+-Was ist Ranexa und wann wird es angewendet?

Auf Verschreibung des Arztes wird Ranexa in Kombination mit anderen Arzneimitteln zur Behandlung von Angina pectoris verwendet. Bei Angina pectoris handelt es sich um Brustschmerzen oder Beschwerden, die an anderen Stellen Ihres Oberkörpers zwischen Hals und Oberbauch auftreten, häufig ausgelöst durch körperliche Bewegung oder zu viel Aktivität.

Sprechen Sie mit Ihrem Arzt oder Apotheker, falls Sie sich nicht besser fühlen oder falls Sie sich schlechter fühlen.

+-Wann darf Ranexa nicht angewendet werden?

Ranexa darf nicht eingenommen werden,

  • wenn Sie überempfindlich (allergisch) gegen Ranolazin oder einen der sonstigen in dieser Packungsbeilage aufgeführten Bestandteile von Ranexa sind.
  • wenn Sie schwerwiegende Probleme mit den Nieren haben.
  • wenn Sie mässig starke oder schwerwiegende Probleme mit der Leber haben.
  • wenn Sie bestimmte Arzneimittel zur Behandlung von bakteriellen Infektionen (Clarithromycin, Telithromycin), Pilzinfektionen (Itraconazol, Ketoconazol, Voriconazol, Posaconazol), HIV-Infektion (Proteasenhemmer), Depression (Nefazodon) oder Herzrhythmusstörungen (z.B. Quinidin, Dofetilid oder Sotalol) anwenden.
+-Wann ist bei der Anwendung von Ranexa Vorsicht geboten?

Besondere Vorsicht bei der Einnahme von Ranexa ist erforderlich:

Sprechen Sie vor der Einnahme von Ranexa mit Ihrem Arzt:

  • wenn Sie leichte oder mässig starke Probleme mit den Nieren haben.
  • wenn Sie leichte Probleme mit der Leber haben.
  • wenn Sie jemals ein Elektrokardiogramm (EKG) mit anormalen Werten hatten
  • wenn Sie älter sind.
  • wenn Sie ein geringes Gewicht haben (60 kg oder weniger).
  • wenn Sie an Herzinsuffizienz leiden.

Wenn eine oder mehrere dieser Bedingungen auf Sie zutreffen, wird Ihr Arzt Ihnen möglicherweise eine geringere Dosis verordnen oder sonstige Vorsichtsmassnahmen treffen. 

Einnahme von Ranexa mit anderen Arzneimitteln:

Wenn Sie Ranexa einnehmen, dürfen Sie die folgenden Arzneimittel nicht anwenden:

  • bestimmte Arzneimittel zur Behandlung von bakteriellen Infektionen (Clarithromycin, Telithromycin), Pilzinfektionen (Itraconazol, Ketoconazol, Voriconazol, Posaconazol), HIV-Infektion (Proteasenhemmer), Depression (Nefazodon) oder Herzrhythmusstörungen (z.B. Quinidin, Dofetilid oder Sotalol)

Bitte informieren Sie vor der Einnahme von Ranexa Ihren Arzt oder Apotheker:

