+-Was ist Medikinet MR und wann wird es angewendet?

Medikinet MR enthält den Wirkstoff Methylphenidat, ein zentralnervöses Stimulans. Medikinet MR (Modified Release = veränderte Freisetzung mit verlängerter Wirkung) ist für eine 1× tägliche Einnahme vorgesehen.

Medikinet MR ist indiziert zur Behandlung einer Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) bei Kindern ab 6 Jahren und Jugendlichen bis 18 Jahren als Teil eines Behandlungsprogramms, das auch psychologische, pädagogische und soziale Massnahmen umfasst.

Die Symptome von ADHS bestehen unter anderem in rasch nachlassender Aufmerksamkeit, Flüchtigkeitsfehlern, nicht zuhören können, Aufgaben nicht beenden, Anweisungen nicht ausführen, leicht ablenkbar und störbar sein, impulsiv, unruhig, zappelig sein, übermässig viel reden, zu unpassenden Gelegenheiten herumlaufen, andere stören oder unterbrechen und sich auf Dinge nur schwer konzentrieren können. Das Lernen ist beeinträchtigt. Das alles kann zu Schwierigkeiten im sozialen Umfeld, in der Schule, bei der Arbeit führen. Es kann sein, dass die Betroffenen nicht alle diese Symptome haben. Viele Menschen haben diese Symptome von Zeit zu Zeit, aber bei Patienten oder Patientinnen mit ADHS beeinträchtigen diese Symptome deren Alltagstätigkeiten und treten in mehr als einer Umgebung auf (d.h. zuhause, in der Schule, bei der Arbeit). ADHS kann sich bis in das Erwachsenenalter fortsetzen.

Nach einer gründlichen Untersuchung Ihres Kindes hat sich Ihr Arzt bzw. Ihre Ärztin für Medikinet MR entschieden.

Durch die Behandlung mit Medikinet MR können die Hauptsymptome des ADHS wie mässige bis starke Ablenkbarkeit, rasch nachlassende Aufmerksamkeit, Impulsivität, verstärkte motorische Aktivität und gestörtes Sozialverhalten gemildert werden.

Medikinet MR darf nur auf Verschreibung eines Arztes bzw. einer Ärztin mit Erfahrung in der Behandlung von Verhaltensstörungen bei Kindern und Jugendlichen und unter ihrer regelmässigen Kontrolle verwendet werden.

Abklärungen vor Behandlungsbeginn

Vor Beginn der Behandlung muss der Arzt/die Ärztin abklären, ob Ihr Kind an einer Herz-Kreislauf-Erkrankung leidet, die einer Behandlung mit Methylphenidat entgegensteht. Ausserdem misst der Arzt/die Ärztin bei Ihrem Kind vor der Behandlung und im Verlauf der Behandlung das Gewicht und die Grösse.

Medikinet MR ist für die 1× tägliche Einnahme vorgesehen.

+-Wann darf Medikinet MR nicht angewendet werden?

Patienten und Patientinnen dürfen Medikinet MR nicht einnehmen

  • wenn sie an ausgeprägten Angst-, Spannungs- und Erregungszuständen leiden, weil die Einnahme des Arzneimittels diese Zustände verschlimmern kann;
  • wenn sie unter einer schweren Depression mit depressiven Symptomen wie Traurigkeit, Minderwertigkeitsgefühlen und Hoffnungslosigkeit leiden; die abnorme Gedanken oder Visionen haben, abnorme Laute hören oder bei denen eine Psychose diagnostiziert wurde; oder in der Vorgeschichte Suizidneigungen gezeigt haben;
  • wenn sie unter Erkrankungen mit schizophrener Symptomatik leiden;
  • wenn sie übermässig Alkohol trinken, übermässig Arzneimittel oder Drogen nehmen oder abhängig davon sind oder wenn sie früher übermässig Alkohol getrunken, übermässig Arzneimittel zugeführt oder Drogen genommen haben oder abhängig waren;
  • wenn sie unter Magersucht leiden;
  • wenn bei Ihnen oder einem Familienmitglied das Tourette-Syndrom (Tics) diagnostiziert wurde;
  • wenn sie ein Glaukom (grüner Star) haben (erhöhter Augeninnendruck);
  • wenn sie unter einem Phäochromozytom (seltene Tumorerkrankung der Nebenniere) leiden;
  • wenn sie an einer Erkrankung der Schilddrüse (Hyperthyreose) leiden;
  • wenn sie an Herzrhythmusstörungen leiden;
  • wenn sie an Erkrankungen des Herzens (schwerer Bluthochdruck, Herzschwäche, Angina Pectoris, angeborene Herzfehler, Herzmuskelerkrankungen, Herzinfarkt oder Erkrankungen, die lebensbedrohliche Herzrhythmusstörungen auslösen können) leiden;
  • wenn sie an arteriellen Durchblutungsstörungen leiden;
  • wenn sie an Veränderungen der Blutgefässe des Kopfes leiden oder einen Schlaganfall erlitten haben;
  • wenn sie gegenwärtig sogenannte Monoaminoxidase-Hemmer (z.B. Arzneimittel gegen Depressionen mit dem Wirkstoff Moclobemid) einnehmen oder innerhalb der letzten 14 Tage vor der Behandlung mit Methylphenidat  eingenommen haben;
  • wenn sie gegen Medikinet MR oder einen der Inhaltsstoffe des Produkts allergisch sind;
  • Kinder unter 6 Jahren dürfen Medikinet MR nicht einnehmen.
  • Medikinet MR darf nicht eingenommen werden, um eine normale Müdigkeit zu beseitigen.
+-Wann ist bei der Einnahme von Medikinet MR Vorsicht geboten?

