+-Was ist Sutent und wann wird es angewendet?

Sutent Kapseln enthalten den Wirkstoff Sunitinib, der die Teilung und Vermehrung von Tumorzellen hemmt.

Sutent wird zur Behandlung von Nierenzellkarzinomen eingesetzt, welche fortgeschritten sind und/oder Ableger im Körper gebildet haben.

Sutent wird auch zur Behandlung von gastrointestinalen Stromatumoren (einer speziellen Form von Tumoren im Magen-Darm-Trakt) verwendet, wenn eine Therapie mit Imatinib, einem anderen Tumor-Arzneimittel, nicht wirkt oder nicht vertragen wird.

Sutent wird zudem zur Behandlung von fortgeschrittenen und/oder metastasierenden neuroendokrinen Tumoren des Pankreas eingesetzt. Dabei handelt es sich um Tumore der hormonproduzierenden Zellen in der Bauchspeicheldrüse.

Sutent darf nur auf Verschreibung des Arztes bzw. der Ärztin angewendet werden.

+-Wann darf Sutent nicht eingenommen werden?

Bei Überempfindlichkeit gegenüber Sunitinib oder einem der in Sutent enthaltenen Hilfsstoffe.

+-Wann ist bei der Einnahme von Sutent Vorsicht geboten?

Sutent wird Ihnen nur durch einen Arzt bzw. eine Ärztin mit Erfahrung in der Krebsbehandlung verschrieben. Folgen Sie sorgfältig allen Anweisungen des Arztes bzw. der Ärztin.

Besondere Vorsicht ist in folgenden Fällen geboten:

  • Blutgerinnsel: Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie kürzlich Probleme mit Blutgerinnseln in Ihren Venen oder Arterien (Arten von Blutgefässen) hatten, inklusive Schlaganfall, Herzinfarkt, Embolie oder Thrombose. Wenn bei Ihnen unter der Behandlung mit Sutent folgende Beschwerden auftreten, müssen Sie sofort medizinische Nothilfe in Anspruch nehmen und Ihren Arzt oder Ihre Ärztin informieren: Brustschmerzen oder Druckgefühl in der Brust, Schmerzen in den Armen, dem Rücken, dem Nacken oder dem Kiefer, Kurzatmigkeit, Atemnot, Taubheitsgefühl oder Schwäche einer Körperhälfte, Sprachstörungen, Lähmungserscheinungen, Sehstörungen, Kopfschmerzen oder Schwindel.
  • Blutungen: Unter der Behandlung mit Sutent kann es zu teilweise starken Blutungen in Organen kommen. Da dies lebensbedrohlich sein kann, sollten Sie sofort medizinische Nothilfe in Anspruch nehmen, wenn Sie folgende Symptome feststellen: schwarzer oder blutiger Stuhl, blutiger Urin, schmerzhafter, geschwollener Magen (Bauch), Erbrechen von Blut oder blutiger Auswurf, starkes Nasenbluten, starke bis stärkste Kopfschmerzen (teilweise kombiniert mit Erbrechen), Lähmungen, Sprach-, Seh- oder Gefühlsstörungen.
  • Herz: Unter der Behandlung mit Sutent kann es zu einer Beeinträchtigung der Herzfunktion, einer Minderversorgung des Herzmuskels mit Sauerstoff (einschliesslich Herzinfarkt) und einer Beeinflussung der Reizleitung des Herzens kommen. Der Arzt bzw. die Ärztin wird deshalb vor und während der Behandlung regelmässig Ihre Herzfunktion und das EKG kontrollieren. Informieren Sie umgehend Ihren Arzt oder Ihre Ärztin, wenn Schwindel, Ohnmacht oder Brustschmerzen (Angina pectoris) auftreten, sowie wenn Sie während der Einnahme von Sutent einige der folgenden Symptome haben: Kurzatmigkeit, Leistungsschwäche, Erschöpfung, Gewichtszunahme und häufiges nächtliches Wasserlassen, verbunden mit geschwollenen Füssen/Beinen, geschwollenem oder straffem Bauch und Appetitlosigkeit, und/oder mit Atemnot und Husten mit schaumigem Auswurf.
  • Blutdruck: Unter der Behandlung mit Sutent kann ein Bluthochdruck entstehen. Ihr Arzt bzw. Ihre Ärztin wird Ihren Blutdruck aus diesem Grund überwachen und bei Bedarf geeignete Massnahmen treffen.
  • Krampfanfälle: In seltenen Fällen kann es unter der Behandlung mit Sutent zu Krampfanfällen kommen. Informieren Sie Ihren Arzt bzw. Ihre Ärztin wenn Sie an Krämpfen leiden oder gelitten haben, und nehmen Sie sofort medizinische Nothilfe in Anspruch, wenn bei Ihnen unter der Behandlung mit Sutent folgende Beschwerden auftreten: Kopfschmerzen, Konzentrationsprobleme/Verwirrtheit, Krämpfe (Anfälle) und Sehstörungen mit oder ohne hohen Blutdruck.
  • Tumorlyse-Syndrom: Unter der Behandlung mit Sutent kann es in seltenen Fällen durch raschen Zerfall von Tumorzellen und die dabei freiwerdenden Substanzen zu einer Stoffwechselentgleisung kommen (Tumorlyse-Syndrom). Informieren Sie umgehend Ihren Arzt bzw. Ihre Ärztin, wenn Sie sich schwach fühlen, Muskelkrämpfe, und/oder Magendarmbeschwerden haben und/oder viel weniger Urin als normalerweise produzieren.
  • Haut: Unter der Behandlung mit Sutent kann es in seltenen Fällen zu schweren Hautausschlägen kommen, welche lebensbedrohlich sein können. Wenn bei Ihnen Hautausschlag mit Bläschenbildung, der an kleine Zielscheiben erinnert (Erythema multiforme), oder ausgedehnter Hautausschlag mit Bläschenbildung und Schälen der Haut, insbesondere im Bereich von Mund, Nase, Augen und Genitalien und möglicherweise kombiniert mit Fieber (Stevens-Johnson-Syndrom, toxisch-epidermale Nekrolyse) auftritt, sollten Sie umgehend Ihren Arzt/Ihre Ärztin informieren.
  • Muskeln: In seltenen Fällen kann es unter der Behandlung mit Sutent zu schwerwiegenden Muskelproblemen kommen, welche zu einer Schädigung der Nieren führen können. Kontaktieren Sie umgehend Ihren Arzt bzw. Ihre Ärztin, wenn ungeklärte Muskelschmerzen, Muskelempfindlichkeit oder -Schwäche auftreten.
  • Verdauungsapparat: Während der Behandlung mit Sutent kann es gelegentlich zu Problemen im Verdauungsapparat kommen wie einem Durchbruch (Perforation) im Magen-Darm-Trakt oder einer Entzündung der Bauchspeicheldrüse oder der Gallenblase. Ihr Arzt oder Ihre Ärztin wird Sie daher regelmässig auf entsprechende Symptome untersuchen lassen. Informieren Sie unverzüglich Ihren Arzt oder Ihre Ärztin wenn bei Ihnen folgenden Beschwerden auftreten: plötzlich auftretende starke Bauchschmerzen, Verhärtung der Bauchdecke, Übelkeit, Erbrechen und Fieber.
  • Leber: Sutent kann die Funktion der Leber beeinflussen. Informieren Sie umgehend Ihren Arzt oder Ihre Ärztin, wenn bei Ihnen unter der Behandlung mit Sutent folgende Beschwerden auftreten: gelbliche Verfärbung der Haut oder der Augen, Dunkelfärbung des Urins und/oder Bauchschmerzen im rechten oberen Bereich. Je nach Befund muss eventuell die Behandlung mit Sutent unterbrochen oder abgesetzt werden.
  • Blutzucker: Sutent kann zu tiefen Blutzuckerwerten führen. Falls Sie an Diabetes leiden und mit Insulin oder oralen Antidiabetika behandelt werden, sollten die Blutzuckerwerte regelmässig überwacht werden, da die Dosierung Ihrer aktuellen Therapie möglicherweise angepasst werden muss.
  • Wundheilung: Sutent kann die Wundheilung beeinträchtigen. Falls Sie sich einer grossen Operation unterziehen müssen, wird der Arzt bzw. die Ärztin die Behandlung mit Sutent möglicherweise während dieser Zeit unterbrechen, damit die Wundheilung nicht beeinträchtigt wird.
  • Zähne: Vor Beginn der Behandlung mit Sutent wird der Arzt bzw. die Ärztin Sie möglicherweise anweisen, eine zahnärztliche Kontrolle vornehmen zu lassen. Falls Sie Schmerzen im Mund, den Zähnen und/oder im Kiefer, Schwellungen oder Geschwüre im Mund, Taubheitsgefühl oder ein Gefühl der Schwere im Kiefer haben oder hatten, oder meinen, einen Zahn zu verlieren, informieren Sie umgehend Ihren Arzt bzw. Ärztin oder Zahnarzt bzw. Zahnärztin.
    Falls Sie sich einer invasiven zahnärztlichen Behandlung oder einem zahnärztlichen chirurgischen Eingriff unterziehen müssen, informieren Sie Ihren Zahnarzt bzw. Ihre Zahnärztin, dass Sie mit Sutent behandelt werden, insbesondere wenn Sie intravenöse Gabe von bestimmten Arzneimitteln für den Knochenstoffwechsel (Bisphosphonate) erhalten oder erhalten haben.
  • Schilddrüse: Sutent kann zu Problemen mit der Schilddrüse führen. Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie während der Einnahme von Sutent schneller ermüden, kälteempfindlicher als andere Menschen sind oder wenn Ihre Stimme tiefer wird. Ihr Arzt oder Ihre Ärztin wird deshalb Ihre Schilddrüsenfunktion vor Beginn und regelmässig während der Einnahme von Sutent überprüfen. Je nach Befund wird gegebenenfalls eine Behandlung mit Schilddrüsenhormon erfolgen.

