Temodal enthält den Wirkstoff Temozolomid, welcher das Wachstum von Tumorzellen hemmt, und wird angewendet:
Die Behandlung mit Temodal darf nur auf Verschreibung des Arztes bzw. der Ärztin hin erfolgen und muss regelmässig überwacht werden.
Temodal darf nicht angewendet werden bei Überempfindlichkeit gegenüber Temozolomid oder gegenüber einem anderen Bestandteil des Arzneimittels oder wenn jemals eine Überempfindlichkeitsreaktion gegenüber Dacarbazin, einem ähnlichen Wirkstoff zur Behandlung von Tumoren, aufgetreten ist oder wenn bestimmte Blutzellen stark vermindert sind.
Bei einer schweren Nieren- oder schweren Lebererkrankung darf Temodal ebenfalls nicht angewendet werden.
Temodal darf während Schwangerschaft und Stillzeit nicht eingenommen werden.
Temodal ist ein stark wirksames Arzneimittel. Befolgen Sie bitte genau die Anweisungen Ihres Arztes oder Ihrer Ärztin.
Es werden häufig Blutuntersuchungen durchgeführt, um Ihren Zustand zu überwachen.
Ihr Arzt bzw. Ihre Ärztin wird Sie hinsichtlich des Auftretens einer bestimmten Form von Lungenentzündung (mit dem Erreger Pneumocystis carinii) regelmässig kontrollieren. Falls Sie Temodal in Kombination mit einer Strahlenbehandlung einnehmen, wird er bzw. sie Ihnen ein Arzneimittel zur Vorbeugung dieser Lungenentzündung verschreiben.
Ihr Arzt bzw. Ihre Ärztin wird Sie ebenfalls hinsichtlich des Auftretens einer Leberschädigung überwachen.
Temodal enthält Laktose (Milchzucker). Wenn Sie an einer seltenen erblichen Störung des Zuckerstoffwechsels wie Galaktose-Intoleranz, Laktasemangel oder Glukose-Galaktose-Malabsorption leiden, sollten Sie dieses Arzneimittel nicht einnehmen.
Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie Valproinsäure einnehmen (Arzneimittel zur Behandlung von Epilepsie oder psychischen Erkrankungen), da diese die Ausscheidung von Temozolomid vermindern und damit die Wirkung verstärken kann.
Temodal kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen, z.B. infolge Ermüdung, Schläfrigkeit, Übelkeit und Erbrechen.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!
Temodal kann zu Schädigungen des ungeborenen Kindes führen. Wenn Sie schwanger sind oder es werden möchten, dürfen Sie Temodal unter keinen Umständen einnehmen. Bei Frauen im gebärfähigen Alter muss vor der Therapie eine Schwangerschaft mit Sicherheit ausgeschlossen und während, sowie bis 6 Monate nach der Therapie, eine wirksame Methode der Empfängnisverhütung angewendet werden.
Temodal darf auch nicht während der Stillzeit angewendet werden.
Männliche Patienten, die mit Temodal behandelt werden, dürfen während und bis 6 Monate nach der Behandlung kein Kind zeugen.
Ihr Arzt bzw. Ihre Ärztin bestimmt die korrekte Dosis Temodal, die Sie einnehmen sollen, ausgehend von Ihrer Körperoberfläche, Ihren Blutwerten (weisse Blutkörperchen und Blutplättchen) und abhängig davon, ob Sie chemotherapeutisch vorbehandelt sind. Eventuell erhalten Sie vor und/oder nach der Einnahme von Temodal noch ein anderes Arzneimittel, um Übelkeit und Erbrechen zu vermeiden oder unter Kontrolle zu bringen.
Nehmen Sie Temodal nüchtern ein, z.B. mindestens eine Stunde vor dem Frühstück. Die Kapseln dürfen nicht geöffnet oder zerkaut werden; ein Kontakt des pulvrigen Kapselinhalts mit der Haut oder den Schleimhäuten und das Einnehmen des Pulvers sind zu vermeiden. Die Kapsel/n ist/sind mit einem Glas Wasser ganz zu schlucken.