  • wenn Sie bestimmte Arzneimittel zur Behandlung einer bakteriellen Infektion (Erythromycin) oder einer Pilzinfektion (Fluconazol) oder ein Arzneimittel zur Verhinderung der Abstossung eines transplantierten Organs (Ciclosporin) anwenden, oder wenn Sie bestimmte Herztabletten wie Diltiazem oder Verapamil einnehmen. Diese Arzneimittel können zu vermehrtem Auftreten von Nebenwirkungen wie Schwindel, Übelkeit oder Erbrechen führen, die mögliche Nebenwirkungen von Ranexa sind (siehe Abschnitt «Welche Nebenwirkungen kann Ranexa haben?»). Ihr Arzt wird Ihnen dann möglicherweise eine niedrigere Dosis verordnen.
  • wenn Sie Arzneimittel zur Behandlung von Epilepsie oder anderer neurologischer Störungen einnehmen (z.B. Phenytoin, Carbamazepin oder Phenobarbital), wenn Sie wegen einer Infektion (z.B. Tuberkulose) Rifampicin einnehmen, oder wenn Sie das pflanzliche Mittel Johanniskraut einnehmen, da diese Arzneimittel die Wirkung von Ranexa beeinträchtigen können.
  • wenn Sie Arzneimittel zur Behandlung von Herzerkrankungen einnehmen, die Digoxin oder Metoprolol enthalten, da Ihr Arzt die Dosis dieses Arzneimittels möglicherweise ändern will, solange Sie Ranexa einnehmen.
  • wenn Sie bestimmte Arzneimittel zur Behandlung von Allergien (z.B. Terfenadin, Astemizol, Mizolastin), Herzrhythmusstörungen (z.B. Disopyramid, Procainamid) und Depression (z.B. Imipramin, Doxepin, Amitriptylin) anwenden, da diese Arzneimittel Ihr EKG beeinflussen können.
  • wenn Sie bestimmte Arzneimittel zur Behandlung von Depression (Bupropion), Psychose, HIV-Infektion (Efavirenz) oder Krebs (Cyclophosphamid) anwenden.
  • wenn Sie bestimmte Arzneimittel zur Behandlung von erhöhten Cholesterinwerten im Blut (z.B. Simvastatin, Atorvastatin) einnehmen. Diese Arzneimittel können Muskelschmerzen, Muskelempfindlichkeit oder ?schwäche auslösen. Ihr Arzt wird Ihnen dann möglicherweise die Dosis dieses Arzneimittels reduzieren, während Sie Ranexa einnehmen.
  • wenn Sie bestimmte Arzneimittel einnehmen, welche verwendet werden, um die Abstossung von transplantierten Organen zu verhindern (z.B. Tacrolimus, Ciclosporin, Sirolimus, Everolimus). Ihr Arzt wird Ihnen dann möglicherweise die Dosis dieses Arzneimittels reduzieren, während Sie Ranexa einnehmen. 

Einnahme von Ranexa zusammen mit Nahrungsmitteln und Getränken:

Ranexa kann vor oder nach den Mahlzeiten eingenommen werden. Sie dürfen keinen Grapefruitsaft trinken, solange Sie mit Ranexa behandelt werden. 

Patientinnen und Patienten, die auf den Farbstoff Tartrazin (E102), Acetylsalicylsäure sowie Rheuma- und Schmerzmittel (Prostaglandinhemmer) überempfindlich reagieren, sollen die Tabletten mit 750 mg Ranolazin nicht anwenden.

Die 750 Retardtabletten enthalten Lactose. Wenn Ihr Arzt festgestellt hat, dass bei Ihnen eine Unverträglichkeit gegenüber bestimmten Zuckern vorliegt, sprechen Sie mit Ihrem Arzt, bevor Sie dieses Arzneimittel einnehmen.

Es wurden keine Studien zu den Auswirkungen von Ranexa auf die Verkehrstüchtigkeit und die Fähigkeit zum Bedienen von Maschinen durchgeführt. Fragen Sie in Bezug auf die Verkehrstüchtigkeit und das Bedienen von Maschinen Ihren Arzt um Rat.

Ranexa kann Nebenwirkungen wie Schwindel (häufig) und verschwommenes Sehen (gelegentlich), Verwirrtheit (gelegentlich), Halluzinationen (gelegentlich), Doppeltsehen (gelegentlich) und Koordinationsstörungen (selten) verursachen, die Ihre Verkehrstüchtigkeit und Ihre Fähigkeit zum Bedienen von Maschinen beeinträchtigen können. Wenn Sie Symptome dieser Art bei sich feststellen, dürfen Sie kein Fahrzeug führen und keine Maschinen bedienen, bis die Symptome vollständig zurückgegangen sind. 

Informieren Sie Ihren Arzt oder Apotheker, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte) einnehmen.

+-Darf Ranexa während einer Schwangerschaft oder in der Stillzeit angewendet werden?

