+-Was ist Jurnista und wann wird es angewendet?

Jurnista gehört zu einer Gruppe von Arzneimitteln, die als Analgetika oder Schmerzmittel bezeichnet werden.

Jurnista wird zur Behandlung mittelstarker bis starker chronischer Schmerzen verordnet.

Die Jurnista-Tabletten werden in einer retardierten Form hergestellt. Das heisst, dass der Wirkstoff über einen bestimmten Zeitraum hinweg schrittweise in Ihrem Körper freigesetzt wird, nachdem Sie eine Tablette eingenommen haben.

Jurnista darf nur auf Verschreibung und unter regelmässiger Kontrolle des Arztes/der Ärztin verwendet werden.

+-Was sollte dazu beachtet werden?

Die Retardtablette löst sich nach Abgabe der gesamten Wirkstoffmenge nicht vollständig auf, deswegen kann die Tablettenhülle manchmal im Stuhl erscheinen. Das ist kein Grund zur Beunruhigung.

+-Wann darf Jurnista nicht angewendet werden?

Jurnista darf nicht eingenommen werden,

  • wenn Sie überempfindlich (allergisch) gegen Hydromorphonhydrochlorid oder einen der Tablettenhilfsstoffe von Jurnista sind;
  • wenn bei Ihnen eine schwere Verengung des Magens und/oder des Darms diagnostiziert worden ist;
  • bei schweren Kopfverletzungen und erhöhtem Hirndruck;
  • bei krampfartigen Störungen (Epilepsie/Konvulsion);
  • bei schwerwiegenden Reaktionen auf einen Alkoholentzug (auch «Delirium tremens» genannt);
  • bei stark verminderter Leberfunktion;
  • bei ausgeprägter Beeinträchtigung der Atemfunktion, wie z.B. schwerem akutem Asthma;
  • bei gestörter Magen-Darm-Funktion;
  • wenn Ihr Darm nach einer Operation möglicherweise «blinde Schlingen» aufweist;
  • bei Verwendung von Arzneimitteln aus der Gruppe der Monoaminoxidase-Inhibitoren (dies sind gewisse Arzneimittel gegen Depressionen oder Parkinson-Krankheit) oder innert 2 Wochen nach Beendigung deren Einnahme;
  • bei Kindern und Jugendlichen unter 18 Jahren.

Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn eines oder mehrere dieser Kriterien auf Sie zutreffen.

Jurnista darf nicht bei Säuglingen oder Kindern, Frühgeborenen oder Frauen mit vorzeitigen Wehen angewendet werden.

+-Wann ist bei der Einnahme von Jurnista Vorsicht geboten?

Manche Menschen müssen bei der Einnahme von Jurnista besonders vorsichtig sein.

Fragen Sie Ihren Arzt oder Ihre Ärztin um Rat, wenn Sie an folgenden Symptomen oder Erkrankungen leiden oder in letzter Zeit gelitten haben:

  • Probleme mit der Lunge oder beim Atmen;
  • bei Einnahme anderer Opioidanalgetika;
  • Probleme mit dem Herzen;
  • bestimmten Formen von tiefem Blutdruck;
  • Probleme mit der Leber oder den Nieren;
  • Kopfschmerzen oder eine Kopfverletzung;
  • chronische Verstopfung (Obstipation);
  • Erkrankungen des Darms, einschliesslich Darmverschluss oder entzündliche Darmerkrankung;
  • Pankreatitis (Entzündung der Bauchspeicheldrüse) oder ? Erkrankungen der Gallengänge;
  • Alkohol- oder Medikamentenabhängigkeit;
  • Symptome einer Funktionsstörung des Zentralen Nervensystems ? einschliesslich extremer Schläfrigkeit, niedriger Körpertemperatur und manchmal Koma;
  • Unterfunktion der Schilddrüse (Hypothyreose);
  • Probleme mit der Nebenniere;
  • eine Vergrösserung der Prostata;
  • Probleme beim Harnlassen;
  • toxische Psychose (eine schwerwiegende geistige Erkrankung in Folge einer Infektion);
  • Kyphoskoliose (abnorme Verkrümmung der Wirbelsäule).

Ältere Patienten

Wenn Sie älter als 60 Jahre alt sind, können bei Ihnen möglicherweise eher Nebenwirkungen auftreten. Seien Sie daher besonders vorsichtig.