Ein Therapiezyklus mit Temodal umfasst 28 Tage. In den ersten fünf Tagen («Einnahmetage») nehmen Sie die Kapseln einmal täglich ein; darauf folgt ein 23-tägiges Intervall ohne Einnahme von Temodal; dies ergibt einen insgesamt 28-tägigen Therapiezyklus. Nach Tag 28 beginnt der nächste Zyklus, in dem Sie Temodal erneut einmal täglich über die Dauer von fünf Tagen einnehmen; darauf folgt ein 23-tägiges Intervall ohne Temodal. Vor jedem neuen Therapiezyklus wird Ihr Blut untersucht, um festzustellen, ob die Temodaldosis angepasst werden muss.
Zu Beginn nehmen Sie die Temodal Kapseln einmal täglich ununterbrochen während 42-49 Tagen ein. Während dieser Phase erhalten Sie auch die Strahlentherapie. Dann folgt eine Pause von 4 Wochen ohne Behandlung. Anschliessend nehmen Sie die Temodal Kapseln wie oben unter «alleinige Therapie mit Temodal» beschrieben ein (28-tägige Therapiezyklen mit Einnahme von Temodal während fünf Tagen und 23-tägigem Intervall ohne Temodal). Während der Behandlung wird der Arzt bzw. die Ärztin regelmässig Ihr Blut untersuchen, um festzustellen, ob die Temodal-Dosis angepasst oder die Behandlung eventuell unterbrochen werden muss.
Temodal ist als Kapseln in verschiedenen Dosierungsstärken erhältlich (Angabe in mg auf der Faltschachtel und der Etikette). Jede Dosierungsstärke ist mit einem unterschiedlichen Farbbalken versehen. Je nach der Temodal-Dosis, die Ihr Arzt bzw. Ihre Ärztin verordnet, müssen Sie eventuell mehrere Kapseln der gleichen oder unterschiedlicher Stärke an jedem Einnahmetag des Therapiezyklus einnehmen.
Wenn Sie an einer leichten bis mässiggradigen Beeinträchtigung der Nierenfunktion leiden oder älter als 70 Jahre sind, wird Sie Ihr Arzt bzw. Ihre Ärztin besonders engmaschig kontrollieren.
Temodal als alleinige Therapie kann auch bei Kindern/Jugendlichen im Alter von 3 bis 18 Jahren angewendet werden. Auch hier wird der Arzt bzw. die Ärztin die Dosis individuell festlegen und die Behandlung genau überwachen.
Bitte halten Sie sich unbedingt an die vom Arzt bzw. von der Ärztin festgelegte Dosierung. Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.
Temodal darf nur entsprechend der Verschreibung Ihres Arztes bzw. Ihrer Ärztin eingenommen werden. Bei einer versehentlichen Einnahme einer grösseren Anzahl an Temodal Kapseln als vorgeschrieben informieren Sie bitte umgehend Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin.
Nehmen Sie die vergessene Dosis so bald wie möglich am selben Tag ein. Ist bereits ein ganzer Tag vergangen, besprechen Sie dies mit Ihrem Arzt bzw. Ihrer Ärztin. Nehmen Sie beim nächsten Mal keine doppelte Dosis ein, es sei denn, Ihr Arzt bzw. Ihre Ärztin fordert Sie ausdrücklich dazu auf.
Folgende Nebenwirkungen können bei der Einnahme von Temodal auftreten:
Sehr häufig sind Übelkeit und Erbrechen in den Stunden nach der Einnahme. Ihr Arzt oder Ihre Ärztin kann als Gegenmassnahme ein Arzneimittel verschreiben, das diese Begleiterscheinungen bekämpft.
