Caprelsa enthält den Wirkstoff Vandetanib und wird zur Behandlung des medullären Schilddrüsenkarzinoms eingesetzt, wenn der Tumor nicht operativ entfernt werden kann oder in anderen Teilen des Körpers Metastasen (Ableger) gebildet hat.
Caprelsa entfaltet seine Wirkung, indem es das Wachstum neuer Blutgefässe in Tumoren (Krebsgeschwüren) verlangsamt. Dadurch wird der Tumor von der Versorgung mit Nährstoffen und Sauerstoff abgeschnitten. Caprelsa kann auch direkt auf Krebszellen wirken oder ihr Wachstum verlangsamen.
Caprelsa darf nur auf Verschreibung und unter ständiger Kontrolle eines Arztes oder einer Ärztin verwendet werden.
Bei bekannter Überempfindlichkeit gegenüber dem Wirkstoff oder einem der Hilfsstoffe darf Caprelsa nicht angewendet werden.
Wenn Sie an einer angeborenen Herzerkrankung, dem sogenannten kongenitalen Long-QT-Syndrom, leiden. Dieses zeigt sich im Elektrokardiogramm (EKG).
Wenn Sie an einer schweren Nierenfunktionsstörung leiden, dürfen Sie Caprelsa nicht einnehmen.
Caprelsa darf nicht von stillenden Frauen eingenommen werden.
Caprelsa kann Ihren Herzrhythmus verändern. Aus diesem Grund sollten Ihre Kalium-, Calcium-, Magnesium- und TSH-Werte (Thyreoidea-stimulierendes Hormon) im Blut sowie Ihre Herzaktivität durch Ihren Arzt oder das Pflegepersonal mittels Bluttests bzw. Elektrokardiogramm (EKG) untersucht werden. Dies sollte sowohl vor wie auch während der Behandlung sowie bei Veränderungen der Therapie geschehen:
? 2 bis 4 Wochen nach Beginn der Behandlung mit Caprelsa.
? 8 bis 12 Wochen nach Beginn der Behandlung mit Caprelsa.
? Anschliessend alle 3 Monate.
? Wenn Ihr Arzt oder Apotheker die Dosis von Caprelsa verändert.
? Wenn Sie mit der Anwendung von Arzneimitteln beginnen, die Ihre Herzfunktion beeinträchtigen.
? Auf Anweisung Ihres Arztes oder Apothekers.
Bei manchen Patienten führt die Einnahme von Caprelsa zu einer erhöhten Empfindlichkeit gegenüber Sonnenlicht, was z.B zu einem Sonnenbrand führen kann. Schützen Sie sich deswegen im Freien stets mit Sonnenschutzmittel und durch das Tragen von Kleidung, solange Sie Caprelsa einnehmen, um eine Exposition gegenüber Sonnenstrahlung zu vermeiden.
Caprelsa kann Müdigkeit, Schwächegefühl oder Sehstörungen hervorrufen. Deshalb ist bei der Teilnahme am Strassenverkehr und der Bedienung von Maschinen Vorsicht geboten.
Bitte informieren Sie Ihren Arzt oder Apotheker, wenn Sie andere Arzneimittel einnehmen bzw. vor kurzem eingenommen haben, auch wenn es sich um nicht verschreibungspflichtige bzw. pflanzliche Mittel handelt. Dies ist erforderlich, da Caprelsa die Wirkungsweise anderer Arzneimittel beeinflussen kann und umgekehrt.
Informieren Sie Ihren Arzt oder Apotheker insbesondere, wenn Sie eines der folgenden Arzneimittel einnehmen:
? Itraconazol, Ketoconazol, Ritonavir und Clarithroycin (zur Behandlung von Infektionen).
? Rifampicin (ein Antibiotikum), Carbamazepin und Phenobarbital (zur Kontrolle von Krampfanfällen).
? Ondansetron (gegen Übelkeit und Erbrechen).
? Cisaprid (gegen Sodbrennen), Halofantrin (zur Behandlung von Malaria) und Moxifloxacin (zur Behandlung von Infektionen).
? Blutgerinnungshemmer zum Einnehmen («Blutverdünner»).
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an andern Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!