Sutent kann die Funktion verschiedener Organe wie z.B. Herz, Nieren, Leber, Bauchspeicheldrüse, Schilddrüse und Knochenmark beeinflussen. Ihr Arzt bzw. Ihre Ärztin wird daher gegebenenfalls vor und regelmässig während der Behandlung mit Sutent Untersuchungen (z.B. Blutwerte und/oder Urinproben, EKG, Blutdruck) durchführen. Je nach Befund muss eventuell die Behandlung mit Sutent verschoben, unterbrochen oder abgesetzt werden.

Wechselwirkungen mit anderen Arzneimitteln:

Gewisse Arzneimittel wie Pilzmittel (z.B. Ketoconazol, Itraconazol, Voriconazol), Antibiotika (z.B. Erythromycin, Clarithromycin), Cimetidin (zur Behandlung von Magen- und Zwölffingerdarmgeschwüren), Diltiazem (zur Behandlung von Bluthochdruck und Herzkrankheiten), Arzneimittel gegen Depressionen (z.B. Sertralin, Paroxetin, Venlafaxin) und HIV-Arzneimittel sowie Grapefruitsaft können den Abbau von Sunitinib, den Wirkstoff von Sutent, hemmen und die Wirkung von Sutent verstärken. Der Arzt bzw. die Ärztin wird entscheiden, ob Sie solche Arzneimittel mit Sutent zusammen anwenden dürfen, und allenfalls die Dosis von Sutent reduzieren.

Gewisse Arzneimittel wie das Antibiotikum Rifampicin, Arzneimittel gegen Epilepsie (z.B. Phenytoin, Carbamazepin, Phenobarbital), Kortikosteroide (Cortison-artige Arzneimittel) oder Johanniskraut-Präparate gegen Depressionen können den Abbau von Sunitinib beschleunigen und die Wirkung von Sutent aufheben. Der Arzt bzw. die Ärztin wird entscheiden, ob Sie solche Arzneimittel mit Sutent zusammen anwenden dürfen, und allenfalls die Dosis von Sutent erhöhen.

Wenn Sie gleichzeitig mit Sutent ein blutgerinnungshemmendes Arzneimittel (Phenprocoumon, Acenocoumarol) oder ein Arzneimittel, das den Herzrhythmus beeinflusst (z.B. Chinidin, Sotalol, Flecainid, Disopyramid, Indapamid, Risperidon, Haloperidol), anwenden, wird Sie der Arzt bzw. die Ärztin regelmässig kontrollieren.

Fahrtüchtigkeit und Bedienen von Maschinen

Aufgrund der möglichen unerwünschten Wirkungen wie Schwindel, Erbrechen und Übelkeit kann Sutent die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Es ist deshalb Vorsicht geboten.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin, Apothekerin, wenn Sie

  • an anderen Krankheiten leiden,
  • Allergien haben oder
  • andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
+-Darf Sutent während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Sutent darf während der Schwangerschaft grundsätzlich nicht eingenommen werden (abgesehen von aussergewöhnlichen Situationen). Frauen im gebärfähigen Alter, die mit Sutent behandelt werden, müssen eine sichere Schwangerschaftsverhütung betreiben. Der Arzt bzw. die Ärztin wird Sie über die für Sie geeignete Empfängnisverhütungsmethode beraten. Falls während der Behandlung mit Sutent dennoch eine Schwangerschaft eintreten sollte, ist der Arzt bzw. die Ärztin umgehend zu informieren.

