+-Was ist Clopidogrel Zentiva und wann wird es angewendet?

Clopidogrel, der Wirkstoff in Clopidogrel Zentiva Tabletten, gehört zu einer Gruppe von Arzneimitteln, die als Thrombozytenaggregationshemmer bezeichnet werden. Blutplättchen (Thrombozyten) sind sehr kleine Blutzellen (kleiner als rote oder weisse Blutkörperchen), die sich während der Bildung eines Blutpfropfs (Thrombus) zusammenklumpen. Thrombozytenaggregationshemmer verhindern dieses Zusammenklumpen und verringern auf diese Weise das Risiko der Entstehung von Blutgerinnseln (sogenannten Thrombosen).

Da Sie kürzlich einen Schlaganfall oder Herzinfarkt erlitten haben oder an einer peripheren Arterienverengung leiden, haben Sie ein erhöhtes Risiko für ein Wiederauftreten dieser Ereignisse.

Um dieses Risiko zu reduzieren, haben Sie Clopidogrel Zentiva verschrieben bekommen.

Clopidogrel Zentiva wurde Ihnen ebenfalls verschrieben

  • bei peripherer arterieller Verschlusskrankheit, oder
  • falls Sie starke Brustschmerzen erlitten haben, sogenannte «instabile Angina pectoris» oder «Herzinfarkt». In diesem Fall kann Ihr Arzt bzw. Ihre Ärztin Ihnen gleichzeitig Acetylsalicylsäure verschreiben (eine Substanz, die in vielen Arzneimitteln gegen Schmerzen und Fieber vorhanden ist) oder
  • Wenn Sie an einer unregelmässigen Herzschlagfrequenz leiden, das sogenannte Vorhofflimmern und wenn Sie nicht mit oralen Antikoagulanzien behandelt werden dürfen. In diesem Fall wird der Arzt bzw. die Ärztin Ihnen zusätzlich Acetylsalicylsäure verschreiben.

Clopidogrel Zentiva wird ebenfalls zur Vorbeugung von Thrombosen nach Implantation von vaskulären Endoprothesen (Stent) in Kombination mit Acetylsalicylsäure angewendet.

Clopidogrel Zentiva darf nur auf Verschreibung des Arztes bzw. die Ärztin eingenommen und nicht an andere Personen weitergegeben werden.

+-Was sollte dazu beachtet werden?

Gewisse Faktoren wie Bluthochdruck, erhöhtes Cholesterin, Rauchen oder Zuckerkrankheit sind oftmals die Ursache arterieller Erkrankungen; es ist deshalb wichtig, seine Lebensweise im Sinne einer Herabsetzung dieser Risikofaktoren zu verändern.

+-Wann darf Clopidogrel Zentiva nicht angewendet werden?

Clopidogrel Zentiva darf nicht angewendet werden, wenn Sie

  • eine Überempfindlichkeit (Allergie) gegen dieses Arzneimittel aufweisen oder früher allergisch auf Ticlopidin oder der Substanz Prasugrel reagiert haben;
  • an einer Blutgerinnungsstörung leiden, aufgrund derer Sie ein erhöhtes Risiko für innere Blutungen haben (z.B. zu Blutungen im Gewebe, in Organen oder in Gelenken Ihres Körpers);
  • an einer Krankheit leiden, aufgrund derer Sie ein erhöhtes Risiko für innere Blutungen haben (z.B. ein Magen- oder Darmgeschwür);
  • eine erhöhte Blutungsneigung haben;
  • an einer schweren Leberfunktionsstörung leiden.

Bei Kindern und Jugendlichen unter 18 Jahren wurde die Wirksamkeit und Sicherheit der Anwendung von Clopidogrel Zentiva nicht untersucht.

Falls Sie glauben, dass einer dieser krankhaften Zustände bei Ihnen vorliegen könnte, oder falls Sie diesbezüglich unsicher sind, suchen Sie bitte Ihren Arzt bzw. Ihre Ärztin auf, bevor Sie mit der Einnahme von Clopidogrel Zentiva Tabletten beginnen.

+-Wann ist bei der Anwendung von Clopidogrel Zentiva Vorsicht geboten?

