+-Was ist Micardis Amlo und wann wird es angewendet?

Micardis Amlo enthält zwei Wirkstoffe: Telmisartan und Amlodipin. Beide Substanzen helfen, einen hohen Blutdruck zu kontrollieren:

  • Telmisartan gehört zur Wirkstoffgruppe der sogenannten Angiotensin-II-Rezeptor-Antagonisten. Angiotensin II ist eine körpereigene Substanz, die eine Verengung der Blutgefässe und infolgedessen einen Blutdruckanstieg hervorruft. Telmisartan wirkt, indem es die Effekte von Angiotensin II blockiert.
  • Amlodipin gehört zur Wirkstoffgruppe der sogenannten Calciumkanalblocker. Amlodipin stoppt den Einstrom von Calcium in die Wand der Blutgefässe. Dadurch wird verhindert, dass sich die Blutgefässe verengen.

Micardis Amlo wird in folgenden Fällen zur Behandlung von Bluthochdruck verwendet:

  • Bei Patienten, deren Blutdruck mit Amlodipin oder Telmisartan nicht ausreichend kontrolliert werden kann.
  • Als Erstbehandlung bei Patienten mit mässig bis stark erhöhtem Blutdruck.
  • Bei Patienten, die bereits Telmisartan und Amlodipin in separaten Tabletten erhalten und zur Vereinfachung stattdessen die gleichen Wirkstoffdosen in nur einer Tablette einnehmen möchten.

Ein unbehandelter hoher Blutdruck kann zur Schädigung von Blutgefässen in verschiedenen Organen führen. Dies kann Sie dem Risiko aussetzen, schwerwiegenden Ereignissen wie Herzinfarkt, Herz- oder Nierenschwäche, Schlaganfall oder Erblindung zu erleiden. Die Reduzierung Ihres Blutdruckes auf einen normalen Wert vermindert dieses Risiko.

Micardis Amlo darf nur auf Verschreibung des Arztes bzw. der Ärztin angewendet werden.

+-Wann darf Micardis Amlo nicht eingenommen werden?

Micardis Amlo darf nicht eingenommen werden,

  • wenn Sie überempfindlich (allergisch) gegen Telmisartan oder Amlodipin oder einen der sonstigen Bestandteile von Micardis Amlo Tabletten sind,
  • wenn Sie allergisch gegen andere Arzneimittel vom Dihydropyridin-Typ (eine Untergruppe der Calciumkanalblocker) sind,
  • wenn Sie schwanger sind ? siehe den Abschnitt «Darf Micardis Amlo während einer Schwangerschaft oder in der Stillzeit eingenommen werden?»),
  • wenn Sie ein Kind stillen,
  • wenn Sie an einer schweren Lebererkrankung oder einer Gallenwegsobstruktion (Störung des Abflusses von Galle aus Leber und Gallenblase) leiden,
  • wenn die Förderleistung Ihres Herzens aufgrund einer schweren Herzerkrankung vermindert ist.
  • Micardis Amlo darf nicht gleichzeitig mit Aliskiren angewendet werden, wenn Sie an Diabetes mellitus leiden oder eine eingeschränkte Nierenfunktion haben.

Wenn einer der oben genannten Punkte auf Sie zutrifft, informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, bevor Sie Micardis Amlo einnehmen.

+-Wann ist bei der Einnahme von Micardis Amlo Vorsicht geboten?

Bitte informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie an einem/einer der folgenden Umstände oder Erkrankungen leiden oder in der Vergangenheit gelitten haben:

  • Nierenerkrankung oder Nierentransplantation;
  • Nierenarterienstenose (Verengung der zu einer oder beiden Nieren führenden Blutgefässe);
  • Lebererkrankung;
  • Herzbeschwerden;
  • erhöhte Aldosteronspiegel (Ansammlung von Wasser und Salz im Körper, einhergehend mit einem Ungleichgewicht von verschiedenen Mineralstoffen im Blut);
  • niedriger Blutdruck (Hypotonie), der bei Dehydrierung (übermässiger Verlust von Körperwasser), Salzmangel infolge einer Diuretikabehandlung («Entwässerungstabletten»), salzarmer Kost, Durchfall oder Erbrechen auftreten kann;
  • erhöhte Kaliumspiegel im Blut;
  • Diabetes mellitus (Zuckerkrankheit).

Im Fall einer Operation oder Narkose sollten Sie Ihrem Arzt oder Ihrer Ärztin mitteilen, dass Sie Micardis Amlo einnehmen.

Die Anwendung von Micardis Amlo bei Kindern und Jugendlichen unter 18 Jahren wird nicht empfohlen.

