+-Was ist Perindopril Actavis und wann wird es angewendet?

Perindopril Actavis ist ein Arzneimittel zur Behandlung von:

  • Arterieller Hypertonie,
  • Herzinsuffizienz (ein Zustand, bei dem das Herz nicht in der Lage ist, eine für den Körper ausreichende Menge Blut zu pumpen),
  • Vorbeugung gegen Schlaganfall-Rezidive (Gehirnschäden aufgrund einer Hirnblutung oder infolge einer plötzlichen, arteriellen Obstruktion), in Verbindung mit Indapamid, bei Patienten, die in den letzten 5 Jahren einen Hirnschlag erlitten haben,
  • sowie zur Reduzierung des Risikos kardiovaskulärer Komplikationen bei Patienten mit stabiler koronarer Herzerkrankung.

Der in Perindopril Actavis enthaltene Wirkstoff Perindopril, oder genauer gesagt dessen aktiver Metabolit Perindoprilat, verringert drastisch die Bildung einer körpereigenen Substanz, welche für die Entstehung des erhöhten Blutdrucks verantwortlich ist. Der arterielle Blutdruck wird somit reduziert und die Leistungsfähigkeit des Herzens wird verbessert.

Perindopril Actavis gehört zur therapeutischen Kategorie der ACE (Angiotensin-Converting-Enzym)-Hemmer.

+-Was sollte dazu beachtet werden?

Es sind keine besonderen Massnahmen erforderlich.

+-Wann darf Perindopril Actavis nicht eingenommen werden?

Perindopril Actavis darf nicht angewendet werden:

  • bei Kindern und Jugendlichen,
  • während der Schwangerschaft und der Stillzeit,
  • bei bestehender Allergie gegen Perindopril oder einen der Inhaltsstoffe (siehe Liste in «Was ist in Perindopril Actavis enthalten?») oder gegen andere ACE-Hemmer (falls es zum Beispiel während einer vorangegangenen medikamentösen Bluthochdruckbehandlung zu einer Schwellung des Gesichts gekommen ist),
  • bei Patienten mit schwerer Niereninsuffizienz,
  • wenn Sie bereits früher Symptome wie Keuchen, Anschwellen von Gesicht, Zunge oder Hals, intensives Jucken oder schwerwiegende Hautausschläge in Verbindung mit einer früheren ACE-Hemmer-Behandlung hatten, oder wenn diese Symptome bei Ihnen oder einem Familienmitglied unter irgendwelchen anderen Umständen aufgetreten sind (ein Zustand, der als Angioödem bezeichnet wird),
  • wenn Sie Diabetes (Typ 1 oder 2) haben oder Ihre Niereninsuffizienz beeinträchtigt ist und Sie Aliskiren (z.B. Rasilez) oder ein Aliskiren enthaltendes Arzneimittel einnehmen.
+-Wann ist bei der Einnahme von Perindopril Actavis Vorsicht geboten?

Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen!

Die Behandlung erfordert eine regelmässige Überwachung durch den Arzt bzw. die Ärztin. Vor Therapiebeginn muss der Arzt bzw. die Ärztin dafür Sorge tragen, dass jede eventuelle Verringerung des Blutvolumens korrigiert wurde. Eine solche Verringerung kann beispielsweise durch eine Behandlung mit Diuretika, salzarme Ernährung, eine Dialysebehandlung, Durchfall oder Erbrechen verursacht werden.

Wenn Sie sich in einer der folgenden Situationen befinden, informieren Sie Ihren Arzt oder Ihren Apotheker bzw. Ihre Ärztin oder Ihre Apothekerin, bevor Sie Perindopril Actavis einnehmen:

  • Wenn Sie eine Aortenstenose (Verengung des Hauptblutgefässes, das vom Herzen wegführt oder eine hypertrophische Kardiomyopathie (Herzmuskelerkrankung) oder eine renale Arterienstenose (Verengung der Arterie, welche die Nieren mit Blut versorgt) haben,
  • wenn Sie unter anderen Herzerkrankungen leiden,
  • wenn Sie Leberprobleme haben,
  • wenn Sie Probleme mit Ihren Nieren haben oder ein Dialyse erhalten. Falls Sie Dialysepatient/in sind, informieren Sie Ihren Arzt bzw. Ihre Ärztin und seine/ihre Mitarbeiter über Ihre Behandlung mit einem ACE-Hemmer, da manche Membranfilter in diesem Fall nicht verwendet werden dürfen,
  • wenn Sie Anzeichen einer Infektion haben (z.B. Halsschmerzen, Fieber),
  • wenn Sie unter einer Kollagen-Krankheit (Erkrankung des Bindegewebes) wie systemischem Lupus erythematodes oder Sklerodermie leiden,
  • wenn Sie Arzneimittel zur Behandlung von Diabetes einnehmen,
  • wenn Sie eine salzarme Diät einhalten oder Salzersatzstoffe einnehmen, die Kalium enthalten, oder wenn sie andere Arzneimittel, die den Kaliumspiegel im Blut erhöhen einnehmen (siehe «Einnahme von Perindopril Actavis zusammen mit anderen Arzneimitteln»),
  • wenn Sie sich einer LDL-Apherese (maschinelle Entfernung von Cholesterin aus dem Blut) unterziehen,
  • wenn Sie eine Desensibilisierungstherapie erhalten, um die allergischen Reaktionen bei Bienen- und Wespenstichen zu verringern. Ihre Behandlung mit Perindopril Actavis muss vor Beginn der Desensibilisierung umgestellt werden,
  • wenn Sie vor kurzem unter Durchfall oder Erbrechen gelitten haben oder dehydriert sind,
  • wenn Sie eines der folgenden Arzneimittel zur Behandlung von Bluthochdruck einnehmen:
    • einen «Angiotensin-II-Rezeptor-Antagonisten» (ARA-II) (auch bekannt unter dem Namen Sartan ? z. Bsp. Valsartan, Telmisartan, Irbesartan), speziell wenn Sie Nierenprobleme haben zurückzuführen auf Diabetes,
    • Aliskiren.
  • Eventuell erachtet es Ihr Arzt bzw. Ihre Ärztin als nötig, regelmässig Ihre Nierenfunktion, Ihren Blutdruck und Ihre Elektrolytewerte (z. Bsp. Kalium) im Blut zu überwachen. Sehen Sie auch die Informationen im Abschnitt «Wann darf Perindopril Actavis nicht eingenommen werden?»,
  • wenn Sie an einer anderen Krankheit leiden,
  • wenn Sie Allergien haben,
  • wenn Sie schon andere Arzneimittel (auch selbst gekaufte!) einnehmen oder äusserlich anwenden.

Das Angioödem kann häufiger bei Patienten ethnischer Herkunft aus Afrika auftreten.

Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie vermuten, schwanger zu sein (oder schwanger werden könnten). Perindopril Actavis darf nicht eingenommen werden, wenn Sie schwanger sind, da es Ihrem Kind schwerwiegend schaden könnte (siehe «Darf Perindopril Actavis während einer Schwangerschaft oder in der Stillzeit eingenommen werden?»).

Falls Sie gleichzeitig Lithiumpräparate einnehmen, kann der Lithiumblutspiegel während der Behandlung mit Perindopril Actavis ansteigen. Dies kann zu Vergiftungssymptomen führen. Informieren Sie daher Ihren Arzt bzw. Ihre Ärztin über Ihre Therapie.

Falls Sie sich einer Operation unterziehen müssen, denken Sie daran, Ihren Arzt bzw. Ihre Ärztin von Ihrer Therapie mit Perindopril Actavis in Kenntnis zu setzen.

Perindopril Actavis enthält Laktose. Wenn Ihr Arzt bzw. Ihre Ärztin gesagt hat, dass Sie eine Unverträglichkeit gegenüber bestimmten Zuckerarten haben, sprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin, bevor Sie dieses Arzneimittel einnehmen.

Die Verwendung dieses Arzneimittels ist nicht empfohlen, wenn Sie eine Galaktose-Intoleranz, einen Lapp-Laktase-Mangel oder eine Malabsorption von Glukose oder Galaktose haben (seltene Erbkrankheit).