Bevor Sie Clopidogrel Zentiva einnehmen, müssen Sie Ihren Arzt bzw. Ihre Ärztin darüber informieren, wenn Sie

  • vor Kurzem eine ernste Verletzung hatten;
  • sich vor Kurzem einer Operation (einschliesslich eines zahnärztlichen Eingriffes) unterzogen haben;
  • an einer Blutgerinnungsstörung leiden, aufgrund derer Sie ein erhöhtes Risiko für innere Blutungen haben (z.B. zu Blutungen im Gewebe, in Organen oder in Gelenken Ihres Körpers);
  • an einer Krankheit leiden, die Blutungen hervorrufen kann (z.B. ein Magengeschwür);
  • sich in den nächsten zwei Wochen einer Operation (einschliesslich eines zahnärztlichen Eingriffes) unterziehen;
  • an Nieren- oder Leberstörungen leiden;
  • vor Kurzem einen Schlaganfall erlitten haben;
  • vor Kurzem einen Myokardinfarkt erlitten haben (Clopidogrel Zentiva darf nach einer Streptokinase-Therapie nicht angewendet werden);
  • unzureichend oder nicht kontrollierten Bluthochdruck aufweisen;
  • an erworbener Hämophilie leiden (Blutgerinnungsstörungen).
  • bereits eine Allergie oder Reaktion auf irgendein Arzneimittel hatten, das zur Behandlung Ihrer Erkrankung angewendet wird.

Clopidogrel Zentiva verändert die Gerinnungsfähigkeit des Blutes. Wenn Sie sich schneiden oder verletzen, kann die Blutung eventuell etwas länger als sonst andauern.

Kleinere Schnitte und Verletzungen, die z.B. beim Rasieren auftreten können, sind in der Regel ohne Bedeutung. Wenn Sie trotzdem verunsichert sind oder eine ungewöhnliche Blutung feststellen, sollten Sie sich unverzüglich mit Ihrem Arzt bzw. Ihrer Ärztin in Verbindung setzen.

Nach Implantation einer vaskulären Endoprothese (Stent) oder bei Patienten, die einen starken Brustschmerz erlitten haben (instabile Angina pectoris oder Herzinfarkt), kann Clopidogrel Zentiva in Kombination mit Acetylsalicylsäure (75-325 mg täglich) verschrieben werden. Diese Substanz ist in vielen Arzneimitteln gegen Schmerzen und Fieber enthalten. In solchen Fällen wird in der Regel zur langfristigen Kombination mit Clopidogrel Zentiva eine fixe Tagesdosis von 75-100 mg Acetylsalicylsäure empfohlen. Bei anderen Vorbedingungen muss die Verabreichung von Acetylsalicylsäure während einer längeren Zeitspanne mit Ihrem Arzt bzw. Ihrer Ärztin besprochen werden. Eine gelegentliche Verabreichung von Acetylsalicylsäure (nicht mehr als 1'000 mg während 24 Stunden) sollte problemlos sein.

Gewisse Arzneimittel, ob sie nun von Ihrem Arzt bzw. Ihrer Ärztin verschrieben wurden oder frei verkäuflich sind, können die Wirkung dieses Arzneimittels ändern, und es können unerwünschte Wirkungen auftreten. Daher sollten Sie, bevor Sie ? während einer Behandlung mit Clopidogrel Zentiva ? ein anderes Medikament einnehmen, Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin fragen.

Die gleichzeitige Einnahme folgender Medikamente mit Clopidogrel Zentiva wird nicht empfohlen:

  • Protonen-Pumpen-Inhibitor (z.B. Omeprazol) für die Behandlung von Magenschmerzen.
  • Andere Arzneimittel, die zur Hemmung der Blutgerinnung eingesetzt werden, wie Phenprocoumon (Marcoumar), Acenocoumarol (Sintrom), Heparin oder Warfarin.
  • Nichtsteroidale Antirheumatika des Typs Aspirin (Arzneimittel zur Behandlung von schmerzhaften und/oder entzündlichen Zuständen von Muskeln und Gelenken) dürfen nur auf ausdrückliche Verordnung und unter Kontrolle eines Arztes bzw. einer Ärztin gleichzeitig mit Clopidogrel Zentiva eingenommen werden.
  • Antidepressiva (Fluvoxamin, Fluoxetin, Moclobemid, SSRI).
  • Antimykotika (Variconazol, Fluconazol).
  • Antibiotika (Ciprofloxacin).
  • Antiepileptika (Carbamazepin).