Einnahme mit anderen Arzneimitteln

Bitte informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie andere Arzneimittel einnehmen/anwenden bzw. vor kurzem eingenommen/angewendet haben, auch wenn es sich um nicht verschreibungspflichtige Arzneimittel handelt. Ihr Arzt oder Ihre Ärztin muss unter Umständen die Dosierung dieser anderen Arzneimittel anpassen oder sonstige Vorsichtsmassnahmen treffen. In einigen Fällen kann es erforderlich sein, die Einnahme eines Arzneimittels abzubrechen. Dies gilt insbesondere für die nachstehend aufgeführten Arzneimittel, wenn diese gleichzeitig mit Micardis Amlo eingenommen werden:

  • Lithiumhaltige Arzneimittel zur Behandlung bestimmter Arten von Depression.
  • Arzneimittel, die den Kaliumspiegel im Blut erhöhen können, wie kaliumhaltige Salzersatzmittel, kaliumsparende Diuretika (bestimmte «Entwässerungstabletten»), ACE-Hemmer, Angiotensin-II-Rezeptor-Antagonisten, NSAR (nichtsteroidale Antirheumatika, z.B. Aspirin oder Ibuprofen), Heparin, Immunsuppressiva (z.B. Cyclosporin oder Tacrolimus) und das Antibiotikum Trimethoprim.
  • Krampfhemmende Wirkstoffe (z.B. Carbamazepin, Phenobarbital, Phenytoin, Fosphenytoin, Primidon).
  • Rifampicin, Johanniskraut.
  • Arzneimittel zur Behandlung von HIV/AIDS (z.B. Ritonavir) oder zur Behandlung von Pilzinfektionen (z.B. Ketoconazol).
  • Arzneimittel gegen erhöhte Blutcholesterinwerte (z.B. Simvastatin).
  • Grapefruitsaft enthält eine oder mehrere Komponenten, die den Stoffwechsel einiger Arzneimittel einschliesslich Amlodipin herabsetzen. Wenn Sie Micardis Amlo einnehmen, sollten Sie grössere Mengen Grapefruitsaft (mehr als 1 Liter täglich) vermeiden; bei normalen Mengen (1 Glas zu 250 ml täglich) ist es unwahrscheinlich, dass daraus Probleme entstehen können.

Wie bei anderen blutdrucksenkenden Arzneimitteln kann die Wirkung von Micardis Amlo abgeschwächt sein, wenn Sie NSAR (nichtsteroidale Antirheumatika, z.B. Aspirin oder Ibuprofen) oder Kortikosteroide einnehmen.

Micardis Amlo kann die blutdrucksenkende Wirkung anderer Arzneimittel zur Behandlung von hohem Blutdruck oder von Arzneimitteln mit blutdrucksenkendem Potential (z.B. Sildenafil, Antidepressiva, Barbiturate, Betäubungsmittel, Nitroglycerin oder andere Nitrate) oder von Alkohol erhöhen.

Vorsicht ist geboten, wenn Sie mit einem ACE-Hemmer oder Aliskiren (Arzneimittel gegen Bluthochdruck) behandelt werden.

Verkehrstüchtigkeit und Bedienen von Maschinen

Aufgrund möglicher Nebenwirkungen kann dieses Arzneimittel die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit Maschinen zu bedienen beeinträchtigen.

Sorbitol

Micardis Amlo enthält Sorbitol.

Wenn Sie an einer Unverträglichkeit gegenüber bestimmten Zuckern leiden, halten Sie vor der Einnahme von Micardis Amlo Rücksprache mit Ihrem Arzt oder Ihrer Ärztin.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden (Externa)!

+-Darf Micardis Amlo während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Sie müssen Ihrem Arzt oder Ihrer Ärztin mitteilen, wenn Sie vermuten schwanger zu sein (oder schwanger werden könnten). Ihr Arzt oder Ihre Ärztin wird Ihnen normalerweise raten, Micardis Amlo abzusetzen, bevor Sie schwanger werden oder sobald Sie wissen, dass Sie schwanger sind, und Ihnen statt Micardis Amlo ein anderes Arzneimittel verordnen.

Micardis Amlo darf weder in der Schwangerschaft noch in der Stillzeit eingenommen werden. Wenn Sie während der Einnahme von Micardis Amlo schwanger werden, informieren Sie sofort Ihren Arzt bzw. Ihre Ärztin, der/die über eine Absetzung des Präparates entscheiden wird (siehe auch «Wann darf Micardis Amlo nicht angewendet werden?»).

+-Wie verwenden Sie Micardis Amlo?

Nehmen Sie Micardis Amlo immer genau nach Anweisung des Arztes oder der Ärztin ein. Bitte fragen Sie bei Ihrem Arzt oder Apotheker bzw. bei Ihrer Ärztin oder Apothekerin nach, wenn Sie sich nicht ganz sicher sind.

Die übliche Dosis von Micardis Amlo ist eine Tablette pro Tag. Nehmen Sie die Tablette nach Möglichkeit jeden Tag zur gleichen Zeit ein. Entnehmen Sie die Tablette erst unmittelbar vor der Einnahme aus dem Blister.

