Quiril ist ein Arzneimittel, das auf Verschreibung des Arztes bzw. der Ärztin zur Behandlung des Bluthochdrucks (Hypertonie) angewendet wird. Es ist ausserdem zusammen mit einem wasserausschwemmenden Arzneimittel und/oder zusammen mit einem Herzglykosid zur Therapie der Herzleistungsschwäche (Herzinsuffizienz) geeignet. Quinapril, der Wirkstoff von Quiril, gehört zur Gruppe der Angiotensin-Converting-Enzym-Inhibitoren (ACE-Hemmer). Seine Wirkung beruht auf einer Hemmung körpereigener Stoffe, die unter anderem für einen erhöhten Blutdruck verantwortlich sind bzw. das Fortschreiten der Herzleistungsschwäche beeinflussen.
Um Ihr Befinden zu verbessern, kann Ihr Arzt bzw. Ihre Ärztin zusätzliche Massnahmen, wie z.B. eine Gewichtsabnahme und/oder Änderung der Ernährung, empfehlen.
Bei Überempfindlichkeit gegen den Wirkstoff Quinapril oder einen anderen Inhaltsstoff darf Quiril nicht eingenommen werden.
Quiril darf ausserdem nicht angewendet werden, wenn bei Ihnen im Zusammenhang mit der Einnahme anderer ACE-Hemmer Schwellungen im Gesicht, an Augen, Lippen oder Zunge aufgetreten sind.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fähigkeit Werkzeuge oder Maschinen zu bedienen und die Fahrtüchtigkeit beeinträchtigen. Daher ist Vorsicht im Strassenverkehr und beim Bedienen von Maschinen geboten, speziell zu Behandlungsbeginn.
Bitte achten Sie vor allem bei Behandlungsbeginn auf folgende Anzeichen einer möglichen Unverträglichkeit oder zu starken Wirkung:
Falls Sie nach Einnahme von Quiril Schwellungen im Gesicht, an Augen, Lippen oder Zunge bemerken oder wenn Schwierigkeiten beim Atmen auftreten, melden Sie dies bitte sofort Ihrem Arzt bzw. Ihrer Ärztin. Nehmen Sie Quiril ohne Rücksprache mit Ihrem Arzt bzw. Ihrer Ärztin nicht weiter ein.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin ebenfalls, wenn Sie speziell zu Behandlungsbeginn unter starker Benommenheit leiden. Dies könnte mit einer übermässigen Blutdrucksenkung zusammenhängen. Ihr Arzt bzw. Ihre Ärztin wird dann eine Dosisanpassung vornehmen. Die Möglichkeit eines zu starken Blutdruckabfalls besteht auch, wenn Sie z.B. aufgrund von Durchfall, Erbrechen oder übermässigem Schwitzen zu viel Flüssigkeit verloren haben. Sprechen Sie auch in diesem Fall mit Ihrem Arzt bzw. Ihrer Ärztin.
Treten bei Ihnen Anzeichen einer Infektion auf (z.B. Fieber, Halsschmerzen), die nicht innerhalb von 2-3 Tagen wieder abklingen, suchen Sie bitte unverzüglich Ihren Arzt bzw. Ihre Ärztin auf. Er bzw. sie kann durch eine Blutuntersuchung feststellen, ob diese Erscheinungen mit der Quiril-Behandlung zusammenhängen.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden (besonders wenn es sich um Erkrankungen der Blutgefässe handelt); Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
Dies gilt vor allem für wasserausschwemmende Arzneimittel (Diuretika), Kaliumpräparate oder kaliumhaltige Diätsalze, Lithiumpräparate, tetracyclinhaltige Antibiotika, Insulin oder orale Antidiabetika.
Falls Sie sich einer Operation, Narkose oder Dialysebehandlung unterziehen müssen, teilen Sie Ihrem Arzt bzw. Ihrer Ärztin bitte mit, dass Sie einen ACE-Hemmer einnehmen. Ebenfalls sollten Sie Ihrem Arzt bzw. Ihrer Ärztin mitteilen, dass Sie einen ACE-Hemmer einnehmen, wenn bei Ihnen eine Desensibilisierungstherapie mit Bienen- oder Wespengift vorgesehen ist. Selten kann es auch nach Insektenstichen zu einer Allergie kommen.
Quiril darf in der Schwangerschaft grundsätzlich nicht eingenommen werden, da ein klares Risiko für das ungeborene Kind besteht.
Während der Anwendung von Quiril soll nicht gestillt werden.
Quiril kann unabhängig von den Mahlzeiten mit etwas Flüssigkeit eingenommen werden. Die Dosierung wird von Ihrem Arzt bzw. Ihrer Ärztin festgelegt. Im Allgemeinen gelten folgende Dosierungsrichtlinien:
Patientinnen und Patienten, die nicht mit wasserausschwemmenden Arzneimitteln (Diuretika) behandelt werden, erhalten als Anfangsdosis 1× täglich 1 Filmtablette Quiril 10 mg. Bei Patientinnen und Patienten mit Diuretikavorbehandlung wird eine Anfangsdosis von 1× täglich 1 Filmtablette Quiril 5 mg empfohlen, um die Wirkung von Quiril auf den Blutdruck festzustellen. Dazu sollte der Patient bzw. die Patientin 2?4 Stunden nach der ersten Einnahme unter ärztlicher Aufsicht bleiben.
