Carvedilol-Mepha enthält einen blutdrucksenkenden Wirkstoff. Zwei Wirkungen dieses Wirkstoffes führen zur Blutdrucksenkung. Einerseits wirkt er als Betablocker, indem er das Herz und den Kreislauf gegen übermässige Stressreize abschirmt, und anderseits erweitert er die kleinen Blutgefässe in der Peripherie.
Carvedilol-Mepha darf nur auf Verschreibung des Arztes oder der Ärztin zur Behandlung des erhöhten Blutdrucks (Hypertonie) angewendet werden.
Ferner wird Carvedilol-Mepha zur Prophylaxe von Durchblutungsstörungen der Herzkranzgefässe (Angina pectoris) eingesetzt. Carvedilol-Mepha bewirkt, dass das Herz weniger belastet wird. Es kommt daher mit dem vorhandenen Blutangebot eher aus, das infolge der verengten Herzkranzgefässe eingeschränkt ist.
Carvedilol-Mepha wird auch zur Behandlung der Herzinsuffizienz (Herzleistungsschwäche) zusammen mit Herzglykosiden, einem Diuretikum (wassertreibendes Arzneimittel) und einem ACE-Hemmer (Angiotensin-Converting-Enzym-Inhibitor) eingesetzt. Bei Herzleistungsschwäche ist das Herz nicht mehr in der Lage, die ausreichende Menge Blut aus den Herzkammern in die Schlagadern auszustossen. Zusätzlich mit den anderen Arzneimitteln wird Ihr Blutdruck gesenkt, das Schlagvolumen des Herzens nimmt zu.
Sie können die blutdrucksenkende Wirkung von Carvedilol-Mepha unterstützen, indem Sie nicht rauchen, sich möglichst fettarm ernähren, sich viel körperliche Bewegung verschaffen und auf ein normales Körpergewicht achten. Fragen Sie Ihren Arzt oder Ihre Ärztin, ob eine salzarme Diät während der Behandlung mit Carvedilol-Mepha notwendig ist.
Bei Überempfindlichkeit gegen Carvedilol-Mepha oder gegen einen der Hilfsstoffe oder andere Betablocker, langsamen Puls, bestimmten Herz- und Lebererkrankungen, bei stark erniedrigtem Blutdruck (unterhalb von 85 mmHg) schweren Kreislaufstörungen, bestimmten Atemwegserkrankungen (z.B. Asthma, chronische Bronchitis), während einer Desensibilisierung bei Allergien und bei gleichzeitiger Gabe von MAO-Hemmern (Arzneimittel gegen Depressionen) darf Carvedilol-Mepha nicht angewendet werden. Bei Langsam-Metabolisierern kann es zu einer erhöhten Carvedilol-Konzentration kommen. Mangels Erfahrung darf Carvedilol-Mepha bei diesen Patienten oder Patientinnen deshalb nicht angewendet werden.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie
Bei Diabetikern kann Carvedilol-Mepha die Symptome eines Blutzuckerabfalls (Hypoglykämie) verschleiern oder abschwächen. Diabetiker sollten jede Änderung bezüglich ihrer Blutzuckerwerte ihrem Arzt oder ihrer Ärztin mitteilen.
Die gleichzeitige Einnahme von Rifampicin (Antibiotikum, Tuberkulostatikum) kann zu einer Abschwächung der Blutdrucksenkung führen.
Falls Sie Clonidin gegen Bluthochdruck oder Migräne einnehmen, sollten Sie weder Clonidin noch Carvedilol-Mepha von sich aus absetzen, ohne mit Ihrem Arzt bzw. Ihrer Ärztin darüber gesprochen zu haben.
Die gleichzeitige Einnahme von Clonidin kann zu einer Veränderung des Blutdrucks und der Herzfrequenz führen und sollte deshalb mit dem Arzt oder der Ärztin abgesprochen werden.
Bei älteren Patienten oder Patientinnen besteht die Gefahr eines verstärkten Blutdruckabfalles.
Bei gleichzeitiger Behandlung mit Cyclosporin (ein Arzneimittel zur Verhinderung von Abstossungsreaktionen nach Organtransplantationen) kann die Wirkung von Cyclosporin verstärkt werden. Ihr Arzt oder Ihre Ärztin wird Sie deshalb engmaschig überwachen und eventuell die Dosierung von Cyclosporin anpassen.
Carvedilol-Mepha kann aufgrund der möglichen Nebenwirkungen (z.B. Schwindel, Müdigkeit) die Reaktionsfähigkeit, die Fahrtüchtigkeit und Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Dies gilt besonders bei Behandlungsbeginn, nach einer Dosissteigerung, bei Präparatewechsel oder wenn zusätzlich Alkohol konsumiert wird. Deshalb ist Vorsicht im Strassenverkehr und beim Bedienen von Maschinen geboten.
Die gleichzeitige Anwendung von sogenannten nichtsteroidalen Antirheumatika (NSAR ? wie z.B. Acetylsalicylsäure oder Ibuprofen) und Betablockern kann zu einem Anstieg des Blutdrucks und zu einer beeinträchtigten Blutdruckkontrolle führen.
Herzinsuffizienz-Patienten oder Patientinnen sollten ihren Arzt oder ihre Ärztin aufsuchen, sobald sie Zeichen oder Symptome einer Verschlechterung ihrer Herzinsuffizienz bemerken (Gewichtszunahme oder Kurzatmigkeit).
