+-Was ist Amiodar und wann wird es angewendet?

Auf Verschreibung des Arztes oder der Ärztin.

Amiodar ist ein Medikament, das verschiedene gestörte Funktionen des Herzens wieder normalisiert. Es reguliert unregelmässige, zu rasche oder zu starke Herzschläge, ohne die Pumpkraft des Herzens zu beeinträchtigen.

Ihr Arzt bzw. Ihre Ärztin verschreibt Ihnen Amiodar zur Behandlung von unregelmässigem oder zu schnellem Herzschlagrhythmus.

+-Was sollte dazu beachtet werden?

Personen, die an Störungen des Herzens leiden, reagieren besonders empfindlich auf Stress, Nervosität und Erregung; dasselbe gilt auch für das Rauchen und die Einnahme von koffeinhaltigen Getränken.

Versuchen Sie deshalb, diese herzbelastenden Einflüsse nach Möglichkeit auszuschalten.

+-Wann darf Amiodar nicht angewendet werden?

Amiodar darf in folgenden Fällen nicht eingenommen werden:

  • bei Überempfindlichkeitsreaktionen (Allergie) auf Jod oder auf den Wirkstoff Amiodaron;
  • bei zu langsamem Herzschlagrhythmus, kardialen Durchblutungsstörungen;
  • bei Störungen der Schilddrüsenfunktion, Kropf;
  • vermindertem Kaliumgehalt im Blut;
  • Behandlung mit einem Mono-Amino-Oxydase-Hemmer (Medikament zur Behandlung von Depressionen).
+-Wann ist bei der Einnahme von Amiodar Vorsicht geboten?

Falls Sie bereits an gestörten Schilddrüsenfunktionen gelitten haben oder wenn diese Art von Erkrankungen in Ihrer Familie vorkommt, informieren Sie bitte Ihren Arzt bzw. Ihre Ärztin. Dieser wird dann eine regelmässige medizinische Kontrolle durchführen.

Wegen möglicher Hautausschläge ist es empfehlenswert, während der Behandlung mit Amiodar direkte Sonnenbestrahlung zu vermeiden oder sich andernfalls entsprechend davor zu schützen, z.B. mit einer Sonnencreme mit hohem Schutzfaktor.

Verschiedene Medikamente können die Intensität der unerwünschten Nebenwirkungen von Amiodar verstärken oder deren Wirkung kann durch die Einnahme von Amiodar verändert werden.

Dabei handelt es sich insbesondere um Medikamente zur Behandlung von Herzkrankheiten und/oder erhöhtem Blutdruck, mit einer Hypokaliämie assoziierte Medikamente wie bestimmte Abführmittel (Laxantien) und Diuretika, Antibiotika, Antiallergika (Antihistaminika), Schmerz- oder Epilepsiemittel, auch bestimmte Beruhigungsmittel und Antidepressiva, pressorische Amine, Antinausea, abschwellende Mittel, Bronchodilatatoren, Medikamente, die die Blutgerinnung hemmen (Antikoagulantien), Medikamente, welche im Rahmen oder nach einer Organtransplantation eingenommen werden (Ciclosporin), oder Medikamente, die den Blutcholesterinspiegel senken (Statine). Aus diesem Grund müssen Sie Ihren Arzt bzw. Ihre Ärztin unbedingt darüber informieren, welche Medikamente Sie bereits einnehmen.

Vor und während der Behandlung führt Ihr Arzt bzw. Ihre Ärztin möglicherweise Tests zur Beurteilung Ihres Gesundheitszustands wie Bluttests (z.B. eine regelmässige Kontrolle der Schilddrüsenfunktion), eine Röntgenaufnahme der Lungen, ein EKG und eine augenärztliche Kontrolle durch.

Bei einem geplanten chirurgischen Eingriff ist es wichtig, dass Sie Ihren Arzt bzw. Ihre Ärztin zuvor über die Einnahme von Amiodar informieren.

Amiodar soll nicht zusammen mit Grapefruit eingenommen werden, da dies eine verstärkte Wirkung zur Folge haben kann.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

  • an anderen Krankheiten leiden,
  • Allergien haben oder
  • andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!

Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen.

+-Darf Amiodar während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Da der Wirkstoff von Amiodar die Plazenta passiert und in die Muttermilch übergeht, darf Amiodar während einer Schwangerschaft oder in der Stillzeit nicht eingenommen werden, ausser wenn der Arzt bzw. die Ärztin es für absolut unerlässlich hält.

+-Wie verwenden Sie Amiodar?

Ihr Arzt bzw. Ihre Ärztin wird für Sie ein genaues Dosierungsschema festsetzen. Die nachfolgenden Angaben betrachten Sie deshalb bitte nur als Hinweis.

