Isoket Spray ist ein Arzneimittel zur Erweiterung der Herzkranzgefässe, eine Nitroverbindung zum Einsprühen in den Mund. Sobald er in die Mundhöhle gesprüht wird, bewirkt Isoket Spray eine rasche Beseitigung des Angina-Pectoris Anfalles. Das Sprühventil setzt bei jeder Betätigung gleiche Mengen Wirkstoff in Form eines fein verteilten Nebels frei. Dieser dringt mit Leichtigkeit durch die Schleimhäute des Mundes und gelangt auf diese Weise in den Kreislauf.

Der Arzt kann das Arzneimittel auch bei anderen Herzkrankheiten verordnen.
Die Verengung der Herzkranzgefässe wird durch bestimmte Risikofaktoren begünstigt, so z.B. Fettstoffwechselstörungen, die zu Fettablagerungen in den Arterien («Verkalkung») führen können, hoher Blutdruck, Übergewicht infolge falscher Ernährung, Zuckerkrankheit, Bewegungsmangel, Rauchen. Durch Vermeidung oder Verminderung der Risikofaktoren wird die Behandlung der Herzbeschwerden wirksam unterstützt.
Eine Isoket 5 Tablette enthält 218 mg verwertbare Kohlenhydrate. Eine Tablette Isoket Retard 20, 40 oder 60 enthält 151, 157 oder 242 mg verwertbare Kohlenhydrate. Eine Isoket Retard Kapsel enthält 300 mg verwertbare Kohlenhydrate. Isoket Spray enthält keine verwertbaren Kohlenhydrate.
Isoket darf nicht angewendet werden bei:
Wenn Sie als Patient mit einer Herzerkrankung (z.B. Angina pectoris) unter Behandlung mit Isoket stehen, dürfen Sie keinesfalls Phosphodiesterasehemmer (wie z.B. Viagra®, Cialis® oder Levitra®) einnehmen. Die gleichzeitige Anwendung von Phosphodiesterasehemmer kann eine gefährliche Verstärkung der Wirkung von Isoket in Form von Blutdruckabfall und Kreislaufzusammenbruch verursachen.
Sollten Sie aber Viagra®, Cialis® oder Levitra® dennoch eingenommen haben, ist die Anwendung von Isoket innerhalb von 24 Stunden nach Einnahme von Viagra®, Cialis® oder Levitra® strikt verboten. Dies gilt auch im Falle von plötzlich auftretenden Herzbeschwerden. Wenden Sie sich in dieser Notfallsituation an einen Arzt oder eine Ärztin und informieren Sie ihn bzw. sie über die Einnahme von Viagra® oder andere Arzneimittel dieser Gruppe (z.B. Cialis®, Levitra®).
Aufgrund der möglichen Blutdrucksenkung kann Isoket auch bei bestimmungsgemässem Gebrauch das Reaktionsvermögen so weit verändern, dass die Fähigkeit zur aktiven Teilnahme am Strassenverkehr oder zur Bedienung von Maschinen beeinträchtigt wird. Dies gilt in verstärktem Masse im Zusammenwirken mit Alkohol.
Unter bestimmten Bedingungen (Herzinfarkt, eingeschränkte Linksherzfunktion, Neigung zu Kreislaufregulationsstörungen durch niedrigen Blutdruck, Herzklappenfehler, Krankheiten, die mit einer Erhöhung des Hirndrucks einhergehen, Blutdruckabfall) darf Isoket nur unter Aufsicht eines Arztes oder einer Ärztin angewendet werden.
Die gleichzeitige Einnahme von anderen Arzneimitteln mit Blutrucksenkenden Eigenschaften wie z.B. Beta-Rezeptorenblockern, Diuretika, Kalziumantagonisten, Neuroleptika, trizyklischen Antidepressiva, MAO-Hemmern und Alkohol kann die blutdrucksenkende Wirkung von Isoket verstärken.
Falls Sie Sapropterine (Kuvan®) gleichzeitig einnehmen, sollten Sie Ihren Arzt (bzw. Ihre Ärztin) darüber informieren, da es die blutdrucksenkende Wirkung von Isoket verstärken kann.
