+-Was ist Jakavi und wann wird es angewendet?

Jakavi enthält den Wirkstoff Ruxolitinib und ist ein Arzneimittel zur Behandlung erwachsener Patienten mit Myelofibrose oder mit Polycythaemia vera.

Myelofibrose ist eine Erkrankung des Knochenmarks, bei der das Knochenmark durch Narbengewebe ersetzt wird. Das gestörte Knochenmark kann nicht mehr in ausreichendem Masse Blutzellen herstellen, was zu einer deutlich vergrösserten Milz und Symptomen wie Nachtschweiss, Knochenschmerzen, Bauchschmerzen und Gewichtsverlust führt. Durch Jakavi wird bei Patienten mit Myelofibrose die Milzgrösse reduziert und die Symptome gelindert.

Polycythaemia vera ist eine Erkrankung des Knochenmarks, bei der das Knochenmark zu viele rote Blutkörperchen produziert, wodurch das Blut zähflüssiger wird und das Thromboserisiko stark ansteigt. Die gestörte Produktion der Blutzellen führt wie bei der Myelofibrose zu einer vergrösserten Milz und Symptomen wie Nachtschweiss, Juckreiz, Knochenschmerzen, Bauchschmerzen und Gewichtsverlust. Durch die Hemmung der Wirkung bestimmter Enzyme (sogenannte Januskinasen) kann Jakavi die Anzahl roter Blutkörperchen senken und das Risiko schwerwiegender Blut- oder Gefässkomplikationen verringern. Ausserdem kann Jakavi die Milzgrösse reduzieren und die anderen Symptome der Polycythaemia vera lindern.

+-Was sollte dazu beachtet werden?

Vor Beginn Ihrer Behandlung mit Jakavi wird Ihr Arzt bzw. Ihre Ärztin Blutuntersuchungen durchführen, um Ihre Anfangsdosis festzulegen.

Auch während der Behandlung mit Jakavi werden bei Ihnen Blutuntersuchungen durchgeführt, um die Anzahl der Blutzellen (rote und weisse Blutkörperchen sowie Blutplättchen) in Ihrem Körper zu überwachen. Auf diese Weise kann Ihr Arzt bzw. Ihre Ärztin beurteilen, wie Sie auf die Behandlung ansprechen und ob Jakavi einen unerwünschten Einfluss auf diese Zellen hat. Je nach Ergebnis dieser Untersuchungen kann es nötig sein, dass Ihr Arzt bzw. Ihre Ärztin die Dosis von Jakavi anpasst oder Ihre Behandlung mit Jakavi unterbricht.

+-Wann darf Jakavi nicht eingenommen werden?

Sie sollten Jakavi nicht einnehmen, wenn Sie allergisch (überempfindlich) gegen Ruxolitinib oder einen der anderen Inhaltsstoffe von Jakavi sind, wie z.B. Milchzucker (Laktose).

+-Wann ist bei der Einnahme von Jakavi Vorsicht geboten?

Besondere Vorsicht bei der Einnahme von Jakavi ist erforderlich wenn:

  • unerwartete Blutergüsse und/oder Blutungen auftreten, Sie ungewöhnlich müde sind, Sie bei Anstrengung oder in Ruhe kurzatmig sind, Sie blass aussehen oder häufig Infektionen auftreten (Anzeichen für Störungen des Blutes).
  • Anzeichen einer Infektion auftreten oder sich ein schmerzhafter Hautausschlag mit Blasenbildung entwickelt (Anzeichen für eine Gürtelrose).
  • chronischer Husten mit blutig gefärbtem Auswurf, Fieber, nächtliche Schweissausbrüche und Gewichtsverlust auftreten (Anzeichen für Tuberkulose).
  • Sie eines der folgenden Symptome haben oder jemand in Ihrem Umfeld bemerkt, dass Sie eines dieser Symptome haben: Verwirrtheit oder Denkschwierigkeiten, Verlust des Gleichgewichts oder Gehschwierigkeiten, schwerfällige Bewegungen, Sprachschwierigkeiten, verminderte Kraft oder Schwäche auf einer Körperseite, verschwommenes Sehen oder Sehverlust (Anzeichen für progressive, multifokale Leukenzephalopathie).
  • Sie Hautveränderungen bemerken. Dies erfordert möglicherweise weitere Beobachtung, da von gewissen Arten von Hautkrebs (nicht schwarzer Hautkrebs) berichtet worden ist.

Falls während der Behandlung mit Jakavi eines der oben genannten Symptome auftritt, müssen Sie dies umgehend Ihrem Arzt bzw. Ihrer Ärztin mitteilen.

Informieren Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin, wenn Sie

  • an anderen Krankheiten leiden,
  • jemals an Tuberkulose oder Hautkrebs erkrankt waren,
  • Allergien haben oder
  • andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden!

Es ist besonders wichtig, dass Sie die Einnahme folgender Arzneimittel Ihrem Arzt bzw. Ihrer Ärztin mitteilen. Er bzw. sie wird die Dosis von Jakavi eventuell anpassen müssen:

  • einige Arzneimittel zur Behandlung von Infektionen. Diese sind u.a. Arzneimittel zur Behandlung von Pilzinfektionen (z.B. Ketoconazol, Itraconazol, Posaconazol und Voriconazol), bestimmte Antibiotika (z.B. Clarithromycin oder Telithromycin) oder Arzneimittel zur Behandlung von viralen Infektionen inklusive AIDS (z.B. Atazanavir, Indinavir, Nelfinavir, Ritonavir, Saquinavir).
  • Nefazodon, ein Arzneimittel zur Behandlung von Depressionen.