Teilen Sie dem Arzt bzw. der Ärztin mit, wenn Ihr Kind

  • neues oder verstärktes aggressives Verhalten zeigt;
  • Bewegungstics (schwer zu kontrollierende, wiederholte Muskelzuckungen beliebiger Körperteile) oder verbale Tics (schwer zu kontrollierende, wiederholte Äusserungen von Lauten oder Wörtern) hat;
  • abnorme Gedanken oder Visionen hat, abnorme Laute hört oder wenn bei Ihrem Kind eine Psychose diagnostiziert wurde;
  • Krampfanfälle (Epilepsie) oder anomale EEG-Befunde (Elektroenzephalogramme = Hirnscans) hat;
  • hohen Blutdruck hat. Der Blutdruck sollte kontrolliert werden, insbesondere bei Patienten bzw. Patientinnen mit hohem Blutdruck;
  • eine Verengung oder Blockierung des Verdauungskanals (Speiseröhre, Magen, Dünn- oder Dickdarm) hat;
  • Schwierigkeiten mit dem Schlucken von Kapseln hat;
  • Sehtrübung oder andere Sehstörungen hat;
  • an Galactoseunverträglichkeit, Lactosemangel oder Glucose-Galactose-Malabsorption (seltene Stoffwechselerkrankungen) leidet;
  • eine Herzkrankheit oder andere schwerwiegende Herzprobleme hat oder hatte.

Der Arzt bzw. die Ärztin wird Sie fragen, ob Ihr Kind andere Arzneimittel einnimmt oder ob Ihr Kind oder Ihre Familie andere Krankheiten haben (wie z.B. Herzkrankheiten). Es ist wichtig, dass Sie alle Informationen weitergeben, damit der Arzt bzw. die Ärztin entscheiden kann, ob Medikinet MR das richtige Arzneimittel für Ihr Kind ist. Der Arzt bzw. die Ärztin kann entscheiden, dass Ihr Kind weitere medizinische Untersuchungen benötigt, bevor es dieses Arzneimittel einnimmt.

Der Arzt bzw. die Ärztin wird bei Ihrem Kind bei jeder Dosisänderung den Blutdruck und die Herzfrequenz messen oder wenn er bzw. sie entscheidet, dass diese Messungen notwendig sind. Der Arzt bzw. die Ärztin wird zudem kontrollieren, ob Ihr Kind Probleme mit seiner Stimmung oder Gemütsverfassung oder ob es andere ungewöhnlichen Gefühle hat oder ob sich diese während der Zeit, in der es Medikinet MR eingenommen hat, verschlimmert haben.

Informieren Sie den Arzt oder Apotheker bzw. die Ärztin oder Apothekerin, wenn Ihr Kind an anderen Krankheiten leidet, Allergien hat oder andere Arzneimittel (auch von Ihnen selbstgekaufte) einnimmt oder äusserlich anwendet!

Bei lang andauernder Einnahme von Methylphenidat wurde bei Kindern ein verlangsamtes Wachstum (Gewichts- und/oder Grössenzunahme) beobachtet. Wenn Kinder in einem Alter sind, in dem sie noch wachsen, wird der Arzt bzw. die Ärztin ihre Grösse und ihr Gewicht sorgfältig überwachen. Wenn Kinder nicht wie erwartet weiter wachsen oder an Gewicht zunehmen, kann der Arzt bzw. die Ärztin die Behandlung mit Medikinet MR abbrechen.

Es gibt klinische Hinweise darauf, dass während der Verabreichung von Arzneimitteln mit dem Wirkstoff Methylphenidat vermehrt psychiatrische Störungen (einschliesslich Suchtverhalten und suizidales Verhalten) sowie Gewichts- und Appetitverlust auftreten.

