+-Was ist Bisoprolol-Mepha und wann wird es angewendet?

Bisoprolol-Mepha enthält den Wirkstoff Bisoprolol aus der Klasse der ?₁-selektiven Betablocker. Bisoprolol-Mepha senkt den erhöhten Blutdruck (Hypertonie), schützt das Herz bei koronarer Herzkrankheit (Angina pectoris) und bei übersteigerter Herztätigkeit aufgrund überschiessender antreibender Reize (z.B. vom vegetativen Nervensystem).

Bisoprolol-Mepha wird zur Behandlung der stabilen chronischen Herzschwäche eingesetzt. Die chronische Herzschwäche kann sich z.B. durch Atemnot, Beklemmungsgefühl, Herzstechen zeigen. Bisoprolol-Mepha verhindert das Ansprechen des Herzens auf eine übermässige Stimulation durch Adrenalin.

Das Arzneimittel darf nur auf Verschreibung des Arztes oder der Ärztin angewendet werden.

+-Was sollte dazu beachtet werden?

Sie können den Behandlungserfolg in vielen Fällen deutlich unterstützen, wenn Sie Ihre Bewegungs- und Essgewohnheiten nach den Empfehlungen Ihres Arztes ausrichten.

Das Auftreten von Nebenwirkungen (z.B. stark verlangsamter Herzschlag, zu niedriger Blutdruck oder andere Krankheitszeichen, wie zunehmende Atemnot, die auf eine Verschlechterung der Herzschwäche hinweisen) kann zum Abbruch der Behandlung mit Bisoprolol-Mepha führen.

Die Behandlung der chronischen Herzschwäche mit Bisoprolol-Mepha darf nicht abrupt gestoppt werden, da dies zu einer vorübergehenden Verschlechterung der Herzschwäche führen kann. Auch die Behandlung von Angina pectoris darf nicht abrupt abgebrochen werden, da es sonst zu einer akuten Verschlechterung des Zustandes kommen kann. Sollte ein Abbruch der Therapie notwendig sein, muss die Dosis schrittweise reduziert werden (z.B. Halbierung der Dosis über eine Woche und erneute Halbierung in der darauffolgenden Woche, sofern notwendig). In einem solchen Fall befolgen Sie bitte die genauen Anweisungen Ihres behandelnden Arztes oder Ihrer behandelnden Ärztin.

Patienten, die zuckerkrank sind, sollten regelmässig, speziell zu Behandlungsbeginn, ihren Blutzucker kontrollieren, weil die Wirkung der blutzuckersenkenden Arzneimittel durch Bisoprolol-Mepha verstärkt werden kann.

+-Wann darf Bisoprolol-Mepha nicht angewendet werden?

Bisoprolol-Mepha darf nicht angewendet werden bei bestimmten Herzfunktionsstörungen (z.B. AV-Block 2. oder 3. Grades ohne Herzschrittmacher, Sinusknoten-Syndrom, sinuatrialer Block), bei akuter Herzmuskelschwäche, bei plötzlicher Verschlechterung einer Herzmuskelschwäche, bei stark verlangsamtem Herzschlag vor Therapiebeginn (Puls unter 60 Schlägen/Minute), bei schwerem Asthma, bei schweren Durchblutungsstörungen in den Gliedmassen, bei bestimmten unbehandelten Tumoren des Nebennierenmarks (Phäochromozytom), bei Übersäuerung des Blutes (metabolische Azidose), bei zu niedrigem Blutdruck und bei Schockzuständen (mit tiefem Blutdruck und Kreislaufversagen einhergehend).

Bei bekannter Überempfindlichkeit auf Bisoprolol oder einen Hilfsstoff von Bisoprolol-Mepha soll das Arzneimittel nicht angewendet werden.

+-Wann ist bei der Einnahme von Bisoprolol-Mepha Vorsicht geboten?

Bei einer Störung der Herzfunktion (AV-Block 1. Grades);
bei einer Durchblutungsstörung des Herzens (Prinzmetal-Angina);
bei Durchblutungsstörungen in den Armen und Beinen;
bei Schuppenflechte (Psoriasis);
bei strengem Fasten;
bei schweren Atemwegserkrankungen (mit Verengung der Atemwege, welche Symptome wie Atemnot verursachen können, z.B. COPD);
bei gleichzeitiger Desensibilisierungstherapie wegen Allergie/Heuschnupfen;
bei Diabetikern mit stark schwankenden Blutzuckerwerten. Durch Betablocker kann sich die Wirkung von gleichzeitig verabreichten Arzneimitteln zur Blutzuckersenkung (Antidiabetika) verstärken. Bei der Behandlung mit ?₁-selektiven Rezeptorenblockern (wie z.B. Bisoprolol) ist dieses Risiko geringer als bei anderen Betablockern. Bisoprolol-Mepha kann die Symptome eines stark erniedrigten Blutzuckerspiegels (Hypoglykämie), wie z.B. erhöhter Puls, Schwitzen und Herzklopfen, verschleiern oder vermindern.