Clopidogrel Zentiva enthält Laktose. Patienten mit der seltenen hereditären Galaktose-Intoleranz, mit Laktase-Mangel oder Glucose-Galaktose-Malabsorption sollten dieses Medikament nicht einnehmen. Dieses Arzneimittel enthält auch hydriertes Rizinusöl, das Magenverstimmung und Durchfall hervorrufen kann.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

  • an anderen Krankheiten leiden,
  • Allergien haben oder
  • andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
+-Darf Clopidogrel Zentiva während einer Schwangerschaft oder in der Stillzeit angewendet werden?

Falls Sie schwanger sind oder Ihr Kind stillen, müssen Sie dies Ihrem Arzt bzw. Ihrer Ärztin mitteilen. Er bzw. sie wird entscheiden, ob Sie Clopidogrel Zentiva während der Schwangerschaft oder Stillzeit einnehmen dürfen.

Falls Sie während der Einnahme von Clopidogrel Zentiva schwanger werden, teilen Sie dies Ihrem Arzt bzw. Ihrer Ärztin unverzüglich mit.

Während der Behandlung mit Clopidogrel Zentiva dürfen Sie nicht stillen.

+-Wie verwenden Sie Clopidogrel Zentiva?

Die Tablette soll mit etwas Wasser ? unzerkaut und ohne sie zu lutschen ? während oder unabhängig von den Mahlzeiten eingenommen werden. Es wird aber empfohlen, sie immer zum selben Zeitpunkt einzunehmen.

Erwachsene

Prävention atherothrombotischer Ereignisse (Schlaganfall, Herzinfarkt):

Falls vom Arzt bzw. von der Ärztin nicht anders verordnet, nehmen Sie 1 Tablette Clopidogrel Zentiva 75 mg pro Tag. Starke Brustschmerzen (instabile Angina pectoris oder Herzinfarkt): Ihr Arzt bzw. Ihre Ärztin kann Ihnen am Anfang der Behandlung eine Ladedosis von 300 mg Clopidogrel Zentiva verschreiben. Für die Langzeitbehandlung wird 1 Tablette Clopidogrel Zentiva zu 75 mg täglich verabreicht (in Kombination mit 75-325 mg Acetylsalicylsäure täglich).

Prävention von Thrombosen nach Implantation von vaskulärer Endoprothese (Stent):

1× täglich 1 Tablette zu 75 mg Clopidogrel Zentiva, in Kombination mit Acetylsalicylsäure (in den klinischen Studien wurden 325 mg pro Tag verwendet). Im Falle einer Stentimplantation kann die Behandlung am ersten Tag eventuell mit einer Ladedosis von 300 mg Clopidogrel Zentiva begonnen werden.

Bei Vorhofflimmern: 1× täglich 1 Tablette zu 75 mg Clopidogrel Zentiva in Kombination mit 100 mg Acetylsalicylsäure täglich.

Kinder und Jugendliche

Die Wirksamkeit und Sicherheit der Anwendung von Clopidogrel Zentiva bei Kindern und Jugendlichen unter 18 Jahren wurde nicht untersucht.

Bei Nieren- und Leberinsuffizienz

Clopidogrel Zentiva ist bei diesem Patiententyp mit Vorsicht anzuwenden.

Ältere Personen

Eine Dosisanpassung ist bei älteren Patienten nicht erforderlich.

Behandlungsdauer

Bei Clopidogrel Zentiva handelt es sich um eine Langzeitbehandlung. Daher müssen Sie Clopidogrel Zentiva so lange einnehmen, wie es Ihnen Ihr Arzt bzw. Ihre Ärztin verordnet.

Falls Sie eine Überdosis Clopidogrel Zentiva eingenommen haben, informieren Sie umgehend Ihren Arzt bzw. Ihre Ärztin oder suchen Sie die Notaufnahme des nächstgelegenen Spitals auf. Die Einnahme einer grossen Anzahl von Tabletten könnte das Risiko schwerer Blutungen erhöhen, die eine notfallmässige Behandlung erfordern.