Wird das Arzneimittel abgesetzt, kann es zu Entzugserscheinungen, Stimmungsschwankungen oder Überaktivität kommen.

Bei der Verwendung von Methylphenidat wurden Akkommodationsstörungen (Schwierigkeiten für die Umstellung von Fern- auf Nahsehen) und Sehstörungen (z.B. verschwommenes Sehen) beobachtet. Methylphenidat kann Benommenheit verursachen. Daher kann dieses Arzneimittel die Reaktionsfähigkeit, die Fahrtüchtigkeit und Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Ausserdem ist es nicht ratsam, während der Behandlung mit Medikinet MR Alkohol zu trinken.

Wenn andere Arzneimittel angewendet werden, kann Medikinet MR die Wirkung der anderen Arzneimittel beeinflussen oder Nebenwirkungen verursachen. Wenn eines der nachfolgend aufgeführten Arzneimittel verwendet wird, muss mit dem behandelnden Arzt bzw. der behandelnden Ärztin gesprochen werden, bevor Medikinet MR eingenommen werden darf:

  • Arzneimittel zur Behandlung von Depressionen und Arzneimittel zur Verhinderung von Epilepsie oder von Blutgerinnseln (Blutverdünner), weil Methylphenidat die Art und Weise, wie sie vom Körper verarbeitet werden, ungünstig beeinflussen kann;
  • Arzneimittel, die den Blutdruck erhöhen, wegen einer möglichen Verstärkung der Blutdruckerhöhung bei gleichzeitiger Verwendung mit Medikinet MR;
  • Arzneimittel mit dem Wirkstoff Clonidin, weil es Berichte über schwere Nebenwirkungen gibt, wenn Methylphenidat und Clonidin gleichzeitig verabreicht werden.

Wenn Sie sich nicht sicher sind, ob Arzneimittel, die Ihr Kind zu sich nimmt, oben in der Liste aufgeführt sind, fragen Sie den Arzt oder Apotheker bzw. die Ärztin oder Apothekerin, bevor Ihr Kind Medikinet MR einnimmt.

+-Darf Medikinet MR während der Schwangerschaft oder in der Stillzeit eingenommen werden?

Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn eine Schwangerschaft vorliegt oder Sie stillen.

Medikinet MR Kapseln dürfen während der Schwangerschaft nur auf ausdrücklichste ärztliche Anweisung eingenommen werden. Stillende Mütter sollten Medikinet MR nicht einnehmen.

+-Wie verwenden Sie Medikinet MR?

Die Dosierung und Einnahmezeiten von Medikinet MR werden entsprechend den individuellen Bedürfnissen vom Arzt oder der Ärztin festgelegt. Die genaue Dosierung soll zuerst mit Methylphenidat Tabletten ohne verlängerte Wirkung ermittelt werden. Die Gesamttagesdosis sollte 60 mg Methylphenidat (2 Medikinet MR 30 mg Kapseln) nicht überschreiten.

Medikinet MR (Kapseln für eine 1× tägliche Anwendung) soll immer nach dem Frühstück eingenommen werden. Medikinet MR soll mit etwas Wasser als ganze Kapsel geschluckt werden. Die Medikinet MR Kapsel oder ihr Inhalt darf nicht zerkaut, gepresst oder geteilt werden. Die Aufnahme von Methylphenidat aus den Medikinet MR Kapseln erfolgt bei Einnahme auf nüchternen Magen deutlich beschleunigt. Eine ausreichende, langanhaltende Wirkung ist dann nicht mehr gewährleistet, daher ist es besonders wichtig, dass die Einnahme nach dem Früh­stück erfolgt.

Bei Medikinet MR (Kapseln für eine 1× tägliche Anwendung) sollte eine ausgelassene Dosis nicht nachgeholt werden. Wenn Sie dazu Fragen haben, wenden Sie sich an Ihren Arzt oder Ihre Ärztin. Falls einmal aus Versehen mehr Kapseln eingenommen wurden, als vom Arzt oder Ihrer Ärztin verschrieben, suchen Sie unverzüglich Ihren Arzt oder Ihre Ärztin oder das nächstgelegene Krankenhaus auf.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Medikinet MR haben?

