Metformin-Mepha ist ein oral einzunehmendes Mittel gegen Zuckerkrankheit (Diabetes mellitus), das den Wirkstoff Metformin (ein Biguanid) enthält. Es senkt den Zuckergehalt des Blutes ohne Gefahr einer Unterzuckerung (Hypoglykämie), indem es die Bildung von Glucose in der Leber hemmt, die Aufnahme von Glucose in die Muskelzellen und den Glucoseverbrauch verbessert sowie die Glucoseaufnahme im Darm verzögert. Metformin-Mepha trägt ausserdem dazu bei, den Gehalt des Blutes an Cholesterin und Triglyceriden (Blutfetten) zu senken.
Metformin-Mepha wird zur Behandlung des nicht-insulinabhängigen Diabetes (Typ2-Diabetes) verwendet, insbesondere bei übergewichtigen Patienten, bei denen der erhöhte Blutzucker durch Diät und körperliche Aktivität allein nicht kontrolliert werden kann. Metformin-Mepha kann alleine oder in Kombination mit anderen oralen blutzuckersenkenden Arzneimitteln oder mit Insulin angewendet werden. Metformin-Mepha kann beim insulinabhängigen Diabetes (Typ1-Diabetes) als Ergänzung zur Insulintherapie verwendet werden, wenn eine Behandlung mit Insulin nicht zu einer ausreichenden Blutzuckerkontrolle führt.
Metformin-Mepha wird nur auf Verschreibung des Arztes oder der Ärztin und unter dessen bzw. deren sorgfältiger Aufsicht angewendet.
Auch wenn Sie Metformin-Mepha einnehmen, ist es unerlässlich, dass Sie die Diätvorschriften befolgen und die vom Arzt oder der Ärztin verordneten Kontrolltests konsequent durchführen. Diät ist die wichtigste Massnahme gegen Zuckerkrankheit. Wenn Sie übergewichtig sind, sollten Sie Ihre Kalorienzufuhr vermindern und Ihr Körpergewicht reduzieren. Es wäre ein Irrtum zu glauben, dass Sie die Diät weniger streng befolgen müssten, weil Sie ein blutzuckersenkendes Mittel einnehmen.
Metformin-Mepha darf nicht eingenommen werden bei:
Informieren Sie Ihren Arzt oder Ihre Ärztin über die Einnahme dieses Arzneimittels, wenn Sie sich im Spital einer Untersuchung, einer Operation unter Narkose oder Rückenmarksanästhesie, oder der Behandlung einer zusätzlichen Erkrankung (z.B. Herzkrankheit, Infektionskrankheit) unterziehen müssen.
In sehr seltenen Fällen kann es während der Behandlung zur sogenannten Laktatazidose, einer schweren Stoffwechselentgleisung, kommen, die tödlich ausgehen kann, wenn sie nicht unverzüglich behandelt wird. Beim Auftreten von Symptomen wie Erbrechen, Durchfällen, Bauchschmerzen mit Muskelkrämpfen, allgemeinem Unwohlsein mit ausgeprägtem Schwächezustand oder starker Beschleunigung der Atmung sollten Sie sofort Ihren Arzt oder Ihre Ärztin konsultieren und das Arzneimittel bis zum Arzttermin absetzen.
Bisher ist eine Laktatazidose bei Patienten aufgetreten, die eine eingeschränkte Nierenfunktion hatten. Weitere Risikofaktoren für das Auftreten von Laktatazidose sind: komplizierte oder schlecht eingestellte Zuckerkrankheit, längeres Fasten, Alkoholismus, eingeschränkte Leberfunktion und Sauerstoffmangel im Gewebe (z.B. bei Störungen der Blutzirkulation, Herzinfarkt oder anderen Herz-Kreislauf-Erkrankungen).
Gewisse Krankheiten oder die Einnahme von Arzneimitteln können den Diabetes mehr oder weniger schwer aus dem Gleichgewicht bringen, so dass die vorübergehende Anwendung von Insulin notwendig sein kann. Informieren Sie Ihren Arzt oder Ihre Ärztin über jede Begleitbehandlung sowie jede Infektionskrankheit wie z.B. Grippe, Infektionen der Lunge und Harnwege.
Besondere Vorsicht ist in Situationen geboten, in denen eine Einschränkung der Nierenfunktion auftreten kann, z.B. zu Beginn einer Behandlung gegen Bluthochdruck, einer Behandlung mit wasserausschwemmenden Mitteln (Diuretika) oder mit gewissen Schmerz- und Rheumamitteln, z.B. nicht-steroidalen Entzündungshemmern.