Sie könnten beim Stehen unter Blutdruckabfällen leiden, welche zu Schwindel und selten zu Ohnmachtsanfällen führen könnten. Sie sollten sich setzen oder hinlegen, wenn sich diese Symptome bemerkbar machen.
Patienten oder Patientinnen, welche unter Schwindel oder Müdigkeit leiden, sollten nicht Auto fahren oder gefährliche Aufgaben wahrnehmen. Dies gilt zudem grundsätzlich für alle Patienten oder Patientinnen bei Behandlungsbeginn und während der Dosissteigerung.
Bei Kontaktlinsenträgern kann sich die Verminderung der Tränenflüssigkeit negativ auf den Tragkomfort von Linsen auswirken.
Carvedilol-Mepha darf während der Schwangerschaft und Stillzeit nicht eingenommen werden.
Zu Beginn der Behandlung wird während zwei Tagen, ältere Patienten oder Patientinnen während zwei Wochen, ½ Tablette à 25 mg oder 1 Tablette à 12,5 mg eingenommen.
Anschliessend wird normalerweise täglich eine Tablette à 25 mg eingenommen. Falls nötig, kann der Arzt oder die Ärztin eine Erhöhung der Dosierung auf höchstens 2 Tabletten à 25 mg täglich anordnen. Bei einigen Patienten oder Patientinnen, besonders bei älteren, kann der Blutdruck manchmal schon mit einer ½ Tablette à 25 mg oder 1 Tablette à 12,5 mg pro Tag genügend gesenkt werden.
Zu Beginn der Behandlung wird während zwei Tagen morgens und abends je ½ Tablette à 25 mg oder 1 Tablette à 12,5 mg eingenommen. Anschliessend wird normalerweise morgens und abends je 1 Tablette à 25 mg eingenommen. Falls nötig, kann der Arzt oder die Ärztin eine Erhöhung der Dosierung anordnen.
Zu Beginn der Behandlung wird während 2 Wochen 2-mal täglich je 1 Tablette Carvedilol-Mepha à 3,125 mg oder ½ Tablette Carvedilol-Mepha à 6,25 mg eingenommen. Ihr Arzt oder Ihre Ärztin wird die Dosierung von Carvedilol-Mepha genau mit Ihnen besprechen. In der Regel wird anschliessend während 2 Wochen 2-mal täglich je 1 Tablette Carvedilol-Mepha à 6,25 mg, dann während 2 Wochen 2-mal täglich je 1 Tablette Carvedilol-Mepha à 12,5 mg und anschliessend während 2 Wochen 2-mal täglich je 1 Tablette Carvedilol-Mepha à 25 mg eingenommen.
Sie sollten Ihren Arzt oder Ihre Ärztin aufsuchen, sobald Sie unter Schwindel oder Ohnmachtsanfällen beim Anpassen der Dosierung leiden.
Ältere Patienten oder Patientinnen sollten im Allgemeinen die Dosierung von 2x täglich 1 Tablette à 25 mg nicht überschreiten.
Die Tabletten werden mit etwas Flüssigkeit, während oder nach den Mahlzeiten, unzerkaut geschluckt.
Carvedilol-Mepha sollte wie alle diese Arzneimittel nach längerer Einnahme schrittweise abgesetzt werden. Unterbrechen Sie daher die Einnahme des Arzneimittels nicht ohne Anordnung durch Ihren Arzt oder Ihre Ärztin.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme von Carvedilol-Mepha auftreten:
Schwindel, Kopfschmerzen und Müdigkeit können gelegentlich zu Beginn der Behandlung auftreten und gehen meist vorüber.
Gelegentlich wurden auch langsamer Puls, niedriger Blutdruck, Kribbeln oder Kältegefühl in Händen und Füssen, vermehrtes Schwitzen, Ödeme, Gewichtszunahme, verstopfte Nase, Bronchitis, Bauchschmerzen, Durchfall, Übelkeit, Erbrechen, grippeähnliche Symptome, Kraftlosigkeit, Rücken-, Gelenk- und Muskelschmerzen beobachtet.
Selten wurden Schlafstörungen, Impotenz und Augenreizungen beobachtet.
Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn solche Störungen länger anhalten. Vereinzelt sind Atemnot, auffallend langsamer Puls, unregelmässiger Herzschlag, Schmerzen in der Herzgegend, traurige Stimmung, unwillkürlicher Harnverlust bei Frauen, allergische Reaktionen (z.B. Juckreiz, Hautausschläge) und Sehstörungen vorgekommen. Sehr seltene Fälle von schweren unerwünschten Hautreaktionen wie toxische epidermale Nekrolyse (TEN) und Stevens-Johnson-Syndrom (SJS) wurden während der Behandlung mit Dilatrend gemeldet.
Bei solchen Erscheinungen sollten Sie umgehend Ihren Arzt oder Ihre Ärztin informieren.
Zusätzlich wurden nach Markteinführung Fälle von Haarausfall in möglichen Zusammenhang mit Carvedilol berichtet.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

Carvedilol-Mepha Tabletten sind trocken und in der Originalpackung bei Raumtemperatur (15-25 °C) zu lagern.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.