Patienten unter einer Erhaltungsbehandlung mit Isosorbiddinitrat müssen darüber informiert werden, dass sie keine Produkte anwenden dürfen, die Phosphodiesterasehemmer enthalten (z.B. Sildenafil, Tadalafil, Vardenafil). Die Behandlung mit ISDN darf nicht unterbrochen werden, um Präparate auf Basis von Phosphodiesterasehemmern (Sildenafil, Tadalafil, Vardenafil) einzunehmen, weil sich dabei das Risiko für einen Angina-pectoris-Anfall erhöhen kann.
Eine Akutbehandlung mit ISDN (Tabletten 5 mg oder Mundspray) darf nicht bei Patienten erfolgen, die vor kurzem Phosphodiesterasehemmer (z.B. Sildenafil, Tadalafil, Vardenafil) eingenommen haben. Patienten, die eine Akutbehandlung mit ISDN erhalten, sind darauf hinzuweisen, dass sie keine Produkte auf Basis von Phosphodiesterasehemmern (Sildenafil, Vardenafil, Tadalafil) einnehmen dürfen.
Die blutdrucksenkende Wirkung von Isoket wird dadurch verstärkt und kann zu lebensbedrohlichen Komplikationen führen.
Falls dennoch Phosphodiesterasehemmer eingenommen wurden, ist die Anwendung von Isoket innerhalb von 24 h nach deren Einnahme kontraindiziert.
Isoket kann bei gleichzeitiger Verabreichung die Blutkonzentrationen von Dihydroergotamin erhöhen und damit auch dessen Wirkung verstärken.
Die Tabletten sowie die Retardtabletten und die Retardkapseln enthalten Laktose und dürfen daher von Patienten mit seltenen erblichen Störungen wie Galaktoseintoleranz, Lapp-Laktase-Mangel oder Glukose-Galaktose-Malabsorption nicht eingenommen werden.
Die Retardkapseln enthalten Saccharose und dürfen daher von Patienten mit seltenen erblichen Störungen wie Fruktoseintoleranz, Glukose-Galaktose-Malabsorption oder Saccharase-Isomaltase-Mangel nicht eingenommen werden.
Isoket Mundspray enthält mit weniger als 100 mg pro Dosis geringe Mengen an Ethanol (Alkohol).
Bei versehentlichem grossflächigem Hautkontakt mit dem Inhalt des Isoket-Spraybehälters sollte die kontaminierte Hautfläche sofort gereinigt werden, um schwerwiegende Nebenwirkungen durch Absorption des Wirkstoffs über die Haut zu vermeiden.
Im Falle einer versehentlichen Überdosierung kontaktieren Sie umgehend einen Arzt oder eine Ärztin. Erste Anzeichen einer Überdosierung sind: Blutdruckabfall, Blässe, Schwitzen, schwacher Puls, schneller Herzschlag, Schwindel, Kopfschmerzen, Schwäche, Übelkeit, Erbrechen, Durchfall.
Eine Toleranzentwicklung (Abnahme der Wirksamkeit) sowie eine Kreuztoleranz gegenüber anderen Nitratverbindungen (schwächere Wirkung nach früherer Behandlung mit einem anderen Nitrat) wurde beschrieben. Um eine Wirkungsminderung bzw. einen Wirkungsverlust zu vermeiden, sollten kontinuierlich hohe Dosierungen vermieden werden.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbst gekaufte!) einnehmen oder äusserlich anwenden.
Dieses Arzneimittel soll während der Schwangerschaft oder Stillzeit nur auf ausdrückliche Anordnung des Arztes oder der Ärztin angewendet werden.
Die Dosierung ist weitgehend vom Krankheitsstadium und dem individuellen Nitratbedarf abhängig.

Im Anfall und zur Vorbeugung unmittelbar vor Ereignissen, die einen Anfall verursachen können, lässt man 1 Tablette unter der Zunge oder in der Wangentasche zergehen.