Die Symptome einer Schilddrüsenüberfunktion (Thyreotoxikose), u.a. der erhöhte Puls, können durch Bisoprolol-Mepha verdeckt werden.

Vor einer Narkose muss der Narkosearzt über die Behandlung mit Bisoprolol-Mepha informiert werden.

Bei Asthma kann eine Dosisanpassung von bronchienerweiternden Arzneimitteln notwendig werden.

Bei bestimmten Tumoren des Nebennierenmarks (Phäochromozytom) darf Bisoprolol-Mepha erst nach vorheriger Gabe von Alpha-Rezeptoren-Blockern angewendet werden.

Durch individuell auftretende unterschiedliche Reaktionen auf die Blutdrucksenkung kann die Fähigkeit zur aktiven Teilnahme am Strassenverkehr oder zum Bedienen von Maschinen beeinträchtigt werden. Dies gilt in verstärktem Masse bei Behandlungsbeginn, bei einem Wechsel des Arzneimittels sowie im Zusammenwirken mit Alkohol. Gezielte Untersuchungen haben jedoch ergeben, dass eine direkte Beeinträchtigung des Reaktionsvermögens durch den selektiven Betablocker Bisoprolol nicht zu befürchten ist.

Wechselwirkungen mit anderen Arzneimitteln

Zahlreiche andere Arzneimittel und Alkohol können bei gleichzeitiger Einnahme mit Bisoprolol-Mepha die blutdrucksenkende Wirkung verstärken oder abschwächen bzw. unerwünschte Wirkungen auslösen oder verstärken.

Bei gleichzeitiger Anwendung von Bisoprolol-Mepha und gewissen anderen blutdrucksenkenden Arzneimitteln, zentral wirksamen blutdrucksenkenden Arzneimitteln, Kalzium-Antagonisten, Narkosemitteln, Schlaf- und Beruhigungsmitteln, Arzneimitteln zur Behandlung einer Depression wie z.B. Monoaminoxidase-Hemmer, Augentropfen zur lokalen Behandlung des grünen Stars und auch Alkohol, wurde eine verstärkte blutdrucksenkende Wirkung beobachtet.
Bei gleichzeitiger Anwendung von Bisoprolol-Mepha und Arzneimitteln, welche die Wirkung auf den Parasympathikus verstärken (Parasympathomimetika), Herzglykosiden oder Mefloquin-haltigen Malariamitteln, kann sich das Risiko einer Pulsverlangsamung erhöhen.
Bei gleichzeitiger Anwendung von Bisoprolol-Mepha und Arzneimitteln gegen Herzrhythmusstörungen (Antiarrhythmika) kann die dämpfende Wirkung auf das Herz verstärkt sein. Dies ist besonders bei der Anwendung von Verapamil und Diltiazem zu beachten.
Bisoprolol-Mepha kann den blutzuckersenkenden Effekt von oralen Antidiabetika oder Insulin verstärken.
Die blutdrucksenkende Wirkung von Bisoprolol-Mepha kann durch die gleichzeitige Gabe von bestimmten Schmerz- und Rheumamitteln vermindert werden.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbst gekaufte!) einnehmen oder äusserlich anwenden (Externa).

+-Darf Bisoprolol-Mepha während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Während der Schwangerschaft und Stillzeit darf Bisoprolol-Mepha nicht eingenommen werden, ausser auf ausdrückliche Verordnung Ihres Arztes oder Ihrer Ärztin.

+-Wie verwenden Sie Bisoprolol-Mepha?

Allgemeine Dosierungshinweise

Die Behandlung mit Bisoprolol-Mepha wird vom Arzt bzw. der Ärztin individuell dem Patienten angepasst.

Bisoprolol-Mepha-Tabletten sollten einmal täglich morgens nüchtern oder zum Frühstück unzerkaut mit etwas Flüssigkeit eingenommen werden.

Bisoprolol-Mepha ist im Allgemeinen eine Langzeitbehandlung und sollte nicht abrupt beendet werden, da dies zu einer vorübergehenden Verschlechterung des Zustandes des Patienten (speziell bei Patienten mit koronarer Herzkrankheit) führen kann. Die Dosis sollte schrittweise reduziert werden. Die Anwendung und Sicherheit von Bisoprolol-Mepha bei Kindern und Jugendlichen ist bisher nicht geprüft worden. Deshalb wird Bisoprolol-Mepha für die Behandlung von Kindern und Jugendlichen nicht empfohlen. Für ältere Patienten, sowie für Patienten mit leichten bis mittelschweren Nieren- oder Leberfunktionsstörungen ist keine Dosisanpassung notwendig.