Verstopfung

Verstopfung (unzureichender oder harter Stuhlgang) ist eine übliche Nebenwirkung von Arzneimitteln wie Jurnista und es ist unwahrscheinlich, dass diese ohne Behandlung verschwindet. Sprechen Sie mit Ihrem Arzt oder Apotheker, resp. Ihrer Ärztin oder Apothekerin über die Anwendung von Laxantien (Mittel zur Behandlung der Verstopfung) und Stuhlweichmachern, um einer Verstopfung während der Einnahme von Jurnista vorzubeugen oder diese zu behandeln.

Wenn bei Ihnen eine Operation geplant ist

Wenn Sie sich in nächster Zeit einer Operation unterziehen, teilen Sie bitte dem Arzt oder der Ärztin im Krankenhaus mit, dass Sie Jurnista einnehmen. Jurnista sollte bei Ihnen gegen akute Schmerzen oder innerhalb von 24 Stunden vor/nach der Operation nicht angewendet werden.

Einnahme von Jurnista mit anderen Arzneimitteln

Manche Arzneimittel können die Wirkungsweise von Jurnista beeinflussen oder die Wahrscheinlichkeit eines Auftretens von Nebenwirkungen erhöhen.

Die Einnahme von Jurnista ist nicht vorgesehen/empfohlen, wenn Sie die folgenden Arzneimittel einnehmen oder anwenden:

  • morphinähnliche Schmerzmittel (Buprenorphin, Nalbuphin Oxycodon, Fentanyl).

Wenn Sie folgende Arzneimittel einnehmen, sollten Sie vor der Einnahme von Jurnista Ihren Arzt oder Ihre Ärztin um Rat fragen:

  • jegliche Arzneimittel, die Sie müde, schläfrig oder benommen machen (wie zum Beispiel Schlaftabletten oder Beruhigungsmittel);
  • Präparate zur Muskelentspannung (diese können bei Rückenschmerzen verschrieben werden).

Einnahme von Jurnista zusammen mit Nahrungsmitteln und Getränken

Gleichzeitiger Einnahme von Alkohol und Jurnista kann zu verstärkter Schläfrigkeit oder vermehrten Nebenwirkungen wie schwaches Atmen mit Risiko des Atemstillstandes und Bewusstlosigkeit führen. Trinken Sie deshalb während Behandlung mit Jurnista keinen Alkohol.

Es spielt keine Rolle, ob Sie Ihre Jurnista-Retardtablette mit dem Essen einnehmen oder nicht.

Schmerzmittel, die als Wirksubstanz Hydromorphon oder einen anderen opiatähnlichen Stoff enthalten, können bei nicht vorschriftsgemässer oder missbräuchlicher Anwendung eine Abhängigkeit erzeugen. Es ist deshalb wichtig, dass Sie Jurnista nur auf Verschreibung Ihres Arztes/Ihrer Ärztin und genau nach seinen/ihren Anweisungen einnehmen. Bei Absetzen der Therapie sind die Anweisungen des Arztes/der Ärztin sorgfältig zu beachten (z.B. stufenweises Absetzen).

Bei missbräuchlichen parenteraler Anwendung (intravenös) können die Tablettenhilfsstoffe zu tödlichen Komplikationen führen. Jurnista darf nicht intravenös verabreicht werden. Diese Hilfsstoffe sind bei ordnungsgemässer oraler Einnahme unbedenklich.

Verkehrstüchtigkeit und das Bedienen von Maschinen

Jurnista kann zu Schläfrigkeit und Benommenheit führen. Vergewissern Sie sich, dass dies bei Ihnen nicht der Fall ist, bevor Sie Auto fahren, Maschinen bedienen oder gefährliche Arbeiten ausführen. Seien Sie besonders vorsichtig, wenn die Dosierung oder die Art Ihrer Medikation geändert wird.

Die Verwendung von Hydromorphon bei Sportlern wird als Doping eingestuft und führt zur Disqualifikation.

Dieses Arzneimittel enthält Lactose. Bitte nehmen Sie Jurnista daher erst nach Rücksprache mit Ihrem Arzt oder Ihrer Ärztin ein, wenn Ihnen bekannt ist, dass Sie unter einer Unverträglichkeit gegenüber bestimmten Zuckern leiden.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen!

+-Darf Jurnista während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Jurnista soll während einer Schwangerschaft nicht eingenommen werden. Ihr Arzt/Ihre Ärztin kann allerdings Ausnahmen machen.

