+-Was ist Imurek und wann wird es angewendet?

Imurek enthält den Wirkstoff Azathioprin. Dieser hemmt Zellen in Blut und Gewebe, welche der Körperabwehr dienen. Imurek wird, auf Verschreibung des Arztes bzw. der Ärztin, in folgenden Situationen verwendet:

- nach Organtransplantationen, u.a. der Niere, zur Verhinderung einer Abstossung

- bei chronischer Polyarthritis und verschiedenen Krankheiten des Bindegewebes, der Haut, der Blutzellen, der Blutgefässe oder der Leber, welche durch eine gestörte Körperabwehr verursacht werden.

Imurek wird zu diesem Zweck häufig mit anderen Arzneimitteln, z.B. mit Kortikosteroiden (dem körpereigenen Cortisol verwandten Hormonen) kombiniert.

+-Wann darf Imurek nicht eingenommen werden?

Imurek darf nicht angewendet werden bei Patientinnen und Patienten mit bekannter Überempfindlichkeit auf den Wirkstoff Azathioprin, auf andere Inhaltsstoffe von Imurek sowie auf 6-Mercaptopurin.

+-Wann ist bei der Einnahme von Imurek Vorsicht geboten?

Imurek darf grundsätzlich nur unter strenger Kontrolle Ihres Arztes bzw. Ihrer Ärztin eingenommen werden. Er/sie wird vor allem das Blutbild regelmässig kontrollieren.

Dies gilt besonders, wenn weitere Krankheiten wie Nieren- und/oder Lebererkrankungen, ein angeborener Mangel am Enzym Thiopurinmethyltransferase oder eine Verminderung des Enzyms Hypoxanthin-Guanin-Phosphoribosyl-Transferase (Lesch-Nyhan-Syndrom) vorliegen, oder wenn Sie noch andere Arzneimittel einnehmen müssen. Beispielsweise ist bei gleichzeitiger Einnahme eines Arzneimittels, das Allopurinol enthält (zur Senkung erhöhter Harnsäure im Blut), die Dosierung von Imurek erheblich zu verringern. Vorsicht ist auch geboten, wenn Sie Arzneimittel zur Behandlung von entzündlichen Darmerkrankungen (Olsalazin, Mesalazin, Sulfasalazin) oder Arzneimittel, die das Blutbild schädigen, einnehmen.

Infektionen wie Windpocken oder Gürtelrose können bei der Verabreichung von Arzneimitteln wie Imurek, welche Reaktionen des Immunsystems unterdrücken, einen ernsten Verlauf nehmen. Daher sollten Sie den Kontakt mit Personen meiden, die an Windpocken oder Gürtelrose erkrankt sind, und Ihren Arzt bzw. Ihre Ärztin informieren, falls Sie selber jemals eine dieser Erkrankungen durchgemacht haben.

Sie sollten auch Ihren Arzt bzw. Ihre Ärztin informieren, wenn Sie eine Hepatitis B Infektion hatten, da ein Risiko zur Reaktivierung der Infektion besteht.

Patientinnen und Patienten, die Arzneimittel einnehmen, welche das Immunsystem beeinträchtigen, können ein erhöhtes Risiko für die Entstehung von Tumoren einschliesslich Hautkrebs tragen. Deshalb sollten Sie sich während der Einnahme von Imurek der Sonne nicht übermässig aussetzen. Schützen Sie sich mit entsprechender Kleidung und Sonnencrème mit hohem Lichtschutzfaktor.

Imurek kann wegen seiner hemmenden Wirkung auf das Immunsystem dazu führen, dass Impfungen wirkungslos bleiben oder Impfungen mit Lebendimpfstoffen eine Infektion auslösen. Während der Therapie mit Imurek sollen deshalb keine Impfungen mit Lebendimpfstoffen durchgeführt werden.

Aufgrund der möglichen unerwünschten Wirkungen wie Schwindel und Übelkeit ist beim Lenken von Fahrzeugen und Bedienen von Maschinen Vorsicht geboten.

Bei Patientinnen und Patienten, welche Imurek zur Verhinderung der Abstossungsreaktion nach Nierentransplantation erhalten, kann durch die wiederhergestellte Nierenfunktion auch die Fruchtbarkeit wiedererlangt werden und es besteht die Möglichkeit einer Schwangerschaft. Betreffend Schwangerschaft beachten Sie auch die Angaben in der Rubrik «Darf Imurek während der Schwangerschaft oder in der Stillzeit eingenommen werden?».