Sofern vom Arzt nicht anders verordnet, dürfen Sie Ranexa nicht einnehmen, wenn Sie schwanger sind. Sie dürfen Ranexa nicht einnehmen, wenn Sie stillen. Fragen Sie Ihren Arzt um Rat bevor Sie Ranexa einnehmen, wenn Sie schwanger sind, vermuten schwanger zu sein, planen schwanger zu werden oder stillen.

+-Wie verwenden Sie Ranexa?

Nehmen Sie Ranexa immer genau nach Anweisung des Arztes ein. Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker.

Schlucken Sie die Tabletten immer unzerkaut mit etwas Wasser. Sie dürfen die Tabletten nicht zerkleinern, lutschen, kauen oder halbieren, weil die besondere Art der Freisetzung des Arzneimittelwirkstoffs in Ihrem Körper dadurch beeinträchtigt werden kann.

Die übliche Anfangsdosis für Erwachsene ist eine 375 mg Tablette zweimal täglich. Nach 2?4 Wochen kann Ihr Arzt die Dosis erhöhen, um die richtige Wirkung für Sie zu erzielen. Die Maximaldosis von Ranexa beträgt 750 mg zweimal täglich.

Es ist wichtig, dass Sie Ihren Arzt informieren, wenn bei Ihnen Nebenwirkungen wie Schwindel, Übelkeit oder Erbrechen auftreten. Ihr Arzt wird die Dosis dann möglicherweise senken oder, wenn dies nicht ausreicht, die Behandlung mit Ranexa abbrechen.

Kinder und Jugendliche unter 18 Jahren dürfen Ranexa nicht anwenden. 

Wenn Sie eine grössere Menge Ranexa eingenommen haben, als Sie sollten:

Wichtig: Informieren Sie unverzüglich Ihren Arzt, wenn Sie aus Versehen zu viele Ranexa- Retardtabletten oder eine höhere Dosis als vom Arzt empfohlen eingenommen haben. Wenn Sie Ihren Arzt nicht erreichen können, begeben Sie sich in die nächste Unfall- und Notfallaufnahme einer Klinik. Nehmen Sie die übrig gebliebenen Tabletten einschliesslich des Behältnisses und des Umkartons mit, damit das Klinikpersonal leicht feststellen kann, was Sie eingenommen haben. 

Wenn Sie die Einnahme von Ranexa vergessen haben:

Wenn Sie eine Dosis vergessen haben, nehmen Sie sie ein, sobald Sie sich daran erinnern, ausser wenn es fast schon wieder Zeit für die nächste Dosis ist (weniger als 6 Stunden). Nehmen Sie nicht die doppelte Dosis ein, wenn Sie die vorige Einnahme vergessen haben.

+-Welche Nebenwirkungen kann Ranexa haben?

Wie viele Arzneimittel kann Ranexa Nebenwirkungen haben, die aber nicht bei jedem auftreten müssen.

Brechen Sie die Einnahme von Ranexa ab und suchen Sie unverzüglich Ihren Arzt auf, wenn bei Ihnen die folgenden Symptome eines Angioödems, einer seltenen, jedoch möglicherweise schwerwiegenden Erkrankung, auftreten:

  • Schwellung des Gesichts, der Zunge oder des Rachens
  • Schwierigkeiten beim Schlucken
  • Nesselausschlag oder Atemnot

Informieren Sie Ihren Arzt, wenn bei Ihnen häufige Nebenwirkungen wie Schwindel, Übelkeit oder Erbrechen auftreten. Ihr Arzt wird die Dosis dann möglicherweise senken oder die Behandlung mit Ranexa abbrechen. 