Während 8 bis 10 Tagen werden täglich 3 Tabletten zu 200 mg eingenommen.

Bei Langzeitbehandlung kann die Dosis zwischen 100 und 400 mg pro Tag schwanken.

Amiodar kann entweder alle 2 Tage eingenommen werden oder an 5 aufeinanderfolgenden Tagen mit anschliessender Pause von 2 Tagen.

Die Einnahmepause ist zwar nicht nötig, kann aber wegen der langen Wirkungsdauer des Medikamentes gemacht werden, ohne dass ein Wirkungsverlust eintritt.

Bei älteren Patienten kann eine geringere Tagesdosis verordnet werden.

Die festgesetzte Tagesdosis wird vorzugsweise während den Mahlzeiten mit etwas Flüssigkeit eingenommen.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Amiodar haben?

Folgende Nebenwirkungen können bei der Einnahme von Amiodar auftreten: Störungen der Schilddrüse, Atembeschwerden, Sehstörungen, Hautreaktionen, Verdauungsstörungen, neurologische Störungen, Arrythmien, neuromuskuläre Störungen.

Machen Sie Ihren Arzt bzw. Ihre Ärztin sofort darauf aufmerksam, wenn Sie unter Amiodar Atemnot, Husten, Fieber, Schmerzen im Lungenbereich, ausgeprägte Müdigkeit, Gewichtsabnahme oder -zunahme, leichtes Zittern in den Gliedern oder Wiederauftreten von zu schnellen oder unregelmässigen Herzschlägen oder Verdauungsstörungen (Übelkeit, Erbrechen, Metallgeschmack) feststellen. Auch eine aussergewöhnliche Senkung des Pulses ist dem Arzt bzw. der Ärztin zu melden. Alle diese Symptome können auf eine Störung der Schilddrüse hinweisen oder sonstwie mit der Amiodar-Behandlung zusammenhängen.

Die häufig beobachteten Störungen der Schilddrüsenfunktion traten in Form einer Über- oder Unterfunktion auf.

Sehr häufig kann es bei Einnahme von Amiodar zu Sehstörungen kommen (farbige Kreise um Gegenstände oder Nebelsehen, Gesichtsfeldausfälle oder Lichtempfindlichkeit), die nach Behandlungsabbruch völlig verschwinden; in selteneren Fällen kann die visuelle Wahrnehmung herabgesetzt sein.

Sehr häufig können unter Sonnenbestrahlung Hautreaktionen auftreten; in selteneren Fällen kann es zu einer schieferfarbigen Verfärbung der unbedeckten Hautpartien oder zu Haarausfall kommen, sehr selten zu Hautausschlägen, manchmal mit ernstem Verlauf und möglichem tödlichen Ausgang. Falls Sie Anzeichen eines fortschreitenden Hautausschlags beobachten, der mit Blasenbildung (Bläschen) und/oder Schleimhautbeteiligung einhergeht, brechen Sie die Einnahme von Amiodar ab und nehmen Sie umgehend Kontakt mit einem Arzt auf.

Sehr häufig wurde auch über folgende Nebenwirkungen berichtet: vorübergehende Verdauungsstörungen hauptsächlich zu Behandlungsbeginn; des Weiteren Leberstörungen, die im Allgemeinen mässig und vorübergehend sind; häufig kam es zu Herzrhythmusstörungen, neurologischen Störungen, Tremor, Alpträumen, Schlafstörungen, Kopfschmerzen; in seltenen Fällen zu Asthma oder akuter Ateminsuffizienz; sehr selten traten Gliederschmerzen, Muskelschwäche, Koordinationsstörungen, Anomalien des Blutbildes, Schmerzen in den Hoden, Impotenz, Störungen des Immunsystems wie Ödeme und anaphylaktischer Schock, Gefässentzündung, Bauchspeicheldrüsenentzündung oder eine schwere Leberentzündung auf, die eine regelmässige Leberkontrolle rechtfertigt. In diesem Fall müssen Sie die Empfehlungen Ihres Arztes bzw. Ihrer Ärztin befolgen.

Unter Amiodar kann eine unregelmässige Herzschlagfolge neu auftreten oder verstärkt werden.

Bei der Einnahme von Amiodar kann es zu Appetitverlust, Mundtrockenheit, Verstopfung, Libidoverlust, Riechstörungen und psychiatrischen Störungen wie Halluzinationen und geistige Verwirrtheit kommen.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

In der Originalverpackung, bei Raumtemperatur (15?25 °C), vor Licht geschützt und ausser Reichweite von Kindern lagern.

Es darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