Ihr Arzt bzw. Ihre Ärztin wird in regelmässigen Abständen mittels Stoffwechselkontrollen die Nierenfunktion, den Blutzucker und andere Blutwerte überprüfen. Die Nierenfunktion muss ausreichend sein, weil die Ausscheidung des Wirkstoffes von Metformin-Mepha zum grössten Teil über die Nieren erfolgt.
Zahlreiche andere Arzneimittel sowie Alkohol können bei gleichzeitiger Einnahme mit Metformin-Mepha die blutzuckersenkende Wirkung von Metformin-Mepha verstärken oder abschwächen bzw. unerwünschte Wirkungen auslösen oder verstärken.
Zu diesen Arzneimitteln gehören gewisse Antibiotika (z.B. Chinolone, Isoniazid, Trimethoprim, Vancomycin), Schmerz- und Rheumamittel (Morphin, Kortikosteroide), gewisse Substanzen mit Hormonwirkung (z.B. hormonale Empfängnisverhütungsmittel (z.B. Pille), Arzneimittel gegen Wechseljahrbeschwerden, Schilddrüsenhormone, Tetracosactid), wasserausschwemmende Mittel (Diuretika, z.B. Furosemid, Amilorid, Triamteren), Arzneimittel zur Behandlung von Herz-Kreislauf-Erkrankungen (z.B. Betablocker, ACE-Hemmer, Calciumkanalblocker wie Nifedipin, Digoxin, Chinidin, Clonidin, Reserpin), Arzneimittel zur Regulierung der Blutfettwerte (z.B. Nicotinsäure), Mittel gegen psychische Erkrankungen und Depressionen (z.B. Chlorpromazin, MAO-Hemmer), Arzneimittel gegen Epilepsie (Phenytoin), Asthmamittel (z.B. Salbutamol, Terbutalin), und Mittel gegen übermässige Säureproduktion im Magen (z.B. Cimetidin, Ranitidin).
Falls Sie eines oder mehrere solcher Arzneimittel einnehmen müssen, wird Ihr Arzt bzw. Ihre Ärztin möglicherweise die Dosis von Metformin-Mepha anpassen. Auf jeden Fall muss der Blutzucker häufig kontrolliert werden.
Vermeiden Sie den Konsum alkoholischer Getränke; Alkohol kann die blutzuckersenkende Wirkung von Metformin-Mepha verstärken, übermässiger Alkoholkonsum kann die Gefahr einer Laktatazidose erhöhen.
Dies sind zum Beispiel gewisse wasserausschwemmende Arzneimittel (Diuretika), iodhaltige Kontrastmittel (vgl. Wann darf Metformin-Mepha nicht eingenommen werden?) sowie Alkohol.
Dies sind zum Beispiel gewisse wasserausschwemmende Arzneimittel (Diuretika wie Furosemid) und Arzneimittel zur Behandlung von Blutgerinnungsstörungen (z.B. Phenprocoumon).
Gewisse blutdrucksenkende Arzneimittel (z.B. Betablocker, Clonidin, Reserpin) können die Wahrnehmung der Warnzeichen einer Unterzuckerung beeinträchtigen. Metformin-Mepha allein führt nicht zur Unterzuckerung; wenn Sie das Arzneimittel jedoch in Kombination mit Insulin oder anderen blutzuckersenkenden Mitteln (z.B. Sulfonylharnstoffen) nehmen, sollten Sie beachten, dass die Wirkung dieser anderen Präparate verstärkt werden kann.
Metformin-Mepha alleine führt nicht zu einer Unterzuckerung und hat deshalb keinen Effekt auf die Fähigkeit, ein Fahrzeug zu lenken oder Maschinen zu bedienen. Bei einer Kombination mit anderen blutzuckersenkenden Arzneimitteln ist jedoch wegen des Risikos einer Unterzuckerung Vorsicht geboten.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen.
Metformin-Mepha soll während einer Schwangerschaft grundsätzlich nicht eingenommen werden. Während der Schwangerschaft wird zur Behandlung des Diabetes Insulin verordnet. Falls Sie während der Behandlung mit diesem Arzneimittel schwanger werden, ist auf Insulin umzustellen. Wenden Sie sich an Ihren Arzt oder an Ihre Ärztin, wenn Sie schwanger sind oder schwanger werden möchten; er bzw. sie wird Sie zur weiteren Behandlung des Diabetes beraten.
Weil nicht bekannt ist, ob Metformin-Mepha in die Muttermilch übergeht, soll das Arzneimittel auch während der Stillzeit nicht eingenommen werden.
Ihr Arzt bzw. Ihre Ärztin wird die Dosierung von Metformin-Mepha angepasst an den Schweregrad Ihrer Zuckerkrankheit und die Ergebnisse der Stoffwechseluntersuchungen festlegen. Üblicherweise wird die Behandlung mit einer niedrigen Dosis begonnen und die Dosis schrittweise erhöht. Die Tagesdosis Metformin-Mepha sollte auf 2?3 Einzeldosen aufgeteilt und während oder nach den Mahlzeiten eingenommen werden.