Falls Sie die Einnahme von Clopidogrel Zentiva einmal vergessen haben und dies innerhalb der nächsten zwölf Stunden nach dem üblichen Einnahmezeitpunkt merken, sollten Sie die Einnahme sofort nachholen und die nächste Filmtablette dann zur gewohnten Zeit einnehmen.

Wenn Sie den üblichen Einnahmezeitpunkt um mehr als zwölf Stunden verpasst haben, nehmen Sie einfach die nächste Tablette zum vorgesehenen nächsten Einnahmezeitpunkt. Nehmen Sie nicht die doppelte Tablettenmenge ein, um die vergessene Tablette nachzuholen.

Sie können anhand des Kalenders auf der Durchdrückpackung überprüfen, an welchem Tag Sie die letzte Clopidogrel Zentiva Tablette eingenommen haben.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Medikament wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Clopidogrel Zentiva haben?

Folgende Nebenwirkungen können bei der Einnahme von Clopidogrel Zentiva auftreten:

  • Häufig: Blutungen, Verdauungsstörungen, Bauchschmerzen, Durchfälle, kleine Blutergüsse.
  • Gelegentlich: Magen- oder Darmgeschwüre und -blutungen, Nasenbluten, blaue Flecken (Ekchymosen), Blut im Urin, Augen-, Lungen-, Muskel-, Gelenk- und Hirnblutungen. Schwindelgefühl, Kribbeln in Händen und Füssen, Kopfschmerzen. Übelkeit, Magenschleimhautentzündung (Gastritis), Blähungen, Verstopfung, Erbrechen. Leber- und Gallenstörungen. Juckreiz.
  • Selten: Schwindelanfälle, Erhöhung der Brustvolumen beim Mann.
  • Sehr selten: Generalisierte allergische Reaktionen (Ödem des Gesichts, der Lippen und/oder der Zunge). Schwere hämatologische Zwischenfälle (Neutropenie: Verminderung der Anzahl einer Fraktion der weissen Blutkörperchen) können auftreten und schwere Infektionen verursachen, erworbene Hämophilie (Blutgerinnungsstörungen). Geschmacksstörungen. Verwirrtheit, Halluzinationen. Gefässentzündungen, niedriger Blutdruck. Atembeschwerden. Dickdarmentzündung (Kolitis), Bauchspeicheldrüsenentzündung, Entzündung der Mundschleimhaut. Leberentzündung (Hepatitis). Hautausschlag, Ekzem, Hautabschälung. Gelenkschmerzen, Muskelschmerzen. Nierenerkrankung. Fieber.

Beim Auftreten folgender Symptome sollten Sie die Therapie mit Clopidogrel Zentiva absetzen und sofort Ihren Arzt bzw. Ihre Ärztin aufsuchen: Fieber, Halsentzündungen, Geschwüre im Mund- und Halsbereich, verlängerte oder verstärkte Blutungen, Blutergüsse, Hauteinblutungen, Schwarzverfärbung des Stuhls.

Wenn Sie Clopidogrel zusammen mit Acetylsalicylsäure (Aspirin) (Acetylsalicylsäure: Substanz, die in zahlreichen Arzneimitteln enthalten ist und die zur Linderung von Schmerzen und zur Fiebersenkung angewendet wird), nichtsteroidalen Entzündungshemmer, Heparin oder irgendeinem anderen Arzneimittel einnehmen, das eine Blutung infolge einer Verletzung, eines chirurgischen Eingriffs oder jeglicher anderer Ursache auslösen bzw. verlängern könnte, müssen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin über jedes Auftreten einer Blutung unterrichten.

Wenn Sie das Auftreten irgendeiner anderen unerwünschten Wirkung feststellen, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden. Sollten Sie Tabletten besitzen, deren Haltbarkeit abgelaufen ist, so bringen Sie diese bitte in Ihre Apotheke zurück.

Filmtabletten zu 75 mg: Nicht über 25 °C lagern.

Filmtabletten zu 300 mg: Bei Raumtemperatur (15-25 °C) lagern.

Ausserhalb der Reichweite von Kindern aufbewahren.