Unter der Behandlung mit Temodal kommt es auch sehr häufig zu einer Abnahme der weissen und roten Blutkörperchen und der Blutplättchen. Dies kann zu verstärkter Bildung blauer Flecken oder Blutungen, Blutarmut, Fieber und/oder Auftreten von Infektionen führen (durch Pilze, Viren oder andere Erreger; z.B. Lungenentzündung, Infektionen der oberen Luftwege oder im Mund, Mittelohrenentzündung, Wundinfektionen). Ihr Arzt bzw. Ihre Ärztin wird Ihr Blut regelmässig auf etwaige Veränderungen untersuchen und erforderlichenfalls eine besondere Therapie anordnen. In einigen Fällen wird die Temodal-Dosierung vermindert oder unterbrochen. Bei Fieber oder falls Hautausschläge, Blutungen, kleine blaue Flecken auf der Haut oder schwarzer Stuhl auftritt, müssen Sie dies Ihrem Arzt bzw. Ihrer Ärztin sofort mitteilen.
In Verbindung mit einer Strahlentherapie traten die folgenden Nebenwirkungen sehr häufig auf: ungewöhnlicher Haarausfall, Müdigkeit, Übelkeit, Erbrechen, Appetitlosigkeit, Kopfschmerzen, Verstopfung, Hautausschlag, Krämpfe, Durchfall, Entzündungen im Mund, verschwommenes Sehen, verminderte Anzahl von Blutplättchen.
Ebenfalls häufig berichtet wurden: tiefe Kaliumspiegel im Blut, erhöhte Leberenzyme, verlängerte oder wiederholte Krampfanfälle ohne Erholung zwischen den Attacken, Erregtheit, Teilnahmslosigkeit, Mittelohrinfektion, Hirnblutung, erhöhte Sonnenempfindlichkeit der Haut, Impotenz, Hitzegefühl, Verfärbung der Zunge, gestörter Geruchssinn und Durst.
Die folgenden unerwünschten Wirkungen wurden während der Anwendung von Temodal berichtet:
Verminderte Anzahl weisser oder roter Blutkörperchen oder Blutplättchen, Appetitlosigkeit, Kopfschmerzen, Krämpfe, Übelkeit, Erbrechen, Verstopfung, ungewöhnlicher Haarverlust, Ermüdung
Pilzinfektionen im Mund (Soor), Herpes und Gürtelrose, Halsschmerzen, Wundinfektionen, Mittelohrinfektion, Blutarmut, mit Fieber verbundene Abnahme der weissen Blutkörperchen, Blutung, Gewichtsverlust, tiefe Kaliumspiegel im Blut, Angst, Depression, emotionale Labilität, Schlaflosigkeit, Erregung, Teilnahmslosigkeit, Benommenheit, Sprachstörung, Schwierigkeiten Wörter zu sprechen oder zu verstehen, Gleichgewichtsstörungen, Konzentrationsschwierigkeiten, Gedächtnisstörungen, Verwirrung, Bewusstseinstrübung, Taubheitsgefühl, Schläfrigkeit, Schwindelgefühl, Kribbeln oder Taubheit der Hände oder Füsse, Störung der Geruchs- oder Geschmacksempfindung, Zittern, Lähmung einer Seite des Körpers, Taubheit oder Schwäche der Arme oder Beine, verlängerte oder wiederholte Krampfanfälle ohne zwischenzeitliche Erholung, Hirnblutung, verschwommenes Sehen, Doppeltsehen, Gesichtsfeldausfälle, eingeschränktes Hörvermögen, andauerndes Geräusch in den Ohren (Tinnitus), Gewebeschwellung (Ödeme), Blutung, Blutgerinnsel in den Beinen, Erröten, Kurzatmigkeit, Husten, Durchfall, Magenschmerzen, Verdauungsstörung, Schluckbeschwerden, Bauchschmerzen, Entzündungen im Mund, trockener Mund, Verfärbung der Zunge, erhöhte Leberenzyme, Hautausschlag, Juckreiz, rote punktförmige Flecken unter der Haut, trockene Haut, Gelenksschmerzen, Muskelschmerzen, Muskelschwäche, Skelettschmerzen, Harninkontinenz, häufiges Wasserlösen, Impotenz, Fieber, Schwäche, Schmerzen, Schüttelfrost, allgemeines Unwohlsein, Verletzung durch Bestrahlung, erhöhte Sonnenempfindlichkeit der Haut.
