+-Was ist Esbriet und wann wird es angewendet?

Esbriet enthält den Wirkstoff Pirfenidon und wird zur Behandlung von idiopathischer pulmonaler Fibrose (IPF) bei Erwachsenen angewendet.

IPF ist eine besonders schwere Erkrankung der Lunge, bei der das Lungengewebe sich mit der Zeit verdickt und vernarbt. Diese Veränderungen führen dazu, dass die Lunge starr und unelastisch wird und ihre lebenswichtige Aufgabe des Luftaustauschs (Sauerstoff-Aufnahme und Kohlendioxid-Abgabe) immer schlechter erfüllen kann.

Esbriet darf nur auf Verschreibung des Arztes bzw. der Ärztin angewendet werden.

+-Was sollte dazu beachtet werden?

Vor und während der Behandlung mit Esbriet sollten Sie mit dem Rauchen aufhören. Rauchen kann die Wirkung von Esbriet herabsetzen.

+-Wann darf Esbriet nicht eingenommen werden?

Esbriet darf nicht eingenommen werden,

  • wenn Sie überempfindlich (allergisch) gegen den Wirkstoff Pirfenidon oder einen der sonstigen Bestandteile von Esbriet sind (siehe «Was ist in Esbriet enthalten?»).
  • wenn Sie ein Arzneimittel mit dem Wirkstoff Fluvoxamin (zur Behandlung von Depressionen und Zwangsstörungen) einnehmen.
  • wenn Sie eine schwere Lebererkrankung haben.
  • wenn Sie eine schwere Nierenerkrankung haben oder sich einer Dialyse unterziehen müssen.

Wenn Sie sich nicht sicher sind, fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin.

+-Wann ist bei der Einnahme von Esbriet Vorsicht geboten?
  • Wenn Sie Esbriet einnehmen, können Sie empfindlicher auf Sonnenlicht reagieren (Photosensibilitätsreaktion). Meiden Sie die Sonne (und Sonnenstudios), während Sie Esbriet einnehmen. Tragen Sie täglich ein Sonnenschutzmittel auf und bedecken Sie Arme, Beine und den Kopf zum Schutz vor Sonnenlicht (siehe Abschnitt «Welche Nebenwirkungen kann Esbriet haben?»).
  • Sie dürfen keine anderen Arzneimittel einnehmen, die dazu führen können, dass Sie empfindlicher auf Sonnenlicht reagieren.
  • Wenn Sie an einer leichten bis mittelschweren Leber- oder Nierenerkrankung leiden.

Bevor Sie mit der Einnahme von Esbriet beginnen, muss ein Bluttest durchgeführt werden, um Ihre Leberfunktion zu überprüfen. Dieser Kontrolltest muss in den ersten 6 Monaten der Behandlung einmal monatlich und danach alle 3 Monate wiederholt werden. Es ist wichtig, dass diese Bluttests regelmässig durchgeführt werden, solange Sie Esbriet einnehmen.

Kinder und Jugendliche

Esbriet darf Kindern und Jugendlichen unter 18 Jahren nicht verabreicht werden.

Bei Einnahme von Esbriet mit anderen Arzneimitteln

Dies ist besonders wichtig, wenn Sie die folgenden Arzneimittel einnehmen, da diese die Wirkung von Esbriet beeinflussen können.

  • Ciprofloxacin (ein Antibiotikum)

Arzneimittel, die die Wirkung von Esbriet verringern können:

  • Omeprazol (zur Behandlung von Verdauungsbeschwerden oder Refluxkrankheit)
  • Rifampicin (ein Antibiotikum)
  • Esbriet darf nicht gleichzeitig mit dem Wirkstoff Fluvoxamin (zur Behandlung von Depressionen und Zwangsstörungen) eingenommen werden

Bei Einnahme von Esbriet zusammen mit Nahrungsmitteln und Getränken

Nehmen Sie dieses Arzneimittel während einer Mahlzeit ein, um das Risiko für Nebenwirkungen wie Übelkeit und Schwindel zu verringern (siehe Abschnitt «Welche Nebenwirkungen kann Esbriet haben?»).

Verkehrstüchtigkeit und das Bedienen von Maschinen

Esbriet kann Schwindel und Müdigkeit verursachen. Seien Sie vorsichtig, wenn Sie Tätigkeiten ausüben müssen, bei denen Sie wach und koordinationsfähig sein müssen. Sie dürfen sich nicht an das Steuer eines Fahrzeugs setzen oder Maschinen bedienen, wenn Sie nach Einnahme von Esbriet müde sind oder Ihnen schwindlig ist.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

  • an anderen Krankheiten leiden,
  • Allergien haben oder
  • andere Arzneimittel (auch selbst gekaufte!) anwenden!
+-Darf Esbriet während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Nehmen Sie Esbriet nicht ein, wenn Sie schwanger sind, eine Schwangerschaft planen oder glauben, dass Sie schwanger sein könnten.

Wenn Sie stillen, nehmen Sie Esbriet nur nach Rücksprache mit Ihrem Arzt oder Apotheker ein. Es ist nicht bekannt, ob Esbriet in die Muttermilch übergeht. Falls Sie stillen und Esbriet einnehmen müssen, spricht Ihr Arzt mit Ihnen über die Risiken und den Nutzen der Behandlung mit Esbriet während der Stillzeit.

+-Wie verwenden Sie Esbriet?