Während einer Behandlung mit Sutent soll nicht gestillt werden.

+-Wie verwenden Sie Sutent?

Folgen Sie bitte sorgfältig den Anweisungen Ihres Arztes bzw. Ihrer Ärztin. Er bzw. sie begleitet Ihre Behandlung und überprüft den Erfolg und die Verträglichkeit der Therapie. Er bzw. sie entscheidet über die Dauer der Behandlung mit Sutent, über eine allfällige Dosisanpassung oder über eine vorübergehende Unterbrechung der Behandlung.

Für die Behandlung von Nierenzellkarzinomen oder gastrointestinalen Stromatumoren beträgt die Dosierung üblicherweise 50 mg einmal täglich. Sutent wird während 4 Wochen eingenommen, dann folgt eine Pause von 2 Wochen. Anschliessend beginnt ein neuer Behandlungszyklus.

Für die Behandlung von neuroendokrinen Tumoren des Pankreas beträgt die Dosierung üblicherweise 37.5 mg einmal täglich ohne Behandlungspausen.

Ihr Arzt oder Ihre Ärztin kann anschliessend die Dosis erhöhen oder vermindern, je nachdem wie Sie auf Sutent ansprechen.

Sutent Kapseln sollen mit etwas Wasser eingenommen werden. Sie können mit oder ohne Nahrung eingenommen werden. Vor der Einnahme soll man sich vergewissern, dass die Kapseln unbeschädigt sind. Sie dürfen nicht gelutscht, zerbissen oder gekaut und auch nicht geöffnet werden.

Falls Sie eine Einnahme vergessen haben, holen Sie diese nicht nach, sondern nehmen Sie am nächsten Tag wie gewohnt eine Kapsel ein.

Falls Sie an einer Leber- oder Nierenfunktionsstörung leiden, wird der Arzt bzw. die Ärztin Sutent bei Ihnen mit spezieller Vorsicht anwenden.

Da Sutent bei Kindern und Jugendlichen nicht untersucht worden ist, soll es bei diesen Altersgruppen nicht angewendet werden.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Sutent haben?

Es können während der Behandlung mit Sutent Gelbverfärbung oder Depigmentierung der Haut (bei ca. 25%) und/oder Haarverfärbungen (bei ca. 12%) auftreten. Diese sind jedoch von vorübergehender Natur und sollten Sie nicht weiter beunruhigen.

Folgende weiteren Nebenwirkungen können bei der Einnahme von Sutent auftreten:

Sehr häufig (bei mehr als 1 von 10 Personen): Schwere Infektionen (z.B. Lungenentzündung, Bronchitis, Harnwegsinfektionen, Entzündungen des Unterhautbindegewebes, Abszesse, Blutvergiftung), Verminderung der weissen und roten Blutkörperchen sowie der Blutplättchen, Unterfunktion der Schilddrüse, verminderter Appetit, Schlaflosigkeit, Störungen des Geschmackempfindens, Kopfschmerzen, Bluthochdruck, Atemnot in Ruhe oder bei normaler körperlicher Belastung, Nasenbluten, Durchfall, Übelkeit, Erbrechen, Bauchschmerzen, Mundschleimhautentzündung, Verstopfung, Verdauungsstörungen, Brennen, Rötung und/oder Blasenbildung auf Handflächen und Fusssohlen, Hautverfärbung, Hautausschlag, Veränderung der Haarfarbe, trockene Haut, Schmerzen in Armen und Beinen, Gelenkschmerzen, Müdigkeit, Schleimhautentzündungen, Ödeme (Flüssigkeitsansammlung im Gewebe von Beinen/Armen und Gesicht), Fieber.

