einnehmen, auch über solche, die Sie ohne Rezept erhalten, da einige Arzneimittel sich einander in der Wirkung beeinflussen können. Um die korrekte Dosis von Epril zu verordnen, ist es für Ihren Arzt oder Ihre Ärztin besonders wichtig zu erfahren, ob Sie andere Arzneimittel zur Blutdrucksenkung, harntreibende Mittel (Diuretika), kaliumhaltige Mittel (einschliesslich Diätsalze), Arzneimittel zur Behandlung von Diabetes (einschliesslich oraler Antidiabetika und Insulin), Lithium (ein Arzneimittel zur Behandlung einer bestimmten Art von Depression) oder gewisse Arzneimittel gegen Schmerzen und Rheuma einnehmen, einschliesslich Behandlungen mit Arzneimitteln die Gold enthalten.
Die Sicherheit und Wirksamkeit von Enalapril bei Kindern und Jugendlichen wurde untersucht. (Siehe auch «Wie verwenden Sie Epril?»).

Wenn Sie denken, dass Sie schwanger sein könnten (oder werden könnten), müssen Sie Ihren Arzt bzw. Ihre Ärztin informieren. Normalerweise wird Ihnen Ihr Arzt bzw. Ihre Ärztin empfehlen, anstelle von Epril ein anderes Arzneimittel einzunehmen, da Epril während der frühen Schwangerschaft nicht empfohlen wird und Ihr Baby schwerwiegend geschädigt werden kann, wenn Epril nach dem 3. Schwangerschaftsmonat eingenommen wird.
Normalerweise muss Epril vor dem Beginn einer Schwangerschaft durch ein anderes geeignetes blutdrucksenkendes Arzneimittel ersetzt werden. Epril sollte nicht während dem zweiten und dritten Trimester der Schwangerschaft eingenommen werden.
Normalerweise wird Ihr Arzt Ihnen, sobald Sie wissen, dass Sie schwanger sind, empfehlen, Epril abzusetzen.
Wenn Sie während der Therapie mit Epril schwanger werden, informieren und sehen Sie Ihren Arzt bitte unverzüglich.
Epril wird in sehr kleinen Mengen in der menschlichen Milch ausgeschieden. Konsultieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie stillen oder wenn Sie beabsichtigen, zu stillen.
Epril kann vor, während oder nach den Mahlzeiten eingenommen werden. Epril wird mit einem Glas Wasser eingenommen. Die Dosierung wird vom Arzt oder der Ärztin festgelegt.
Ihr Arzt oder Ihre Ärztin wird abhängig von Ihrem Zustand und Ihrer Begleitmedikation mit anderen Arzneimitteln die für Sie geeignete Dosis von Epril bestimmen.
Nehmen Sie Epril jeden Tag genau nach Anweisung Ihres Arztes oder Ihrer Ärztin ein. Es ist sehr wichtig, mit der Einnahme des Arzneimittels so lange fortzufahren, wie Ihr Arzt bzw. Ihre Ärztin es Ihnen verschrieben hat.
Die übliche empfohlene Anfangsdosis beträgt bei den meisten Patientinnen und Patienten 10 bis 20 mg (1 Tablette Epril mite oder Epril oder ½ Tablette Epril 40) einmal täglich. Einige Patientinnen und Patienten können eine niedrigere Anfangsdosis benötigen.
Die übliche Erhaltungsdosis beträgt eine Tablette zu 20 mg Epril einmal täglich.
Über die Anwendung bei Kindern und Jugendlichen ab ca. 6 Jahren liegen begrenzte Daten vor. Die entsprechende Dosierung wird Ihr Arzt bzw. Ihre Ärztin festlegen.
Die übliche empfohlene Anfangsdosis beträgt 2,5 mg (½ Tablette Epril submite) einmal täglich. Ihr Arzt oder Ihre Ärztin wird diese Dosis schrittweise erhöhen, bis die richtige Dosis für Sie gefunden worden ist.
Die übliche Erhaltungsdosis beträgt eine Tablette zu 20 mg Epril einmal täglich oder 2 Tabletten Epril mite aufgeteilt in 2 Gaben pro Tag oder einmal täglich ½ Tablette Epril 40.
Achten Sie besonders darauf, wenn Sie die erste Dosis einnehmen oder wenn Ihre Dosis erhöht wird. Falls Sie Benommenheit oder Schwindel verspüren, verständigen Sie unverzüglich Ihren Arzt oder Ihre Ärztin.
Die Behandlung der Herzinsuffizienz sollte nur unter strenger ärztlicher Kontrolle erfolgen. Die Dosierung wird vom Arzt oder der Ärztin festgelegt und dient zur Ergänzung einer bereits laufenden Behandlung (meist Digitalis und/oder Diuretikum).
Es ist wichtig, dass Sie während der Einnahme von Epril die Kontrolluntersuchungen bei Ihrem Arzt oder Ihrer Ärztin regelmässig einhalten, damit er bzw. sie die Therapie individuell Ihren Beschwerden anpassen kann.
Sollten Sie jedoch einmal die Einnahme vergessen, so nehmen Sie keine Extradosis ein, sondern fahren Sie am folgenden Tag wie gewohnt mit der Einnahme fort.
Bei einer Überdosierung sollten Sie sofort mit Ihrem Arzt oder Ihrer Ärztin Kontakt aufnehmen, sodass eine allfällige medizinische Behandlung rechtzeitig erfolgen kann. Das wahrscheinlichste Symptom einer Überdosierung wäre ein Gefühl von Benommenheit oder Schwindel infolge eines plötzlichen, übermässigen Blutdruckabfalls.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme von Epril auftreten: die häufigsten Nebenwirkungen sind Schwindel, Kopfschmerzen, Müdigkeit und Schwäche. Andere weniger häufig auftretende Nebenwirkungen sind Benommenheit infolge Blutdruckabfall, kurzdauernder Bewusstseinsverlust (Ohnmacht), Übelkeit, Durchfall, Muskelkrämpfe, Hautausschlag, Husten, Nierenfunktionsstörungen, verminderte Harnausscheidung, Schwindelgefühl oder Benommenheit beim raschen Aufstehen infolge Blutdruckabfall, Herzklopfen, Brustschmerz.
Selten können auch andere als die oben aufgeführten Nebenwirkungen auftreten; einige davon können schwerwiegend sein. Fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin nach weiteren Informationen über Nebenwirkungen, die über eine vollständige Liste der unerwünschten Wirkungen verfügen.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin sofort, wenn Sie solche oder andere ungewöhnliche Symptome beobachten.
Unterbrechen Sie die Einnahme von Epril in irgendeinem der folgenden Fälle und kontaktieren Sie sofort Ihren Arzt oder Ihre Ärztin, wenn Sie:
Sie sollten wissen, dass schwarze Patienten und Patientinnen einem erhöhten Risiko für diese Art von Reaktionen gegenüber ACE-Hemmern ausgesetzt sind.
Die Anfangsdosis kann einen grösseren Blutdruckabfall bewirken, als anschliessend bei fortgesetzter Behandlung. Sie können dies als Schwäche oder Schwindel bemerken und es kann hilfreich sein, sich hinzulegen. Wenn Sie beunruhigt sind, fragen Sie bitte Ihren Arzt oder Ihre Ärztin um Rat.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
