Exelon ist ein Arzneimittel mit dem Wirkstoff Rivastigmin. Dieser Wirkstoff gehört zur Arzneimittelgruppe der sogenannten Cholinesterasehemmer.
Exelon wird zur Behandlung einer relevanten Abnahme der geistigen Leistungsfähigkeit (z.B. Lernschwierigkeiten, Vergesslichkeit, Verständnis und Orientierung) und Demenz verwendet, wie sie bei der Alzheimer-Krankheit oder bei einem Teil der Patienten bzw. Patientinnen mit der Parkinson-Krankheit beobachtet werden. Menschen mit dieser Erkrankung leiden an einem Acetylcholinmangel im Gehirn. Die Substanz Acetylcholin wird im Gehirn für die Aufrechterhaltung der geistigen Leistungsfähigkeit benötigt. Exelon wirkt, indem es die Acetylcholinkonzentration im Gehirn erhöht und somit die geistigen Funktionen und die Fähigkeit des Patienten und der Patientin zur Erledigung der Aktivitäten des täglichen Lebens verbessert. Auf diese Weise hilft Exelon, den bei der Alzheimer-Krankheit oder Parkinson-Krankheit auftretenden geistigen Leistungsabbau zu verzögern. Exelon wird aber die Krankheit nicht heilen können.
Exelon darf nur auf Verschreibung Ihres Arztes oder Ihrer Ärztin eingenommen werden.
Exelon darf nicht angewendet werden, wenn Sie
Exelon darf bei Kindern nicht angewendet werden.
Ihr Arzt oder Ihre Ärztin muss Sie besonders sorgfältig überwachen, sofern Sie an einer der folgenden Krankheiten leiden oder gelitten haben:
Herzrhythmusstörungen, Magengeschwür (Magenulcus), Asthma oder anderen Atemwegserkrankungen, Schwierigkeiten beim Wasserlösen, Krampfanfällen, Zittern, eingeschränkte Nieren- oder Leberfunktion, niedriges Körpergewicht (weniger als 50 kg).
Besondere Vorsicht ist geboten, falls es zu Magen-Darmbeschwerden wie Übelkeit, Erbrechen und Durchfall kommt. Langanhaltendes Erbrechen und Durchfall können zu Dehydration (starker Flüssigkeitsverlust) führen.
Informieren Sie sofort Ihren Arzt bzw. Ihre Ärztin, wenn Sie Hautentzündungen, Blasen und Hautanschwellungen bekommen, die sich verschlimmern oder ausbreiten.
Wenn Sie sich während der Behandlung mit Exelon einer Operation unterziehen müssen, sollten Sie Ihren Arzt oder Ihre Ärztin und den Narkosearzt oder Ihre Narkoseärztin davon in Kenntnis setzen. Exelon sollte rechtzeitig vor Beginn der Narkose abgesetzt werden.
Exelon sollte nicht gleichzeitig mit Arzneimitteln angewendet werden, die ähnliche (anticholinerge) Wirkungen haben wie Exelon. Solche Arzneimittel sind z.B. Arzneimittel gegen Magenkrämpfe oder Reisekrankheit. Exelon könnte diese Arzneimittel beeinträchtigen.
Besondere Vorsicht ist auch geboten, wenn Sie gleichzeitig bestimmte Arzneimittel gegen Schmerzen, Rheuma und Entzündungen einnehmen, da die Gefahr von Magenblutungen erhöht ist.
Exelon Patch sollte nicht gleichzeitig mit Metoclopramid (ein Arzneimittel gegen Übelkeit und Erbrechen) angewendet werden. Dies kann zu additiven Effekten, wie steife Glieder und zitternden Händen, führen.
Besondere Vorsicht ist geboten, wenn Exelon Patch zusammen mit Beta-blockern (Arzneimittel wie Atenolol, die verwendet werden um Bluthochdruck, Angina, und andere Herzerkrankungen zu behandeln) angewendet wird. Es kann zu additiven Effekten kommen, wie Bradykardie (langsamer Herzschlag), die zu einer Synkope (Ohnmacht, Bewusstseinsverlust) führen kann.
Wenn Sie Exelon während mehr als 3 Tagen nicht eingenommen haben, so teilen Sie dies Ihrem Arzt resp. Ihrer Ärztin mit, bevor Sie das Arzneimittel erneut einnehmen.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen. Ihr Arzt bzw. Ihre Ärztin wird Sie darüber informieren, inwieweit Ihre Krankheit die aktive Teilnahme am Strassenverkehr und das Bedienen von Maschinen zulässt.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie
Sie dürfen Exelon während der Schwangerschaft nur nach Rücksprache mit Ihrem Arzt oder Ihrer Ärztin einnehmen. In der Stillzeit darf Exelon nicht eingenommen werden.