Die folgenden weiteren Nebenwirkungen können auftreten:

Häufige Nebenwirkungen (die bei 1 bis 10 Anwendern von 100 auftreten) sind:

Verstopfung, Schwindel, Kopfschmerzen, Übelkeit, Erbrechen, Schwächegefühl

Gelegentliche Nebenwirkungen (die bei 1 bis 10 Anwendern von 1.000 auftreten) sind:

Beklemmung, Schlafstörungen, Verwirrtheit, Halluzination, verschwommenes Sehen, Sehstörung, Doppeltsehen, veränderte Sinneswahrnehmung (Tast- oder Geschmackssinn), Empfindungsstörungen, Zittern, Müdigkeits- oder Trägheitsgefühl, Schläfrigkeit oder Benommenheit, Schwächeanfall oder Ohnmacht, Schwindel beim Aufstehen, dunkler Harn, Blut im Harn, Probleme beim Harnlassen, Austrocknung, Atembeschwerden, Husten, Nasenbluten, starkes Schwitzen, Juckreiz, Schwellungs- oder Blähungsgefühl, fliegende Hitze, niedriger Blutdruck, vermehrter Gehalt einer Substanz namens Kreatinin oder vermehrter Harnstoffgehalt im Blut, vermehrte Anzahl von Blutplättchen oder weissen Blutkörperchen, veränderte EKG-Werte, Gelenkschwellung, Schmerz in den Extremitäten, Appetitlosigkeit und/oder Gewichtsverlust, Muskelkrampf, Muskelschwäche, Klingeln in den Ohren und/oder Drehschwindelgefühl, Magenschmerzen oder -beschwerden, Verdauungsstörung, Mundtrockenheit oder Abgehen von Darmwinden.

Seltene Nebenwirkungen (die bei 1 bis 10 Anwendern von 10.000 auftreten) sind:

Harnretention, anormale Laborwerte für die Leber, akutes Nierenversagen, verändertes Geruchsempfinden, Taubheit an Mund oder Lippen, eingeschränktes Hörvermögen, kalter Schweiss, Ausschlag, Koordinationsstörungen, abfallender Blutdruck beim Aufstehen, Schwinden oder Verlust des Bewusstseins, Desorientiertheit, Kältegefühl in Händen und Beinen, Nesselausschlag, allergische Hautreaktion, Impotenz, Gleichgewichtsstörungen, Entzündung von Bauchspeicheldrüse oder Darm, Gedächtnisverlust, Engegefühl im Rachen.

Patientinnen und Patienten, die auf den Farbstoff Tartrazin (E102), Acetylsalicylsäure sowie Rheuma und Schmerzmittel (Prostaglandinhemmer) überempfindlich reagieren, sollen Ranexa 750 mg nicht anwenden (kann Überempfindlichkeitsreaktionen der Haut- und Atmungsorgane auslösen).

Informieren Sie bitte Ihren Arzt oder Apotheker, wenn Sie Nebenwirkungen bemerken, auch Nebenwirkungen, die hier nicht beschrieben sind.

+-Was ist ferner zu beachten?

Das Arzneimittel ist bei Raumtemperatur (15-25 °C) und ausser der Reichweite von Kindern aufzubewahren. Es darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden. Arzneimittel sollen nicht im Abwasser oder Haushaltsabfall entsorgt werden. Fragen Sie Ihren Apotheker oder Ihre Apothekerin wie das Arzneimittel zu entsorgen ist, wenn Sie es nicht mehr benötigen. Diese Massnahme hilft die Umwelt zu schützen.

+-Was ist in Ranexa enthalten?

Ranexa 375: Die ovalen blassblauen Tabletten enthalten 375 mg Ranolazin (Wirkstoff), den Farbstoff Indigotin (E132) sowie weitere Hilfsstoffe zur Herstellung einer Retardtablette. Die Tabletten tragen auf einer Seite die Prägung 375.

Ranexa 500: Die ovalen hellorangen Tabletten enthalten 500 mg Ranolazin (Wirkstoff), den Farbstoff Eisenoxid (E172) sowie weitere Hilfsstoffe zur Herstellung einer Retardtablette. Die Tabletten tragen auf einer Seite die Prägung 500.

Ranexa 750: Die ovalen, blassgrünen Tabletten enthalten 750 mg Ranolazin (Wirkstoff), Lactose, die Farbstoffe Brillantblau FCF (E133) und Tartrazin (E102) sowie weitere Hilfsstoffe zur Herstellung einer Retardtablette. Die Tabletten tragen auf einer Seite die Prägung 750.