Spezielle Dosierungshinweise

Übliche Dosierung zur Behandlung von Hypertonie, Angina pectoris und übersteigerter Herztätigkeit

Die Anfangsdosis ist in der Regel einmal täglich eine Tablette Bisoprolol-Mepha 5 mg. Das reicht in vielen Fällen auch in der Langzeitbehandlung. Ihr Arzt oder Ihre Ärztin kann bei Bedarf die Dosis auf 10 mg (einmal täglich eine Tablette Bisoprolol-Mepha 10 mg) steigern.

Übliche Dosierung zur Behandlung der chronischen Herzschwäche

Üblicherweise wird mit einer niedrigen Dosis begonnen, die bei guter Verträglichkeit langsam und schrittweise erhöht wird:

1. Woche: 1,25 mg (½ Tablette zu 2,5 mg) einmal täglich.
2. Woche: 2,5 mg (1 Tablette zu 2,5 mg) einmal täglich.
3. Woche: 3,75 mg (1½ Tabletten zu 2,5 mg) einmal täglich.
4.?7. Woche: 5 mg (1 Tablette zu 5 mg) einmal täglich.
8.?11. Woche: 7,5 mg (1½ Tabletten zu 5 mg) einmal täglich.
Ab 12. Woche: 10 mg (1 Tablette zu 10 mg) einmal täglich als Erhaltungstherapie.

Die maximal empfohlene Dosis beträgt einmal täglich 10 mg.

Vorgehen bei vergessenen Tabletten

Falls Sie die Einnahme von Bisoprolol-Mepha vergessen haben, so dürfen Sie dies nicht durch die Einnahme der doppelten Menge Tabletten kompensieren. Sie führen die Einnahme wie gewohnt und von Ihrem Arzt oder Ihrer Ärztin verordnet fort.

Vorgehen bei Überdosierung

Bei einer Überdosierung benachrichtigen Sie bitte Ihren Arzt bzw. Ihre Ärztin. Diese können entsprechend der Schwere der Überdosierung über die gegebenenfalls erforderlichen Massnahmen entscheiden.

Die häufigsten Zeichen einer Überdosierung mit Bisoprolol-Mepha sind starker Blutdruckabfall, verlangsamter Herzschlag (Bradykardie), plötzlich auftretende Atemprobleme (Bronchialverkrampfung), plötzlich auftretende Herzprobleme (akute Herzmuskelschwäche), und bei Diabetikern Unterzuckerung (Hypoglykämie). Ihr Arzt oder Ihre Ärztin wird Sie über die weitere Behandlung genau orientieren.

Ändern Sie nicht von sich aus die verschriebene Dosis. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Bisoprolol-Mepha haben?

Sehr häufig (über 10%):

Verlangsamter Puls (bei Patienten mit chronischer Herzschwäche)

Häufig (1-10%):

Erschöpfungszustände (Fatigue), Schwindel, Kopfschmerzen und Schwitzen. Diese Erscheinungen treten bei Patienten mit Bluthochdruck oder Angina pectoris insbesondere zu Behandlungsbeginn auf, sind meist leichter Natur und verschwinden im weiteren Verlauf der Behandlung.

Taubheit und Kältegefühl in den Extremitäten, allgemeine Schwäche (Asthenie) bei Patienten mit chronischer Herzschwäche, verstärkter Blutdruckabfall (speziell bei Patienten mit chronischer Herzschwäche), Magen-Darm-Störungen wie Übelkeit, Erbrechen, Durchfall, Verstopfung, Bauchschmerzen.

Gelegentlich (0,1-1%):

Schlafstörungen, depressive Verstimmung, allgemeine Schwäche (Asthenie) bei Patienten mit Bluthochdruck oder Angina pectoris, verlangsamter Herzschlag (bei Patienten mit Bluthochdruck oder Angina pectoris), Verschlechterung einer Herzschwäche (bei Patienten mit Bluthochdruck oder Angina pectoris). Bei Patienten mit Durchblutungsstörungen in den Armen und Beinen kann es zu einer Verstärkung der Beschwerden kommen. Erhöhter Atemwiderstand bei Patienten mit Asthma und chronischen Atemwegserkrankungen, Muskelschwäche, Muskelkrämpfe.

Selten (0,01-0,1%):

Gesteigerte Traumaktivität, Halluzinationen, Mundtrockenheit, reduzierter Tränenfluss (bei Kontaktlinsenträgern zu beachten), Hörstörungen, allergischer Schnupfen, Veränderung der Leberwerte, Leberentzündung, Gelbsucht, Überempfindlichkeitsreaktionen der Haut wie Jucken, Rötung, Ausschlag, Potenzstörungen.

Sehr selten (unter 0,01%):

Bindehautentzündung des Auges (Konjunktivitis), kreisrunder Haarausfall, Auslösung bzw. Verschlechterung einer Schuppenflechte (Psoriasis).

Häufigkeit unbekannt:

Kreislaufkollaps