Sprechen Sie mit Ihrem Arzt oder Ihrer Ärztin bevor Sie Jurnista einnehmen, wenn Sie schwanger sind oder vorhaben, schwanger zu werden.

Nehmen Sie Jurnista nicht ein, wenn Sie stillen, denn der Wirkstoff kann in die Muttermilch übergehen.

+-Wie verwenden Sie Jurnista?

Grundsätzlich wird Ihr Arzt bzw. Ihre Ärztin die für Sie geeignete Retardtablette mit der nötigen Dosierung festlegen. Die Dosierung ist dabei abhängig von individuellen Faktoren, wie z.B. dem Ausmass der Schmerzen, Alter und Gewicht, dem Vorliegen gewisser Krankheiten usw.

Nehmen Sie Jurnista immer genau nach Anweisung des Arztes oder der Ärztin ein. Bitte fragen Sie bei Ihrem Arzt oder Apotheker resp. Ihrer Ärztin oder Apothekerin nach, wenn Sie sich nicht ganz sicher sind.

Falls vom Arzt oder der Ärztin nicht anders verordnet, ist die übliche Anfangsdosis für Jurnista 8 mg täglich, jeweils zur gleichen Tageszeit. Einige Patienten können von einer Startdosis von 4 mg täglich profitieren, um die Verträglichkeit zu verbessern. Falls Sie von einem anderen morphinähnlichen Schmerzmittel zu Jurnista wechseln, kann Ihnen Ihr Arzt oder Ihre Ärztin zu Beginn auch eine andere Dosierung verordnen.

Ihr Arzt bzw. Ihre Ärztin kann die Dosierung erhöhen, bis eine angemessene Schmerzlinderung eintritt. Die Dosierung sollte nicht häufiger als bei jeder vierten Dosis erhöht werden.

Einnahme Ihrer täglichen Retardtablette

Schlucken Sie Ihre Jurnista-Retardtablette unzerkaut mit einem Glas Wasser.

Sie dürfen die Retardtablette weder zerkauen noch zerbrechen oder zerdrücken ? andernfalls besteht die Gefahr einer Überdosierung, da das Arzneimittel auf diese Weise zu schnell in Ihrem Körper freigesetzt wird.

Versuchen Sie, die Jurnista-Retardtablette jeden Tag zur gleichen Zeit einzunehmen.

Jurnista sollte nicht häufiger als einmal innerhalb von 24 Stunden eingenommen werden.

Sie könnten in Ihrem Stuhl etwas sehen, das wie Ihre Jurnista-Retardtablette aussieht. Seien Sie nicht beunruhigt ? es handelt sich dabei lediglich um die äussere Tablettenhülle, die ihren Körper unverändert passiert. Dies bedeutet nicht, dass die Retardtablette nicht wirkt.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

Wenn Sie eine grössere Menge von Jurnista eingenommen haben, als Sie sollten, informieren Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin oder die nächstgelegene Krankenhaus-Notaufnahme.

Wenn möglich, geben Sie an, welche und wie viele Retardtabletten eingenommen wurden. Wenn Sie eine Überdosis eingenommen haben, fühlen Sie sich möglicherweise sehr schläfrig und haben Atemprobleme. Die Auswirkungen einer Überdosierung können sich noch verschlimmern ? mögliche Symptome sind unter anderem feuchtkalte Haut, Verengung der Pupillen, Absinken des Blutdrucks sowie Koma (Bewusstlosigkeit). Jemand, der eine hohe Überdosis eingenommen hat, kann aufhören zu atmen, einen Herzanfall haben und sterben.

Wenn Sie die Einnahme von Jurnista vergessen haben

Nehmen Sie keine zusätzlichen Retardtabletten oder die doppelte Dosis ein, wenn Sie die vorherige Einnahme vergessen haben. Fragen Sie Ihren Arzt oder Ihre Ärztin, was in diesem Falle zu tun ist.

Wenn Sie die Einnahme von Jurnista abbrechen

Wenn es an der Zeit ist, Ihre Behandlung mit Jurnista zu beenden, wird Ihr Arzt bzw. Ihre Ärztin die Dosierung schrittweise verringern ? üblicherweise indem die Dosis alle zwei Tage halbiert wird. Wenn die geringstmögliche Dosierung erreicht ist, wird Ihr Arzt bzw. Ihre Ärztin Ihnen sagen, wann Sie die Einnahme von Jurnista beenden können.