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.

+-Darf Imurek während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Eine Behandlung mit Imurek sollte bei einer geplanten oder bestehenden Schwangerschaft nicht begonnen werden. Ob jedoch eine Therapie mit Imurek weitergeführt werden soll, wenn eine Frau schwanger wird, oder ob die Schwangerschaft abzubrechen ist, hängt von der zu behandelnden Krankheit ab. Der Nutzen für die Mutter muss im Gespräch mit dem behandelnden Arzt bzw. der behandelnden Ärztin gegenüber dem möglichen Risiko für das Kind abgewogen werden. Besprechen Sie auf jeden Fall eine geplante Schwangerschaft mit Ihrem Arzt bzw. Ihrer Ärztin.

Sowohl weibliche als auch männliche Patienten im fortpflanzungsfähigen Alter sollten während der Einnahme von Azathioprin und bis mindestens 3 Monate nach Therapieende empfängnisverhütende Massnahmen treffen.

Ein Abbauprodukt von Azathioprin konnte in der Muttermilch nachgewiesen werden. Während der Behandlung mit Imurek soll nicht gestillt werden.

+-Wie verwenden Sie Imurek?

Imurek sollte streng nach Vorschrift des Arztes bzw. der Ärztin eingenommen werden. Er/sie wird die Dosierung entsprechend der Krankheit und dem Körpergewicht individuell anpassen; dies gilt auch für die Anwendung von Imurek bei Kindern und Jugendlichen. Um Übelkeit zu vermeiden, werden die Filmtabletten mit Flüssigkeit am besten nach einer Mahlzeit eingenommen. Die Einnahme erfolgt im Allgemeinen einmal täglich oder zweimal täglich.

Die Filmtabletten müssen ganz geschluckt werden und dürfen nicht geteilt oder zerstossen werden. Waschen Sie unmittelbar nach dem Berühren der Filmtabletten die Hände. Achten Sie darauf, dass Sie allfällige Tablettenteilchen (z.B. bei einer Beschädigung einer Filmtablette) nicht einatmen und dass diese nicht mit der Haut oder Schleimhaut (Augen!) in Kontakt kommen. Falls es dennoch zu einem Hautkontakt kommt, waschen Sie die Stelle mit Wasser und Seife, bei Augenkontakt spülen Sie gründlich mit Wasser.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Imurek haben?

Folgende Nebenwirkungen können bei der Einnahme von Imurek auftreten:

allgemeines Unwohlsein, Schwindel, Übelkeit, Erbrechen, Durchfall, Fieber, Muskelstarre, Muskelschmerzen, Hautausschläge, Gelenkbeschwerden, Gefässentzündungen, Nierenstörungen, Leberstörungen, Gelbsucht, Bauchspeicheldrüsen-Entzündung, Herzrhythmusstörungen und niedriger Blutdruck. In sehr seltenen Fällen sind schwere Hautreaktionen mit Blasenbildung und/oder Ablösen der Haut möglich. Sollten Sie insbesondere Zeichen einer Leberstörung/Gelbsucht (wie Gelbfärbung der Haut und des weissen Bereiches der Augen, dunkler Urin, heller Stuhl, Abgeschlagenheit) oder deutliche Hautreaktionen beobachten, so informieren Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin!

Es besteht ein erhöhtes Risiko für die Entstehung von Tumoren einschliesslich Hautkrebs und für das Auftreten von Infektionen wie Windpocken und Gürtelrose mit ernstem Verlauf (siehe auch unter «Wann ist bei der Einnahme von Imurek Vorsicht geboten?»).

Nach einer Nierentransplantation kann es häufig zu gesteigertem Haarausfall kommen.

Ferner kann Imurek das Blutbild schädigen. Sie sollten sich sofort bei Ihrem Arzt bzw. Ihrer Ärztin melden, wenn eine Infektion, Fieber, blaue Flecken am Körper oder eine Blutung auftreten!

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie ebenfalls Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Imurek soll nicht über 25 °C, vor Licht geschützt in der Originalpackung und ausser Reichweite von Kindern aufbewahrt werden. Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.

Bringen Sie nicht gebrauchte oder beschädigte Filmtabletten Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin zur fachgerechten Entsorgung zurück.