Halten Sie vor der Einnahme von Caprelsa Rücksprache mit Ihrem Arzt, wenn Sie schwanger sind oder schwanger werden möchten. Dies ist erforderlich, da Caprelsa ein ungeborenes Kind schädigen kann. Ihr Arzt wird Nutzen und Risiken der Einnahme von Caprelsa während dieser Zeit mit Ihnen besprechen.
? Wenn Sie schwanger werden können, müssen Sie während der Einnahme von Caprelsa sowie über mindestens drei Monate nach der letztmaligen Einnahme wirksame Empfängnisverhütung betreiben.
? Um das Neugeborene zu schützen, dürfen Sie während der Behandlung mit Caprelsa nicht stillen.
Die übliche Dosierung beträgt 300 mg pro Tag unabhängig von den Mahlzeiten. Nehmen Sie die ganze Filmtablette immer zur gleichen Tageszeit mit einem Glas Wasser ein.
Wenn Ihnen das Schlucken der Tablette Probleme bereitet, können Sie diese wie folgt mit Wasser mischen:
Nehmen Sie ein halbes Glas stilles (kohlensäurefreies) Wasser. Verwenden Sie ausschliesslich Wasser und keine anderen Flüssigkeiten. Geben Sie die Tablette in das Wasser. Rühren Sie um, bis die Tablette im Wasser vollständig zerfallen ist. Dies kann etwa 10 Minuten dauern. Trinken Sie das Gemisch anschliessend sofort aus. Um sicherzustellen, dass keine Arzneimittelreste im Glas verbleiben, füllen Sie dieses noch einmal bis zur Hälfte mit Wasser und trinken Sie es aus.
Informieren Sie beim Auftreten von Nebenwirkungen stets Ihren Arzt. Möglicherweise setzt dieser Ihre Caprelsa-Dosis herab (zum Beispiel auf zwei oder eine 100-mg-Tablette). Er kann Ihnen auch andere Arzneimittel verschreiben, um die Nebenwirkungen unter Kontrolle zu bringen.
Wenn Sie eine grössere Menge von Caprelsa eingenommen haben, als Ihnen verschrieben wurde, wenden Sie sich unverzüglich an einen Arzt oder ein Krankenhaus.
Wenn Sie die Einnahme einer Caprelsa-Tablette vergessen haben, hängt das weitere Vorgehen davon ab, wie viel Zeit noch bis zur nächsten planmässigen Einnahme verbleibt.
? Wenn die nächste Einnahme nicht früher als in 12 Stunden ansteht:
Holen Sie die Einnahme nach, sobald Sie daran denken. Nehmen Sie dann die nächste Dosis zur gewohnten Zeit.
? Wenn die nächste Einnahme in weniger als 12 Stunden ansteht:
Lassen Sie die vergessene Dosis aus. Nehmen Sie dann die nächste Dosis zur gewohnten Zeit.
Nehmen Sie nicht die doppelte Dosis (zwei Dosen zugleich) ein, wenn Sie die vorherige Einnahme vergessen haben.
Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.
Sollten bei Ihnen Nebenwirkungen auftreten, so verordnet Ihr Arzt möglicherweise eine niedrigere Dosis von Caprelsa. Ausserdem kann er Ihnen andere Arzneimittel verschreiben, um die Nebenwirkungen unter Kontrolle zu bringen.
Nehmen Sie sofort Kontakt mit Ihrem Arzt bzw. Ihrer Ärztin auf, wenn Sie folgende Reaktionen bei sich feststellen:
? Ohnmacht, Schwindel oder Veränderungen des Herzrhythmus. Dabei kann es sich um Zeichen einer Veränderung der elektrischen Aktivität Ihres Herzens handeln. Diese treten bei 8% der wegen eines medullären Schilddrüsenkarzinoms mit Caprelsa behandelten Patienten auf. Möglicherweise empfiehlt Ihr Arzt eine niedrigere Dosierung oder einen Abbruch der Behandlung mit Caprelsa. Caprelsa wurde gelegentlich mit lebensbedrohlichen Veränderungen des Herzrhythmus in Zusammenhang gebracht.