Die häufigsten unerwünschten Wirkungen von methylphenidathaltigen Arzneimitteln wie z.B. Medikinet MR sind:

Kopfschmerzen, Nervosität, Schlaflosigkeit, Gelenkschmerzen, Mundtrockenheit, hohe Körpertemperatur (Fieber), aussergewöhnlicher Haarausfall oder dünner werdendes Haar, aussergewöhnliche Schläfrigkeit oder Benommenheit, Appetitverlust oder verringerter Appetit, Juckreiz, Ausschlag oder juckender Hautausschlag mit Rötung (Nesselsucht), Husten, Halsschmerzen, Reizungen der Nase und des Rachens, hoher Blutdruck, schneller Herzschlag (Tachykardie), Schwindel, unkontrollierbare Bewegungen, Überaktivität, Aggressivität, Erregtheit, Ängstlichkeit, depressive Stimmung, Reizbarkeit und unnormales Verhalten, Magenschmerzen, Durchfall, Übelkeit, Unwohlsein im Magen und Erbrechen.

Über die weiteren bedeutsamen oder schwerwiegenden Nebenwirkungen bei methylphenidathaltigen Arzneimittel sollten Sie von Ihrem Arzt/Ihrer Ärztin informiert worden sein. Es sind:

  • Suizid, Suizidversuch, Suizidgedanken, stereotype (krankhaft häufig wiederholte) Verhaltensweisen, Berührungshalluzinationen,
  • Bewegungsstörungen, Tics oder Verschlechterung bestehender Tics (Tourette-Syndrom), Nervenschädigungen, Migräne, bestimmte Muskel- und Nervenausfälle, Zustände, die mit hohem Fieber und Muskelsteife einhergehen und bis zur Bewusstlosigkeit führen können («malignes neuroleptisches Syndrom»). Wenn dies geschieht, wenden Sie sich sofort an einen Arzt bzw. eine Ärztin oder ein Krankenhaus.
  • verschwommenes Sehen,
  • plötzlicher Herztod, Probleme mit den Blutgefässen des Gehirns (Schlaganfall, Entzündung oder Verschluss der Blutgefässe im Gehirn), Störungen der Blutgefässe oder der Durchblutung von Extremitäten mit Taubheitsgefühl der Finger oder Zehen oder kalten Händen oder Füssen,
  • Leberfunktionsstörungen,
  • spezielle Entzündungen bzw. Veränderungen der Haut (Exfoliative Dermatitis, Steven-Johnson-Syndrom), allergische Hautauschläge,
  • Blut im Urin,
  • Brustdrüsenschwellungen beim Mann.

Folgende Nebenwirkungen können bei der Einnahme von Medikinet MR auftreten:

Sehr häufig treten Nervosität und Schlaflosigkeit auf.

Eine Abnahme des Appetites ist häufig, in der Regel jedoch vorübergehend.

Bei Langzeitanwendung bei Kindern kann Medikinet MR zu mässig reduzierter Gewichtszunahme und geringfügiger Wachstumshemmung führen. Ein direkter Zusammenhang mit der Therapie konnte jedoch nicht bestätigt werden.

Häufig treten Bauchschmerzen, Übelkeit oder Erbrechen auf. Durch gleichzeitige Nahrungsaufnahme können diese jedoch gemildert werden.

Zudem treten häufig Hautausschlag, Juckreiz, Nesselfieber, Fieber, Gelenkschmerzen, Haarausfall, Mundtrockenheit, Schwindel, Kopfschmerzen, Schläfrigkeit, Bewegungsstörungen, Herzklopfen, unregelmässiger Herzschlag und Veränderungen des Blutdrucks auf.

Konsultieren Sie Ihren Arzt oder Ihre Ärztin, wenn diese Probleme länger andauern oder sich verschlimmern.

Selten: Akkommodationsstörungen (Schwierigkeiten für die Umstellung von Fern- auf Nahsicht) und verschwommenes Sehen.

Sehr selten: Hyperaktivität, depressive Verstimmung, Entzündungen oder Verschluss von Hirngefässen, Muskelkrämpfe, Verminderung der Blutplättchen (Thrombozytopenie), der weissen Blutkörperchen (Leukopenie), der roten Blutkörperchen (Anämie) und Leberfunktionsstörungen.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Medikinet MR soll in der Originalpackung und nicht über 25 °C aufbewahrt werden.

Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.

Arzneimittel sollen für Kinder unerreichbar aufbewahrt werden.

Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.

+-Was ist in Medikinet MR Kapseln enthalten?

1 Kapsel enthält 10 mg, 20 mg, 30 mg oder 40 mg Methylphenidat-hydrochlorid als Wirkstoff. Medikinet MR 10 und 20 mg Kapseln enthalten die Farbstoffe Erythrosin (E127), Patentblau V (E 131) und Indigotin (E 132). Medikinet MR 30 und 40 mg Kapseln enthalten die Farbstoffe Erythrosin (E 127) und Indigotin (E 132). In allen Dosisstärken sind das Konservierungsmittel Sorbinsäure (E 200) und weitere Hilfsstoffe enthalten.