Ist eine weitere Blutdrucksenkung erforderlich, kann der Arzt bzw. die Ärztin die Dosierung bis auf 2 Filmtabletten Quiril 20 mg täglich steigern.
Die Dosierung bei Langzeitanwendung richtet sich nach dem Ansprechen des Blutdrucks und liegt gewöhnlich bei 2 Filmtabletten Quiril 10 mg bzw. 1 Filmtablette Quiril 20 mg täglich. Bei älteren Patientinnen und Patienten und bei Patientinnen und Patienten mit eingeschränkter Nierenfunktion wird ebenfalls eine Anfangsdosis von 1× täglich 1 Filmtablette Quiril 5 mg empfohlen. Anschliessend wird Ihr Arzt bzw. Ihre Ärztin die Dosierung für die Langzeitanwendung je nach Ansprechen des Blutdrucks einstellen.
Es empfiehlt sich, die Behandlung herzinsuffizienter Patientinnen und Patienten mit Quiril unter strenger Kontrolle des Arztes bzw. der Ärztin durchzuführen. Die Anfangsdosierung beträgt im Allgemeinen 1× täglich 1 Filmtablette Quiril 5 mg. Dabei sollte die erste Einnahme unter ärztlicher Aufsicht (ca. 2?4 Stunden) erfolgen. Danach kann (bei gleichzeitiger Behandlung mit Diuretika und/oder Herzglykosiden) die Tagesdosis schrittweise auf bis zu 2 Filmtabletten Quiril 20 mg täglich angepasst werden.
Zur Anwendung von Quiril bei Kindern und Jugendlichen liegen bisher keine klinischen Erfahrungen vor. Die Anwendung des Präparates bei Kindern und Jugendlichen kann daher nicht empfohlen werden.
Ändern Sie nicht von sich aus die vorgeschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme bzw. Anwendung von Quiril auftreten:
Bisher beobachtete unerwünschte Wirkungen waren gewöhnlich geringfügig ausgeprägt und von vorübergehender Natur.
Häufig beobachtet wurden Schwindel, Kopfschmerzen, zu niedriger Blutdruck, Übelkeit, Erbrechen, Durchfall, Bauchschmerzen, Husten, Halsschmerzen, Schnupfen, Kurzatmigkeit, Brustschmerzen, Müdigkeit, Schlaflosigkeit, Rückenschmerzen, Muskelschmerzen, Missempfindungen (z.B. Kribbeln).
Gelegentlich kann es zu Mundtrockenheit, Herzklopfen, Nervosität, Schläfrigkeit, Gesichtsrötung, Magenbeschwerden, Blähungen, Infektionen des Harntraktes, Impotenz, Sehschwäche, Verstimmungszuständen, Hautausschlägen, Juckreiz, verstärktem Schwitzen, Wasseransammlung in den Geweben, erhöhter Lichtempfindlichkeit, Gelenkschmerzen und Fieber kommen.
In seltenen Fällen kann Quiril zu Leberschädigungen führen. Beim Auftreten von Müdigkeit, Appetitlosigkeit, Übelkeit, entfärbtem Stuhl, dunklem Urin oder Gelbsucht sollten Sie umgehend mit Ihrem Arzt bzw. Ihrer Ärztin Kontakt aufnehmen, damit das Arzneimittel bei Bedarf abgesetzt werden kann und die erforderlichen Untersuchungen durchgeführt werden können. Beachten Sie bitte auch die Hinweise unter «Wann ist bei der Einnahme von Quiril Vorsicht geboten?».
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Bewahren Sie Quiril bei Raumtemperatur (15?25 °C) in der verschlossenen Originalpackung an einem für Kinder unerreichbaren Ort auf.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «Exp.» bezeichneten Datum verwendet werden.
Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.
Quiril 5 enthält als Wirkstoff 5,42 mg Quinaprilhydrochlorid (entspr. 5 mg Quinapril) sowie Hilfsstoffe und ist erhältlich als teilbare Filmtablette.
Quiril 10 enthält als Wirkstoff 10,84 mg Quinaprilhydrochlorid (entspr. 10 mg Quinapril) sowie Hilfsstoffe. Die Filmtabletten verfügen über eine Bruchrille, sind aber nicht zum Teilen geeignet.
Quiril 20 enthält als Wirkstoff 21,7 mg Quinaprilhydrochlorid (entspr. 20 mg Quinapril) sowie Hilfsstoffe. Die Filmtabletten verfügen über eine Bruchrille, sind aber nicht zum Teilen geeignet.