? Schwere, grosse Bereiche des Körpers betreffende Hautreaktionen wie z.B. Rötung, Schmerzen, Geschwüre, Blasen und Hautabschälung. Auch Lippen, Nase, Augen und Genitalien können betroffen sein. Diese Hautreaktionen treten je nach Typ häufig (bei weniger als 1 von 10 Behandelten) bis gelegentlich (bei weniger als 1 von 100 Behandelten) auf.
? Starker Durchfall.
? Schwere oder sich plötzlich verschlimmernde Atemnot, möglicherweise verbunden mit Husten oder erhöhter Temperatur (Fieber). Dies kann bedeuten, dass Sie an einer sogenannten interstitiellen Lungenentzündung leiden. Diese gelegentlich (bei weniger als 1 von 100 Behandelten) auftretende Nebenwirkung kann lebensbedrohlich sein.
? Krampfanfälle, Kopfschmerzen, Verwirrtheit oder Konzentrationsschwierigkeiten. Dabei kann es sich um Zeichen der Erkrankung PRLES (Posteriores Reversibles Leukenzephalopathie-Syndrom) handeln. In der Regel klingen die Beschwerden ab, wenn Caprelsa abgesetzt wird. PRLES tritt gelegentlich auf (bei weniger als 1 von 100 Behandelten).
Im Weiteren wurde über folgende Nebenwirkungen berichtet:
Sehr häufig: Durchfall (Ihr Arzt kann Arzneimittel gegen Durchfall verschreiben. Wenn Sie an starkem Durchfall leiden, informieren Sie unverzüglich Ihren Arzt), Bauchschmerzen, Hautausschlag oder Akne, Müdigkeit, Übelkeit, Erbrechen, Appetitlosigkeit, Schwächegefühl(, Bluthochdruck, Kopfschmerzen, Erschöpfung, Schlafstörungen, erhöhte Sonnenlichtempfindlichkeit der Haut. Schützen Sie sich im Freien stets mit Sonnencreme und durch das Tragen von Kleidung, solange Sie Caprelsa einnehmen, um eine Exposition gegenüber Sonnnenstrahlung zu vermeiden.
Häufig: Austrocknung, Depression, stark erniedrigter Blutdruck, Gewichtsabnahme, Schlaganfall oder Unterversorgung des Gehirns mit Blut, Ausschlag an Händen und Füssen (Hand-Fuss-Syndrom), Entzündung der Mundschleimhaut, Mundtrockenheit, Steine oder Calciumablagerungen in den Harnwegen, Nasenbluten, Verschwommensehen einschliesslich leichter Veränderungen im Auge, die zu Verschwommensehen führen können (Hornhauttrübung), Augentrockenheit, Bindehautentzündung, Beeinträchtigung der Sehkraft, Geschmacksstörungen.
Folgende Nebenwirkungen können sich in Untersuchungen zeigen, die von Ihrem Arzt durchgeführt werden:
Eiweiss oder Blut im Urin (in einem Urintest).
Veränderungen des Herzrhythmus (in einem EKG). Ihr Arzt weist Sie möglicherweise an, Caprelsa abzusetzen oder in einer niedrigeren Dosierung einzunehmen.
Auffälligkeiten im Hinblick auf Leber oder Bauchspeicheldrüse (in Bluttests). Diese rufen normalerweise keine Beschwerden hervor; eventuell möchte Ihr Arzt den Verlauf dennoch überwachen.
Erniedrigte Calciumspiegel im Blut. Ihr Arzt muss möglicherweise eine Behandlung mit Schilddrüsenhormonen einleiten oder eine bereits bestehende derartige Behandlung umstellen.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin sofort informieren.
Caprelsa muss für Kinder unerreichbar gelagert werden.
Es ist in der Originalverpackung nicht über 30 °C aufzubewahren.
Das Arzneimittel darf nur bis zu dem auf der Schachtel mit «EXP» bezeichneten Datum verwendet werden. Falls Sie nach Beenden der Therapie noch übrige Filmtabletten haben oder Filmtabletten besitzen sollten, deren Haltbarkeit abgelaufen ist, so bringen Sie diese bitte in Ihre Apotheke zur Vernichtung zurück. Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.