Anwendungsart
Exelon wird in der Regel 2x täglich mit den Mahlzeiten, vorzugsweise mit dem Frühstück und dem Abendessen, eingenommen. Ihr Arzt oder Ihre Ärztin kann Ihnen bei Unverträglichkeit auch eine Aufteilung der täglich einzunehmenden Dosis in 3 Portionen verschreiben.
Um eine optimale Wirkung zu erzielen, muss das Arzneimittel jeden Tag eingenommen werden.
Lassen Sie sich von Ihrem Arzt bzw. Ihre Ärztin beraten, bevor Sie Exelon das erste Mal einnehmen. Ihr Arzt bzw. Ihre Ärztin wird Sie auf therapeutische Wirksamkeit regelmässig untersuchen und Ihr Gewicht sorgfältig beobachten.
Exelon Kapseln
Schlucken Sie die Kapseln unzerkaut mit etwas Flüssigkeit, ohne sie zu öffnen oder zu zerkleinern.
Exelon Orale Lösung
| 1. Nehmen Sie die Dosierspritze aus der Schutzhülle. Öffnen Sie die Flasche durch Niederdrücken und Drehen des kindergesicherten Verschlusses. |
| 2. Stecken Sie die Spitze der Dosierspritze in die Öffnung des weissen Stopfens. |
| 3. Entnehmen Sie durch Ziehen des Kolbens die Ihnen verschriebene Menge an Exelon orale Lösung zum Einnehmen aus der Flasche. |
| 4. Entfernen Sie gegebenenfalls grosse Blasen aus der Dosierspritze, indem Sie den Kolben einige Male auf- und abbewegen, bevor Sie die Dosierspritze mit der Ihnen verschriebenen Dosis von der Flasche abnehmen. Wenige kleine Bläschen sind ohne Bedeutung und beeinflussen die Dosierung nicht. |
| 5. Nehmen Sie die Exelon orale Lösung direkt aus der Dosierspritze oder geben Sie diese zunächst in ein kleines Glas mit kaltem Fruchtsaft oder einem Sprudelgetränk. Rühren Sie in diesem Fall die Mischung kurz um und trinken Sie sie innert 4 h vollständig. |
| 6. Wischen Sie die Aussenseite der Dosierspritze nach Gebrauch mit einem sauberen Tuch ab und stecken Sie sie in die Schutzhülle zurück. |
Die Dosierung wird für Sie von Ihrem Arzt oder Ihrer Ärztin festgelegt.
Sie werden die Behandlung mit je 0.75 ml (=1.5 mg) oraler Lösung resp. mit je 1 Kapsel zu 1.5 mg 1× mit dem Frühstück und 1x mit dem Abendessen beginnen. Je nachdem, wie Sie auf das Arzneimittel reagieren, wird Ihr Arzt bzw. Ihre Ärztin die Dosierung ca. alle 2 Wochen langsam erhöhen.
Die Höchstdosis liegt bei 12 mg (=6 ml) pro Tag.
Wenn Sie der Meinung sind, Sie hätten irrtümlicherweise zuviel Exelon eingenommen, informieren Sie Ihren Arzt bzw. Ihre Ärztin. Nach der Einnahme von zuviel Exelon kann es zu Übelkeit, Erbrechen oder Durchfall kommen.
Wenn Sie feststellen, dass Sie die Einnahme von Exelon einmal vergessen haben, warten Sie bis zum nächsten Einnahmezeitpunkt und führen das von Ihrem Arzt bzw. Ihrer Ärztin für Sie vorgesehene Behandlungsschema fort. Wenn Sie Exelon während mehr als 3 Tagen nicht eingenommen haben, so teilen Sie dies Ihrem Arzt bzw. Ihrer Ärztin mit, bevor Sie das Arzneimittel erneut einnehmen.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker, bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme von Exelon auftreten:
Sehr häufig (bei mehr als 1 von 10 Patienten) treten Magen-Darmbeschwerden wie Übelkeit, Appetitverlust, Erbrechen und Durchfall auf. Bei langanhaltendem Erbrechen oder Durchfall besteht die Gefahr einer Dehydration (starker Flüssigkeitsverlust). Informieren Sie in diesem Fall umgehend Ihren Arzt bzw. Ihre Ärztin. Ebenfalls sehr häufig treten Schwindel, Zittern, Unfallverletzungen auf.