Es ist zu beachten, dass bei kontinuierlicher Behandlung (schon bei 3× tgl. Verabreichung) eine Toleranzentwicklung beobachtet wurde. Um die volle Wirksamkeit zu erhalten, ist bei mehrfach täglicher Einnahme darauf zu achten, dass ein therapiefreies Intervall von mindestens 8 Stunden pro Tag eingehalten wird.
Zur Teilung einer Tablette legt man sie mit der Bruchrille nach oben (Wölbung nach unten) auf eine feste Unterlage und halbiert sie durch leichten Druck mit dem Daumen.
Vor der ersten Anwendung muss das Sprühventil mehrmals bis zum Auftreten eines gleichmässigen Sprühnebels betätigt (angepumpt) werden. Danach ist der Spray einsatzbereit.
Bei der Anwendung ist der Behälter senkrecht mit dem Sprühkopf nach oben zu halten. Die Spraylösung wird dann durch Betätigen des Sprühventils in die Mundhöhle gesprüht. Hierbei kann es zu einem leichten Brennen auf der Zunge kommen.
Soweit nicht anders verordnet, werden im Anfall oder unmittelbar vor körperlichen bzw. seelischen Belastungen, die einen Anfall verursachen können, 1?3 Sprühstösse in Abständen von etwa 30 Sekunden in den Mund gesprüht. Der Sprühstoss darf nicht inhaliert (eingeatmet) werden.
Ist innerhalb von 5 Minuten kein Ansprechen festzustellen, kann ein zusätzlicher Sprühstoss verabreicht werden. Eine höhere Dosierung ist auf Anweisung des Arztes bzw. der Ärztin möglich.
Die Etikette der Sprayflasche trägt am unteren Rand eine Pfeilmarkierung. Sobald der Flüssigkeitsspiegel in der Flasche bis auf die Höhe der Pfeilmarkierung abgesunken ist, sollte aus Sicherheitsgründen eine neue Packung Isoket Spray besorgt werden. Die weitere Benutzung des angebrochenen Sprays ist bedenkenlos möglich, solange auch bei leichter Neigung der Sprayflasche das Steigrohr der Pumpe in die Flüssigkeit eintaucht.
Wurde der Spray länger als einen Tag seit der letzten Anwendung nicht benutzt, muss, um eine vollständige Dosierung zu gewährleisten, der erste Sprühstoss verworfen (in die Luft abgegeben) werden.
Unbedenklichkeit und Wirksamkeit von Isosorbidnitrat sind bei Kindern und Jugendlichen (<18 Jahre) noch nicht hinreichend belegt.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Bei empfindlichen Patienten und Patientinnen kann bei Behandlungsbeginn sehr häufig der sog. «Nitratkopfschmerz» auftreten, der erfahrungsgemäss meistens nach einigen Tagen bei weiterer Einnahme wieder abklingt.
Folgende häufige Nebenwirkungen wurden beobachtet:
Abfall des Blutdrucks mit Schwindel und Schwächegefühl, orthostatische Hypotension (lagebedingter Blutdruckabfall),
Schläfrigkeit, Übelkeit und Erbrechen, eine Erhöhung der Pulsfrequenz und Brennen am Verabreichungsort (Zunge beim Mundspray).
Folgende gelegentliche Nebenwirkungen können auftreten:
Verschlechterung der Angina pectoris, Kreislaufkollaps, Übelkeit, Erbrechen, allergische Hautreaktionen (z.B. Ausschlag), Gesichtsrötung.
Sehr selten wurden Sodbrennen, Angioödem (Schwellungen der Haut und Schleimhaut), Stevens-Johnson-Syndrom (fieberhafte Allgemeinerkrankung mit Entzündung der Halbschleimhäute an Mund, Nase, Geschlechtsorganen u. After) und Brennen am Verabreichungsort (Zunge, bei Mundspray) beobachtet.
Bei organischen Nitraten wurden schwerwiegende hypotone Reaktionen berichtet, mit Übelkeit, Erbrechen, Rastlosigkeit, Blässe und Schweissausbrüchen.
Unter der Behandlung mit Isoket kann es vorübergehend zu erniedrigten Sauerstoffgehalt im Blut kommen.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Bei Raumtemperatur (15-25 °C) und ausserhalb der Reichweite von Kindern aufbewahren.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