Einnahme von Perindopril Actavis zusammen mit anderen Arzneimitteln:

Die Behandlung mit Perindopril Actavis kann durch andere Arzneimittel beeinflusst werden. Dazu gehören:

  • Andere Arzneimittel zur Behandlung des Bluthochdrucks einschliesslich Aliskiren, Diuretika (Arzneimittel, welche die von den Nieren produzierte Harnmenge erhöhen),
  • kaliumsparende Arzneimittel (Triamteren, Amilorid), Kaliumpräparate oder kaliumhaltige Kochsalzersatzprodukte,
  • kaliumsparende Diuretika zur Behandlung der Herzinsuffizienz: Eplerenon und Spironolacton in einer Dosierung von 12,5 mg bis 50 mg täglich,
  • Lithium zur Behandlung von Manien und Depressionen,
  • nicht steroidale Entzündungshemmer (z.B. Ibuprofen) zur Schmerzlinderung oder hochdosiertes Aspirin,
  • Arzneimittel zur Behandlung von Diabetes (wie Insulin, Gliptine oder Metformin),
  • Baclofen (zur Behandlung von Muskelverspannungen, welche bei Erkrankungen wie Multipler Sklerose vorkommen),
  • Arzneimittel zur Behandlung von psychischen Erkrankungen wie Depression, Angst, Schizophrenie usw. (z.B. trizyklische Antidepressiva, Arzneimittel zur Behandlung von Psychosen),
  • Immunsuppressiva (Arzneimittel, welche die Abwehrreaktionen des Körpers abschwächen) zur Behandlung von Autoimmunerkrankungen oder nach Organtransplantationen (z.B. Ciclosporin, Tacrolismus),
  • Trimethoprim (zur Behandlung von Infektionen),
  • Estramustin (zur Behandlung von Krebserkrankungen),
  • Allopurinol (zur Behandlung der Gicht),
  • Procainamid (zur Behandlung von Herzrhythmusstörungen),
  • Vasodilatatoren einschliesslich Nitrate (Arzneimittel, die eine Erweiterung der Blutgefässe bewirken),
  • Heparin (Arzneimittel zur Blutverdünnung),
  • Arzneimittel zur Behandlung von niedrigem Blutdruck, Schock oder Asthma (z.B. Ephedrin, Noradrenalin oder Adrenalin),
  • Goldsalze, v.a. injizierbares Gold (zur Behandlung von rheumatoider Arthritis).
+-Darf Perindopril Actavis während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Wenn Sie schwanger sind oder stillen, glauben schwanger zu sein oder vorhaben schwanger zu werden, fragen Sie Ihren Arzt bzw. Ihre Ärztin, bevor Sie das Arzneimittel einnehmen.

Schwangerschaft

Perindopril Actavis darf während der Schwangerschaft nicht eingenommen werden.

Wird während der Behandlung eine Schwangerschaft festgestellt, muss Perindopril Actavis abgesetzt und durch ein anderes Arzneimittel ? kein ACE-Hemmer ? ersetzt werden.

Setzen Sie Ihren Arzt bzw. Ihre Ärztin sofort in Kenntnis, wenn Sie schwanger sind oder eine Schwangerschaft planen.

Stillzeit

Da viele Arzneimittel in die Muttermilch übergehen, ist Perindopril Actavis während der Stillzeit kontraindiziert. Falls Sie stillen möchten, informieren Sie Ihren Arzt bzw. Ihre Ärztin über Ihre Behandlung mit einem ACE-Hemmer.

+-Wie verwenden Sie Perindopril Actavis?

Die Perindopril-Actavis-Tabletten sollten in einer Einnahme vor dem Frühstück unzerkaut mit einem Glas Wasser verabreicht werden.

Die empfohlenen Dosen sind wie folgt:

  • Bei arteriellem Bluthochdruck ist die empfohlene Dosierung 1 Tablette Perindopril Actavis 4 mg täglich morgens in einer Einnahme. Nach einmonatiger Behandlung kann die tägliche Dosierung auf 2 Tabletten Perindopril Actavis 4 mg oder 1 Tablette Perindopril Actavis 8 mg morgens in einer Einnahme gesteigert werden.
  • Bei über 70-jährigen Patienten/Patientinnen liegt die empfohlene tägliche Dosis zu Beginn der Behandlung bei 1 Tablette Perindopril Actavis 2 mg morgens. Falls der Arzt bzw. die Ärztin es als erforderlich erachtet, kann die tägliche Dosis auf 1 Tablette Perindopril Actavis 4 mg morgens in einer Einnahme erhöht werden.
  • Bei Patienten mit Herzinsuffizienz liegt die empfohlene tägliche Dosis zu Beginn der Behandlung bei 1 Tablette Perindopril Actavis 2 mg morgens. Falls der Arzt bzw. die Ärztin es als erforderlich erachtet, kann die tägliche Dosis auf 1 Tablette Perindopril Actavis 4 mg morgens in einer Einnahme erhöht werden.
  • Zur Vorbeugung gegen erneuten Schlaganfall beträgt die empfohlene Dosierung 1 Tablette Perindopril Actavis 2 mg täglich während zwei Wochen, danach täglich 1 Tablette Perindopril Actavis 4 mg während weiteren zwei Wochen, bevor Indapamid SR (1,5 mg) hinzugefügt wird.
  • Bei Patienten mit stabiler koronarer Herzerkrankung liegt die empfohlene tägliche Dosis bei 1 Tablette Perindopril Actavis 4 mg morgens während der ersten zwei Behandlungswochen; danach wird die Dosis auf 1 Tablette Perindopril Actavis 8 mg pro Tag gesteigert; die gewohnte Begleittherapie wird beibehalten. Bei älteren Patienten liegt die während der ersten Behandlungswoche empfohlene tägliche Dosis bei 1 Tablette Perindopril Actavis 2 mg; diese kann in der darauf folgenden Woche auf 1 Tablette Perindopril Actavis 4 mg gesteigert werden, bevor die Erhaltungsdosis von täglich 8 mg Perindopril Actavis erreicht wird.
  • Bei niereninsuffizienten Patienten/Patientinnen muss der Arzt bzw. die Ärztin die Dosierung von Perindopril Actavis dem Ausmass der Funktionsstörung anpassen.
  • Bei Patienten, die mit Diuretika behandelt werden, wird der Arzt bzw. die Ärztin abwägen ob es notwendig ist, die diuretische Therapie kurz zu unterbrechen oder ob die Behandlung mit Perindopril Actavis mit einer niedrigeren Dosierung begonnen werden sollte.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

Die Anwendung und die Sicherheit von Perindopril Actavis bei Kindern und Jugendlichen wurden bis zum jetzigen Zeitpunkt nicht untersucht.

Wenn Sie eine grössere Menge von Perindopril Actavis eingenommen haben, als Sie sollten:

Wenn Sie zu viele Tabletten eingenommen haben, wenden Sie sich bitte sofort an die Unfall- und Notaufnahme des nächstgelegenen Krankenhauses oder an Ihren Arzt bzw. Ihre Ärztin. Die wahrscheinlichste Wirkung einer Überdosierung ist ein niedriger Blutdruck, der bei Ihnen Symptome wie Schwindel oder Schwäche hervorrufen kann. In diesem Fall kann es helfen, sich hinzulegen und die Beine hoch zu lagern.

Wenn Sie die Einnahme von Perindopril Actavis vergessen haben:

Es ist wichtig, das Arzneimittel täglich einzunehmen, da eine regelmässige Behandlung effektiver ist. Wenn Sie eine Dosis Perindopril Actavis vergessen haben, dann nehmen Sie die nächste Dosis zur üblichen Zeit ein. Nehmen Sie nicht die doppelte Menge ein, wenn Sie die vorherige Einnahme vergessen haben.

Wenn Sie die Einnahme von Perindopril Actavis abbrechen:

Da die Behandlung mit Perindopril Actavis normalerweise eine Langzeitbehandlung ist, sollten Sie sich an Ihren Arzt bzw. Ihre Ärztin wenden, bevor Sie die Therapie mit dem Arzneimittel abbrechen.

+-Welche Nebenwirkungen kann Perindopril Actavis haben?

Nehmen Sie das Arzneimittel nicht mehr ein und informieren Sie sofort Ihren Arzt bzw. Ihre Ärztin, wenn eine der folgenden Nebenwirkungen bei Ihnen auftritt:

Anschwellen des Gesichts, der Lippen, des Mundes, der Zunge oder des Halses, Schwierigkeiten bei der Atmung,

starker Schwindel oder Schwäche,

ungewöhnlich schnelles oder unregelmässiges Herzklopfen oder Schmerzen in der Brust,

starke Bauchschmerzen.

Geordnet nach abnehmender Häufigkeit, könnten folgende Nebenwirkungen auftreten:

Häufig (kann bis zu 1 von 10 Behandelten betreffen):

Kopfschmerzen, Müdigkeit, Schwindel und Gleichgewichtsstörungen, Ameisenlaufen und Kribbeln, Muskelkrämpfe, Ohrensausen, Benommenheit aufgrund eines niedrigen Blutdrucks, Sehstörungen, Kurzatmigkeit;

niedriger Blutdruck (Blutdruckabfall), Herzklopfen und beschleunigter Puls;

gelegentlich tritt trockener Reizhusten auf;

Verdauungsbeschwerden (Übelkeit, Bauchschmerzen, Erbrechen, Geschmacksstörungen, Reizmagen oder Verdauungsstörungen, Durchfall, Verstopfung);

nicht auszuschliessen sind allergische Reaktionen wie Exantheme (Hautrötungen), Juckreiz, Hautauschläge.