Während der Behandlung mit Jakavi sollten Sie nie mit der Einnahme eines neuen Arzneimittels beginnen, ohne dies zuerst mit dem Arzt bzw. der Ärztin zu besprechen, der/die Ihnen Jakavi verschrieben hat.

+-Darf Jakavi während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Schwangerschaft

Jakavi soll nicht während der Schwangerschaft eingenommen werden, es sei denn, dies ist unbedingt notwendig. Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie schwanger sind oder glauben schwanger zu sein. Ihr Arzt bzw. Ihre Ärztin wird mit Ihnen die möglichen Risiken einer Einnahme von Jakavi während der Schwangerschaft besprechen. Frauen im gebärfähigen Alter, die Jakavi erhalten, wird zu einer wirksamen Empfängnisverhütung während der Behandlung geraten.

Stillzeit

Es ist nicht bekannt, ob Jakavi in die Muttermilch ausgeschieden wird. Während der Behandlung mit Jakavi sollten Sie deshalb nicht stillen.

+-Wie verwenden Sie Jakavi?

Befolgen Sie die Anweisungen Ihres Arztes bzw. Ihrer Ärztin sorgfältig.

Ihr Arzt bzw. Ihre Ärztin wird Ihnen genau sagen, wie viele Tabletten Jakavi Sie einnehmen müssen.

Nehmen Sie Jakavi zweimal täglich ein. Es ist wichtig, dass Sie Jakavi jeden Tag etwa zu den gleichen Zeiten einnehmen, um eine gleichmässige Konzentration von Jakavi im Blut zu erreichen. Die Einnahme kann mit oder ohne Nahrung erfolgen. Schlucken Sie die Tabletten ganz mit einem grossen Glas Wasser.

Wenn Sie dialysiert werden, sollten Sie eine einzelne Erstdosis von Jakavi vor und die weiteren einzelnen Dosen je direkt nach der Dialyse einnehmen. Ihr Arzt bzw. Ihre Ärztin wird Ihnen mitteilen, wie hoch die Dosis ist, die Sie vor und nach der Dialyse einnehmen sollen.

Wie lange nehmen Sie Jakavi ein: Sie sollten Jakavi so lange einnehmen, wie Ihr Arzt bzw. Ihre Ärztin es Ihnen empfohlen hat. Jakavi ist eine Langzeitbehandlung. Ihr Arzt bzw. Ihre Ärztin wird Ihren Zustand regelmässig überwachen, um sicherzustellen, dass die Behandlung den gewünschten Effekt hat.

Wenn Sie mehr Jakavi einnehmen, als Sie sollten: Wenn Sie versehentlich mehr Jakavi einnehmen als Ihr Arzt bzw. Ihre Ärztin Ihnen verschrieben hat, informieren Sie so rasch wie möglich Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin.

Wenn Sie die Einnahme von Jakavi vergessen: Nehmen Sie keine doppelte Dosis von Jakavi um die vergessene Dosis aufzuholen, sondern nehmen Sie einfach die nächste Dosis zur gewohnten Zeit ein.

Wenn Sie weitere Fragen zur Verwendung oder Einnahmedauer von Jakavi haben, fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin.

Kinder und Jugendliche: Jakavi ist nicht zur Anwendung bei Kindern und Jugendlichen vorgesehen, da Anwendung und Sicherheit von Jakavi bei Kindern und Jugendlichen bisher nicht geprüft worden ist.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt, Apotheker bzw. mit Ihrer Ärztin, Apothekerin.

+-Welche Nebenwirkungen kann Jakavi haben?

Folgende Nebenwirkungen können bei der Einnahme von Jakavi auftreten:

Sehr häufig (betreffen mehr als 1 von 10 Patienten)

  • Harnwegsinfekte,
  • Niedrige Anzahl roter Blutkörperchen (Anämie), niedrige Anzahl weisser Blutkörperchen (Neutropenie) oder niedrige Anzahl der Blutplättchen (Thrombozytopenie),
  • Hoher Cholesterinspiegel (Hypercholesterinämie) oder hoher Triglyzeridspiegel (Hypertriglyzeridämie) im Blut,
  • Schwindel,
  • Kopfschmerzen,
  • Blutergüsse,
  • Gewichtszunahme.

Häufig (betreffen zwischen 1 und 10 von je 100 Patienten)

  • Gürtelrose (Herpes zoster),
  • Chronischer Husten mit blutig gefärbtem Auswurf, Fieber, nächtliche Schweissausbrüche und Gewichtsverlust (Anzeichen für Tuberkulose),
  • Übermässige Gasbildung im Darm (Flatulenz),
  • Verstopfung,
  • Hoher Blutdruck (Hypertonie); dies kann auch die Ursache von Schwindel und Kopfschmerzen sein.

Die meisten Nebenwirkungen sind leicht bis mässig ausgeprägt und verschwinden im Allgemeinen nach wenigen Tagen oder Wochen während der Behandlung.

Wenn eine dieser Nebenwirkungen in erheblichem Masse auftritt, informieren Sie Ihren Arzt bzw. Ihre Ärztin.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin informieren.

+-Was ist ferner zu beachten?

Arzneimittel sind für Kinder unerreichbar aufzubewahren.

In der Originalpackung, vor Licht und Feuchtigkeit geschützt und bei Raumtemperatur (15-25 °C) lagern.

Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