Häufig (bei 1 bis 10 von 100 Patienten) treten Magenbrennen, Magenschmerzen, Durchfall, Kopfschmerzen, Unruhe, Ruhelosigkeit, Verwirrtheit, Ängstlichkeit, Schwächegefühl, Müdigkeit, Schläfrigkeit, Schlafstörungen, vermehrtes Schwitzen, allgemeines Unwohlsein, Gewichtsverlust, zu hohe Speichelproduktion, hoher Blutdruck (mit leichten Kopfschmerzen) und anormaler Gang auf. Bei Patienten bzw. Patientinnen mit Parkinson-Krankheit kann es zu einer Verschlechterung der Parkinsonsymptomatik (z.B. ungewöhnlich langsame Bewegungen, unkontrollierte Bewegungen des Mundes, der Zunge und der Extremitäten, Muskelsteifheit, Muskelschwäche) kommen.
Gelegentlich (bei 1 bis 10 von 1'000 Patienten) treten Schlaflosigkeit, Depression, Ohnmacht, anormale Körperhaltung mit unkontrollierten Bewegungen, Herzrhythmusstörungen (beides schnell und langsam) und veränderte Leberwerte auf.
Selten (bei 1 bis 10 von 10'000 Patienten) erlitten Patienten oder Patientinnen Brustschmerz, Krampfanfälle, Herzinfarkt, Hautausschläge, Juckreiz, Magen- und Darmulkus.
Sehr selten (bei 1 von 10'000 Patienten) wurden Halluzinationen, Magen-Darmblutung (Blut im Stuhl oder beim Erbrechen) oder Entzündung der Bauchspeicheldrüse (starke Schmerzen im Oberbauch, oft mit Übelkeit und Erbrechen) beobachtet. Ebenfalls sehr selten kann es zu Harnwegsinfekten kommen.
Wenn jedoch eine der unerwünschten Wirkungen über längere Zeit anhält oder im Falle von schwerer Unverträglichkeit, sollten Sie sich von Ihrem Arzt bzw. Ihrer Ärztin beraten lassen.
Sehr selten erlitten Patienten oder Patientinnen in Zusammenhang mit sehr heftigem Erbrechen einen Riss der Speiseröhre, was mit, blutigem Erbrechen und nachfolgender zunehmender Atemerschwernis einhergehen kann. Sollten solche Anzeichen auftreten, kontaktieren Sie unverzüglich Ihren Arzt oder Ihre Ärztin.
In einzelnen Fällen wurde über Lebererkrankungen, (gelb verfärbte Haut und Augen, anormale dunkle Verfärbung des Urins, unerwartetes Erbrechen, Müdigkeit, Appetitverlust) Hautentzündungen, Blasen oder Hautanschwellungen, extrapyramidale Symptome (steife Glieder und zitternde Hände), Zittern und Dehydration berichtet.
Sollten solche Anzeichen auftreten, kontaktieren Sie unverzüglich Ihren Arzt oder Ihre Ärztin, da Sie möglicherweise ärztliche Hilfe benötigen.
Ebenso wurde über Aggressivität berichtet.
Bei der Anwendung von Exelon als Pflaster wurden häufig Harninkontinenz (ungewollter Harnverlust) und Hyperaktivität beobachtet.
Ein Bestandteil der Exelon orale Lösung ist das Natriumbenzoat. Die Benzoesäure kann zur Irritationen von Haut, Augen und Schleimhaut führen.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Ihren Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Dieses Arzneimittel wurde Ihnen von Ihrem Arzt bzw. Ihrer Ärztin verschrieben. Sie dürfen es niemandem weitergeben und auch nicht für eine andere Krankheit einnehmen.
Arzneimittel für Kinder unerreichbar aufbewahren.
Exelon Kapseln nicht über 30 °C aufbewahren.
Exelon Orale Lösung in der Originalverpackung in aufrechter Position, nicht über 30 °C aufbewahren. Nicht im Kühlschrank lagern und nicht einfrieren!
Wenn die orale Lösung zur Einnahme mit kaltem Fruchtsaft oder einem Sprudelgetränk gemischt wird, ist die Mischung bei Raumtemperatur 4 Stunden haltbar.
Nach Anbruch der oralen Lösung soll die Flasche innerhalb eines Monats aufgebraucht werden.
Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.

1 ml Exelon Orale Lösung enthält: 2 mg Rivastigmin (Wirkstoff), als Hydrogentartrat, das Konservierungsmittel Natriumbenzoat (E 211), den Farbstoff Chinolingelb (E 104) sowie weitere Hilfsstoffe.