Gelegentlich (kann bis zu 1 von 100 Behandelten betreffen):

Depression, Stimmungsschwankungen, Schlafstörungen, Mundtrockenheit, Schwitzen;

Bronchospasmen (Enge in der Brust, Keuchen und Kurzatmigkeit), Angioödem (Symptome wie Keuchen, Anschwellen von Gesicht, Zunge oder Hals), intensives Jucken oder schwerwiegende Hautauschläge, Blasenbildung auf der Haut,

Urtikaria, Nierenprobleme, Impotenz.

Erhöhte Anzahl eosinophiler Blutzellen (eine Untergruppe von weissen Blutzellen), Schläfrigkeit, Ohnmacht, Herzklopfen, Herzrasen, Vaskulitis (Entzündung der Blutgefässe), Lichtempfindlichkeitsreaktionen (erhöhte Empfindlichkeit der Haut gegenüber Sonnenlicht), Arthralgie (Gelenkschmerzen), Myalgie (Muskelschmerzen), Schmerzen in der Brust, Unwohlsein, peripheres Ödem, Fieber, Stürze, Änderung der Laborwerte: hoher Kaliumwert im Blut (bei Absetzen der Therapie reversibel), niedriger Natriumspiegel im Blut, Hypoglykämie (sehr niedriger Blutzuckerspiegel) bei Diabetiker, hohe Konzentration von Harnsäure und erhöhter Kreatininwert im Blut.

Selten (kann bis zu 1 von 1?000 Behandelten betreffen):

Atemnot, Asthma, Entzündung der Nasennebenhöhlen, Entzündungen der Nasenschleimhäute oder der Zunge, Bronchitis;

Änderung der Laborwerte: erhöhte Leberenzymwerte, erhöhte Bilirubinwerte im Blut.

Sehr selten (kann bis zu 1 von 10?000 Behandelten betreffen):

Verwirrtheit, Herzkreislaufstörungen (unregelmässiger Herzschlag, Schmerzen in der Brust [Angina Pectoris], Myokardinfarkt, transitorische ischämische Attacke, Schlaganfall), akutes Nierenversagen, veränderte Blutwerte wie geringe Anzahl von weissen und roten Blutkörperchen, niedriger Hämoglobinwert, geringe Anzahl von Blutplättchen;

Entzündung der Mundschleimhäute, stoppen des Darmtransits;

Entzündung der Bauchspeicheldrüse (was zu starken Bauch- und Rückenschmerzen führt) oder der Leber (Hepatitis und Leberinsuffizienz können mögliche anhaltende und unerklärte Symptome wie zum Beispiel Übelkeit, Bauchschmerzen, Appetitlosigkeit, ungewohnte Müdigkeit, Erbrechen, gelbliche Färbung der Augen oder der Haut, dunkelfarbigen Urin oder hellen Stuhl auslösen);

Erythema multiforme, Haarausfall;

ein Syndrom aus Fieber, Muskel- und Gliederschmerzen mit Eosinophilie, Anstieg der Blutsenkungsgeschwindigkeit und/oder der antinukleären Antikörper.

Sollten Sie andere Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Um mögliche Wechselwirkungen zu vermeiden, sollten Sie aus eigener Initiative keine anderen Arzneimittel einnehmen.

Bewahren Sie die Tabletten nicht über 30 °C, in der Originalpackung und für Kinder unerreichbar auf.

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP:» bezeichneten Datum verwendet werden.

Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.

+-Was ist in Perindopril Actavis enthalten?

Jede Tablette Perindopril Actavis 2 mg enthält 2 mg Perindopril-tert.-Butylamin entsprechend 1,67 mg Perindopril sowie Hilfsstoffe.

Jede Tablette (mit Bruchrille) Perindopril Actavis 4 mg enthält 4 mg Perindopril-tert.-Butylamin entsprechend 3,34 mg Perindopril sowie Hilfsstoffe.

Jede Tablette Perindopril Actavis 8 mg enthält 8 mg Perindopril-tert.-Butylamin entsprechend 6,68 mg Perindopril sowie Hilfsstoffe.