Bei manchen Menschen treten Entzugssymptome auf, wenn die Dosierung von Jurnista verringert wird.

Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie eines dieser Symptome bemerken:

  • Angstzustände oder Reizbarkeit;
  • Erweiterung Ihrer Pupillen;
  • Erröten oder Schwitzen;
  • grundloses Weinen;
  • Übelkeit oder Erbrechen;
  • Bauchschmerzen oder Durchfall;
  • Schmerzen in den Gelenken.
+-Welche Nebenwirkungen kann Jurnista haben?

Wie alle Arzneimittel kann Jurnista Nebenwirkungen haben, die aber nicht bei jedem auftreten müssen. Folgende Nebenwirkungen können bei der Einnahme von Jurnista auftreten:

Schläfrigkeit, Kopfschmerzen, Schwindel, Verstopfung, Übelkeit, Erbrechen und Kraftlosigkeit.

Häufige Nebenwirkungen

Diese können bei weniger als 10 von 100 der Personen auftreten, die Jurnista einnehmen:

Magenentzündung, Mangel an Körperwasser (Dehydratation), verminderter Appetit, Unfähigkeit zu schlafen, Angstgefühl, Depression, Verwirrtheit, Nervosität, Albträume, Unruhe, Halluzination, Stimmungsschwankung, Sedierung, reduzierte Wahrnehmung (Hypoästhesie), Ameisenlaufen, Zittern, Gedächtnisstörung, Verschwommensehen, Drehschwindel, Rötung des Gesichts, erhöhter Blutdruck, Schwierigkeiten beim Atmen, Durchfall, Bauchschmerzen, Mundtrockenheit, Verdauungsstörungen (Dyspepsie), verstärkter ösophagealer Reflux, Juckreiz, vermehrte Schweissbildung, Hautausschlag, Muskelkrämpfe, Rückenschmerzen, Gelenkschmerzen, Muskelschmerzen, Schmerzen in den Extremitäten, Störung des Blasenentleerung (Dysurie), Wasseransammlung (Ödem), Fieber, Schmerzen, Beschwerden im Brustraum, Schüttelfrost, Entzugssyndrom, Gewichtsabnahme, Sturz, Prellungen.

Seltene Nebenwirkungen

Diese können bei weniger als 1 von 1000 der Personen, die Jurnista einnehmen, auftreten:

Hormonale Unterfunktion der Keimdrüsen, Aggression, Konvulsionen, Hyperaktivität, Bewusstseinsstörungen, verstärkte Reflexe, Pupillenverengung oder extrem enge und starre Pupillen (Miosis),verlangsamter Herzschlag, zu schnelles Atmen (Hyperventilation), Perforation des Darmes, Analfissuren,  Bezoar, Entzündung des Zwölffingerdarmes, Darmverschluss, brennendes Gefühl auf der Haut, Gefühl von betrunken sein, Hangover erhöhte Blutamylase, verminderte Körpertemperatur, verminderte Testosteronspiegel im Blut.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Bewahren Sie die Retardtabletten nicht über 25 °C auf.

Bewahren Sie die Retardtabletten ausserhalb der Sicht- und Reichweite von Kindern auf.

Das Arzneimittel darf nur bis zu dem auf dem Blister mit «Exp.» bezeichneten Datum verwendet werden.

Wenden Sie Jurnista nicht an, wenn die Retardtabletten beschädigt sind.

Bringen Sie ungebrauchte Retardtabletten (in der Originalpackung) zur fachgerechten Entsorgung an die Abgabestelle (Arzt oder Apotheker resp. Ärztin oder Apothekerin) zurück.

1 Jurnista 4 mg Retardtablette enthält 4 mg Hydromorphonhydrochlorid (entsprechend 3.56 mg Hydromorphon) als Wirkstoff sowie Lactosemonohydrat, Polyethylenoxid, das Antioxidans Butylhydroxytoluol (E321) und weitere Hilfsstoffe zur Tablettenherstellung.

1 Jurnista 16 mg Retardtablette enthält 16 mg Hydromorphonhydrochlorid (entsprechend 14,2 mg Hydromorphon) als Wirkstoff sowie Lactosemonohydrat, Polyethylenoxid, das Antioxidans Butylhydroxytoluol (E321) und weitere Hilfsstoffe zur Tablettenherstellung.