Lungenentzündung, Infektionen der oberen Atemwege, Nasennebenhöhlenentzündung, Bronchitis, grippeartige Symptome, Cytomegalovirus (CMV) Infektionen (diese verlaufen normalerweise ohne Symptome, können jedoch mit Ermüdung und Schmerzen in Gelenken und Muskeln einhergehen) und Hepatitis B Infektionen (verursachen eine Entzündung der Leber, die sich durch leichtes Fieber, Krankheitsgefühl, Ermüdung, Appetitverlust, Gelenk- und/oder Abdominalschmerzen und Gelbfärbung des Augenweiss, der Haut und der Zunge äussern kann), welche in einigen Fällen tödlich sein können, verminderte Anzahl der Blutzellen, verminderte Anzahl von weissen Blutkörperchen, Symptome typisch für eine Kortisonbehandlung (Gewichtszunahme in der Körpermitte, rundes Gesicht), Erkrankung, welche charakterisiert ist durch übermassigen Durst und der Ausscheidung von grossen Mengen Urin, hoher Zuckerspiegel im Blut, Gewichtszunahme, Halluzinationen, Verhaltensstörungen, Gedächtnisverlust, Beeinträchtigung der Denkleistung, Koordinationsschwierigkeiten, ungewöhnliche Bewegungen, anormaler Gang, erhöhte Empfindlichkeit der Haut, Lähmung einer Körperhälfte, Störung der Sinnesempfindung, Augenschmerzen, trockene Augen, verminderte Sehschärfe, Ohrenschmerzen, Taubheit, Drehschwindel, schneller oder unregelmässiger Herzschlag, hoher Blutdruck, Verschluss der Lungenarterie (Lungenembolie), verstopfte Nase, Blähungen, Stuhlinkontinenz, infektiöser Durchfall, Hämorrhoiden, Zahnerkrankungen, anormale Pigmentation, Hautabschuppung, Hautrötung, verstärktes Schwitzen, Rückenschmerzen, muskuläre Erkrankung, die zu Muskelschwäche führt, schmerzhaftes Wasserlassen, Ausbleiben der Regelblutung, Brustschmerzen, ungewöhnlich starke oder verlängerte Regelblutung, Scheidenblutung, Entzündung der Scheide, verschlechterter Allgemeinzustand, Muskelsteifheit, Gesichtsschwellung.
Verminderte Abwehrkraft gegen Infektionen.
Entzündung der Lunge, Lungenvernarbung, bösartige Veränderung der Blutzellen (einschliesslich Leukämie), Blutarmut mit reduzierter Anzahl an Blutkörperchen und Blutplättchen, allergische Reaktionen (einschliesslich plötzliche lebensbedrohende allergische Reaktionen), Schwellung des Gesichts, Lippen, Mund, Zunge oder Rachen, Leberschäden (einschliesslich tödliches Leberversagen), Leberentzündung und Probleme mit dem Gallenfluss (Cholestase), Nesselsucht, Hautausschlag, schwere Hautreaktionen.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Temodal darf keinesfalls in Reichweite von Kindern oder nicht informierten Personen aufbewahrt werden.
Zwischen 2 °C und 30 °C lagern. Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
Die Kapseln nicht öffnen. Sollte eine Kapsel beschädigt sein, vermeiden Sie den Kontakt des Pulverinhalts mit Haut, Augen oder Nase. Das Pulver nicht einatmen. Sollten Sie versehentlich etwas von dem Pulver auf die Haut oder in die Augen oder in die Nase bekommen, spülen Sie die betroffene Stelle gründlich mit Wasser und informieren Sie Ihren Arzt bzw. Ihre Ärztin.
Bitte bringen Sie nicht verwendete Kapseln oder Kapseln, deren Verfalldatum abgelaufen ist, in die Apotheke zur Entsorgung zurück.

Temodal Kapseln enthalten 5 mg (grün-weisse), 20 mg (gelb-weisse), 100 mg (rosa-weisse), 140 mg (blau-weisse), 180 mg (orange-weisse) oder 250 mg (weisse Kapseln) des Wirkstoffs Temozolomid sowie Hilfsstoffe. Die Kapseln zu 5 mg und zu 140 mg enthalten zusätzlich den Farbstoff Indigotin (E132).