Die Lactab sind mit etwas Wasser einzunehmen und sollen nicht zerkaut werden. Metformin-Mepha 1000 Lactab können halbiert werden, was die Einnahme unter Umständen erleichtert.
In der Regel wird mit einer Dosis von 500?1000 mg pro Tag begonnen. Danach kann Ihr Arzt bzw. Ihre Ärztin die Dosis alle 5?7 Tage erhöhen, bis Ihr Blutzuckerspiegel gut eingestellt ist. Die übliche Dosis beträgt 1,5?2,0 g pro Tag, die maximale Tagesdosis ist 3 g.
Ihr Arzt bzw. Ihre Ärztin wird festlegen, welche Stärke und wie viel Teildosen von Metformin-Mepha Sie pro Tag einnehmen sollen. Bei einem Wechsel von einem anderen oralen blutzuckersenkenden Mittel zu Metformin-Mepha wird Ihr Arzt bzw. Ihre Ärztin in der Regel anordnen, das bisherige Arzneimittel abzusetzen und ohne Unterbruch mit Metformin-Mepha zu beginnen.
Bei der Kombination von Metformin-Mepha mit anderen oralen Antidiabetika wird Ihr Arzt bzw. Ihre Ärztin die Dosierung von Metformin-Mepha in Abhängigkeit vom Schweregrad Ihrer Zuckerkrankheit und angepasst an die Ergebnisse der Blutzuckeruntersuchungen festlegen. Daher können keine generellen Dosierungsempfehlungen gegeben werden.
Bei einer Kombination mit Insulin wird Ihr Arzt bzw. Ihre Ärztin die übliche (Anfangs)dosis von Metformin-Mepha verordnen und die Insulindosis anhand der Blutzuckerwerte festlegen.
Bei älteren Patienten muss die Dosis von Metformin-Mepha vorsichtig anhand der Nierenfunktion festgelegt werden.
Die Anwendung von Metformin-Mepha wurde nur an Kindern ab 10 Jahren geprüft. Die Dosis wird vom Arzt bzw. der Ärztin individuell festgelegt. Die maximale Tagesdosis beträgt 2 g. Das Arzneimittel sollte bei Kindern unter 10 Jahren nicht angewendet werden.
Wenn vom Arzt bzw. von der Ärztin nicht anders verordnet, muss dieses Arzneimittel täglich ohne Unterbruch eingenommen werden.
Wenn Sie vergessen haben, eine oder mehrere Lactab einzunehmen, dürfen Sie diesen Fehler nicht durch Einnahme einer erhöhten Anzahl Lactab ausgleichen. Nehmen Sie die nächste Dosis zur gewohnten Zeit ein.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme oder Anwendung von Metformin-Mepha auftreten:
Häufig (1?10%) bis sehr häufig (>10%): Magen-Darm-Störungen wie z.B. Erbrechen, Übelkeit, Durchfall, Bauchschmerzen, Appetitmangel. In den meisten Fällen treten diese Erscheinungen zu Beginn auf und verschwinden dann von alleine, ohne dass die Behandlung abgebrochen werden muss. Um solche unerwünschten Wirkungen auf ein Minimum zu beschränken, wird empfohlen, Metformin-Mepha in mehreren Teildosen während den Mahlzeiten einzunehmen. Gehen diese Nebenwirkungen nicht zurück, sollten Sie Ihren Arzt bzw. Ihre Ärztin benachrichtigen. Er bzw. sie wird darüber entscheiden, ob und wie die Behandlung fortgesetzt werden soll.
Häufig (1?10%): Veränderungen des Geschmacksinns (Metallgeschmack).
Sehr selten (unter 0,01%): Überempfindlichkeitsreaktionen der Haut (z.B. Hautrötungen, Juckreiz, Nesselfieber), Laktatazidose. Das Risiko einer Laktatazidose soll durch strikte Beachtung der Vorsichtsmassnahmen reduziert werden (vgl. «Wann ist bei der Einnahme von Metformin-Mepha Vorsicht geboten?»).
Falls Sie über längere Zeit mit Metformin-Mepha behandelt werden, wird Ihr Arzt bzw. Ihre Ärztin regelmässig Ihr Blut untersuchen, da es sehr selten zu einer Verminderung von Vitamin B12 kommen kann.
Einzelfälle: veränderte Leberfunktion, Leberentzündung, Abnahme der weissen Blutkörperchen und der Blutplättchen.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Die Lactab sollen bei Raumtemperatur (15?25 °C) gelagert werden.
Das Arzneimittel muss ausser Reichweite von Kindern aufbewahrt und darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