Sie können Micardis Amlo zu den Mahlzeiten oder unabhängig davon einnehmen. Die Tabletten sollten mit etwas Wasser oder einem anderen alkoholfreien Getränk geschluckt werden. Es ist wichtig, dass Sie Micardis Amlo jeden Tag einnehmen, solange Ihr Arzt bzw. Ihre Ärztin es nicht anders verordnet.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

  • Wenn Ihre Leberfunktion eingeschränkt ist, sollte die übliche Dosis 40/5 mg einmal täglich nicht überschreiten.
  • Wenn Sie versehentlich zu viele Tabletten eingenommen haben, wenden Sie sich unverzüglich an Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin oder die Notaufnahme des nächstgelegenen Krankenhauses.
  • Wenn Sie vergessen haben Ihr Arzneimittel einzunehmen, sollten Sie, sobald Sie dies bemerken, die Dosis am gleichen Tag zu sich nehmen. Ist dies nicht mehr möglich, nehmen Sie Ihre verschriebene Dosis von Micardis Amlo am nächsten Tag ein; nehmen Sie keine doppelte Dosis.

Micardis Amlo wird nicht empfohlen für die Anwendung bei Kindern und Jugendlichen unter 18 Jahren aufgrund des Fehlens von Daten zur Unbedenklichkeit und Wirksamkeit.

+-Welche Nebenwirkungen kann Micardis Amlo haben?

Folgende Nebenwirkungen können bei der Einnahme von Micardis Amlo auftreten:

Häufig: Schwindel, Schwellung der Knöchel (Ödem).

Gelegentlich: Somnolenz, Migräne, Kopfschmerzen, Kribbeln oder Taubheit in Händen oder Füssen, Drehschwindel (Vertigo), langsamer Herzschlag, Palpitationen (wahrnehmbares Herzklopfen), niedriger Blutdruck (Hypotonie), Schwindel beim Aufstehen (orthostatische Hypotonie), einschiessende Rötung im Gesicht (Flush), Husten, Bauchschmerzen, Durchfall, Übelkeit, Juckreiz, Gelenkschmerzen, Muskelkrämpfe, Muskelschmerzen, Impotenz, Schwäche, Schmerzen im Brustkorb, Müdigkeit, erhöhte Leberenzymwerte.

Selten: Blasenentzündung, niedergedrückte Stimmung (Depression), Angst, Schlaflosigkeit, Ohnmacht, Geschmacksstörungen, Zittern, Erbrechen, Verdickung des Zahnfleisches, Bauchbeschwerden, Mundtrockenheit, Ekzem (Hautveränderung), Hautrötung, Ausschlag, Rückenschmerzen, Gliederschmerzen, nächtlicher Harndrang, Unwohlsein, erhöhte Harnsäurewerte, Sensibilitätsstörungen.

Bei Patienten, die mit Telmisartan allein behandelt wurden, sind zusätzlich die folgenden Nebenwirkungen berichtet worden:

Sepsis (oft «Blutvergiftung» genannt, eine schwere Infektion mit entzündlicher Reaktion des ganzen Körpers, die zum Tode führen kann), Harnwegsinfektionen, Infektionen der oberen Atemwege (z.B. Halsschmerzen, Nebenhöhlenentzündung, gewöhnliche Erkältung), Mangel an roten Blutkörperchen (Blutarmut), erhöhte Anzahl bestimmter weisser Blutkörperchen (Eosinophilie), niedrige Anzahl von Blutplättchen (Thrombozytopenie), allergische Reaktion (z.B. Ausschlag, Juckreiz, Schwierigkeiten beim Atmen, pfeifende Atemgeräusche, Anschwellen des Gesichts oder Blutdruckabfall), hohe Kaliumspiegel, Senkung der Blutzuckerspiegel (Hypoglykämie) (bei Diabetikern), Sehstörungen, beschleunigter Herzschlag, Kurzatmigkeit, Blähungen, Magenbeschwerden, Leberfunktionsstörung, rasches Anschwellen der Haut und Schleimhäute (Angioödem), vermehrtes Schwitzen, Nesselsucht (Urtikaria), Arzneimittelexanthem, Sehnenentzündung, Nierenfunktionsstörungen einschliesslich akutes Nierenversagen, grippeähnliche Erkrankung, vermindertes Hämoglobin (ein Bluteiweiss), erhöhte Werte von Kreatinin oder Kreatininphosphokinase im Blut.

Bei Patienten, die mit Amlodipin allein behandelt wurden, sind zusätzlich die folgenden Nebenwirkungen berichtet worden:

Mangel an weissen Blutkörperchen (Leukopenie), erhöhter Blutzucker, Stimmungsschwankungen, Verwirrtheitszustände, Ohrgeräusche, Herzinfarkt, Entzündung der Blutgefässe, Niesen/Nasenlaufen, Veränderung der Stuhlgewohnheiten, Bauchspeicheldrüsenentzündung, Leberentzündung, Gelbsucht, Haarausfall, ungewöhnliche Blutergussbildung und Blutung (Schädigung der roten Blutkörperchen), Hautverfärbung, schwere Hautreaktionen, Lichtempfindlichkeit, Schwierigkeiten beim Wasserlassen, verstärkter Harndrang insbesondere nachts, Brustvergrösserung bei Männern, Schmerzen, Gewichtszunahme, Gewichtsabnahme.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.

Vor Feuchtigkeit und Licht geschützt in der Originalverpackung, bei Raumtemperatur (15-25 °C) und ausser Reichweite von Kindern aufbewahren.