Nehmen Sie Esbriet immer genau nach Anweisung Ihres Arzt/Ihrer Ärztin ein. Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

Nehmen Sie das Arzneimittel in steigenden Dosen folgendermassen ein:

  • an den ersten 7 Tagen 1 Kapsel dreimal täglich zu den Mahlzeiten (insgesamt 801 mg/Tag)
  • vom 8. bis zum 14. Tag 2 Kapseln dreimal täglich zu den Mahlzeiten (insgesamt 1602 mg/Tag)
  • ab dem 15. Tag 3 Kapseln dreimal täglich zu den Mahlzeiten (insgesamt 2403 mg/Tag).

Schlucken Sie die Kapseln als Ganzes mit etwas Wasser während einer Mahlzeit, um das Risiko für Nebenwirkungen wie Übelkeit und Schwindel zu verringern. Falls Beschwerden länger anhalten, suchen Sie Ihren Arzt auf.

Die Anwendung und Sicherheit von Esbriet bei Kindern und Jugendlichen ist bisher nicht geprüft worden. Bei diesen Altersgruppen wird das Arzneimittel daher nicht angewendet.

Dosisreduktion wegen Nebenwirkungen

Der Arzt kann Ihre Dosis herabsetzen, wenn Nebenwirkungen wie z.B. Magenbeschwerden, Hautreaktionen auf Sonnenlicht oder UV-Bestrahlung oder wesentliche Veränderungen Ihrer Leberenzyme bei Ihnen auftreten.

Wenn Sie eine grössere Menge von Esbriet eingenommen haben, als Sie sollten

Wenden Sie sich umgehend an Ihren Arzt, Apotheker oder die Notaufnahme des nächstgelegenen Krankenhauses, wenn Sie mehr Kapseln eingenommen haben, als Sie sollten, und nehmen Sie das Arzneimittel mit.

Wenn Sie die Einnahme von Esbriet vergessen haben

Wenn Sie eine Dosis vergessen haben, nehmen Sie die vergessene Dosis ein, sobald Sie es bemerken. Zwischen den einzelnen Dosen muss jedoch ein Zeitraum von mindestens 3 Stunden liegen. Nehmen Sie nicht die doppelte Dosis ein, um eine vergessene Dosis nachzuholen.

Wenn Sie die Einnahme von Esbriet abbrechen

Beenden Sie die Einnahme von Esbriet nur auf Anweisung Ihres Arztes. Wenn Sie die Einnahme von Esbriet aus irgendeinem Grund länger als 14 Tage in Folge unterbrechen müssen, beginnt der Arzt Ihre Behandlung erneut mit 1 Kapsel dreimal täglich. Anschliessend wird die Dosis nach und nach wieder auf 3 Kapseln dreimal täglich erhöht (gemäss dem Schema von oben).

+-Welche Nebenwirkungen kann Esbriet haben?

Wie alle Arzneimittel kann Esbriet Nebenwirkungen haben, die aber nicht bei jedem auftreten müssen.

Brechen Sie die Einnahme von Esbriet ab und verständigen Sie sofort Ihren Arzt,

  • wenn eine schwere Hautreaktion auf Sonnenlicht oder UV-Bestrahlung wie zum Beispiel Blasenbildung und/oder ausgeprägte Abschälung der Haut bei Ihnen auftritt (siehe Abschnitt «Wann ist bei der Einnahme von Esbriet Vorsicht geboten?»).
  • wenn Sie sich unwohl fühlen und eine Gelbfärbung der Augen oder der Haut haben oder dunklen Urin haben, möglicherweise begleitet von Juckreiz der Haut.

Hautreaktionen, Übelkeit, Müdigkeit, Durchfall, Verdauungsstörung, Appetitlosigkeit oder Kopfschmerzen.

Häufige Nebenwirkungen:

Infektionen der oberen Atemwege, Harnwegsinfektion, Gewichtsverlust, verringerter Appetit, Schlafstörungen, Schwindel, Schläfrigkeit, Geschmacksstörungen, Hitzewallungen, Kurzatmigkeit, Husten (mit Auswurf), Erbrechen, Bauchschmerzen und ?beschwerden, Magenbeschwerden, Sodbrennen, Verstopfungsgefühl und Blähungen, Magenschleimhautentzündungen, Juckreiz, Hautrötung oder gerötete Haut, trockene Haut, Hautausschlag, schmerzende Gelenke/Gelenkschmerzen oder Schwächegefühl, Muskelschmerzen, Energielosigkeit, Brustkorbschmerzen, Sonnenempfindlichkeitsreaktionen.

Gelegentliche Nebenwirkungen:

Schwellung von Gesicht, Lippen und/oder Zunge.

Informieren Sie Ihren Arzt/Ihre Ärztin, Ihren Apotheker/Ihre Apothekerin oder das medizinische Fachpersonal, wenn Sie eine der oben genannten Nebenwirkungen bei sich feststellen.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

Arzneimittel für Kinder unzugänglich aufbewahren.

Esbriet darf nur bis zu dem auf dem Flaschenetikett mit «EXP» bezeichneten Datum verwendet werden.

Nicht über 30 °C lagern.

Jede Kapsel enthält als Wirkstoff 267 mg Pirfenidon.

Ausserdem Croscarmellose-Natrium (aus gentechnisch veränderter Baumwolle hergestellt) und weitere Hilfsstoffe zur Herstellung von Kapseln.

Die Esbriet Hartkapseln sind weisse Kapseln zu 267 mg Pirfenidon.