Häufig (bei 1-10 von 100 Personen): Flüssigkeitsverlust, tiefe Blutzuckerwerte (Hypoglykämie), Gewichtsverlust, Depression, Schwindel, Gefühlsstörung in Armen und Beinen (z.B. Kribbeln, Jucken, Kälte- bzw. Wärmegefühl oder «Ameisenlaufen», sogenannte Parästhesien), verstärkte Tränenbildung, Ödeme (Flüssigkeitsansammlung) in Augenliedern und um Augen, erhöhter Puls, Beeinträchtigung der Herzfunktion, Myokardischämie (Minderversorgung des Herzmuskels mit Sauerstoff), Bildung von Blutgerinnseln und dadurch Verstopfung von Gefässen (thomboembolische Ereignisse), Lungenembolie, Schmerzen im Mund und Hals, Bluthusten, Blähungen, Bauchauftreibung, Mundtrockenheit, Rückfluss von Magensäure in die Speiseröhre, Entzündung der Speiseröhre, Zahnfleischbluten, Zungenbrennen, Haarausfall, Hautrötung, schälende und juckende Haut, Nagelstörungen, Muskelschmerzen, Nierenversagen, Harnverfärbung, Schmerzen im Brustbereich, Schüttelfrost, Grippeähnliche Symptome.

Gelegentlich (bei 1-10 von 1000 Personen): Überfunktion der Schilddrüse, Hirnblutung, Schlaganfall, Herzinfarkt, Herzversagen, Erkrankung des Herzmuskels, Entzündung der Bauchspeicheldrüse, Bildung eines Lochs im Darm (Perforation), Gallenblasenentzündung (meist ohne Gallensteine; mit Symptomen wie Schmerzen im rechten Oberbauch, Brechreiz und Schweissausbrüchen), Leberversagen, Knochenschaden im Kiefer (siehe «Wann ist bei der Einnahme von Sutent Vorsicht geboten?»), Störung der Nierenfunktion.

Selten (bei 1-10 von 10?000 Personen): Schwere Weichteilinfektionen mit Absterben von Gewebe (örtliche Schmerzen, Schwellung, Rötung und Wärme sowie Fieber), Überempfindlichkeitsreaktionen, teilweise mit Schwellungen von Lippen und Augenlidern, Entzündung der Schilddrüse, Tumorlyse-Syndrom (siehe «Wann ist bei der Einnahme von Sutent Vorsicht geboten?»), reversibles posteriores Leukoenzephalopathie-Syndrom (Veränderungen des Gehirns mit Symptomen wie Kopfschmerzen, Verwirrtheit, Krampfanfällen und Sehverlust),Geschmacksverlust, schwere Blutungen in Organen (z.B. Lunge, Magendarmtrakt, Harnwege, Gehirn), thrombotische Mikroangiopathie (Verschluss von kleinsten arteriellen Blutgefässen), Pyoderma gangraenosum (grossflächiges schmerzhaftes Hautgeschwür mit Absterben von Gewebe), schwere Reaktionen und Entzündungen der Haut oder Schleimhaut mit fortschreitendem Ausschlag, Blasenbildung und grossflächigem Abschälen der Haut (Stevens-Johnson-Syndrom, toxisch-epidermale Nekrolyse, Erythema multiforme).

Da Muskelprobleme in seltenen Fällen schwerwiegend sein können, sollten Sie unverzüglich Ihren Arzt bzw. Ihre Ärztin kontaktieren, wenn Sie Muskelschmerzen, Muskelempfindlichkeit oder -Schwäche verspüren.

Wenn Sie diese oder andere Nebenwirkungen, die hier nicht beschrieben sind, feststellen, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Bei Raumtemperatur (15-25 °C) und ausser Reichweite von Kindern aufbewahren.

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «EXP» bezeichneten Datum verwendet werden.

Bringen Sie nicht verwendete oder beschädigte Sutent Kapseln unbedingt zur Entsorgung in die Apotheke zurück.

+-Was ist in Sutent enthalten?

1 Kapsel zu 12.5 mg enthält 12.5 mg Sunitinib als Sunitinib-Malat und ist dunkelorange mit weissem Aufdruck «Pfizer/STN12.5mg».

1 Kapsel zu 25 mg enthält 25 mg Sunitinib als Sunitinib-Malat und ist karamellfarben und dunkelorange mit weissem Aufdruck «Pfizer/STN25mg».

1 Kapsel zu 50 mg enthält 50 mg Sunitinib als Sunitinib-Malat und ist karamellfarben mit weissem Aufdruck «Pfizer/STN50mg».

Die Kapseln enthalten zudem folgende Hilfsstoffe: Mannitol, Croscarmellose-Natrium, Povidon und Magnesiumstearat. Die Kapselhüllen bestehen aus Gelatine, Titandioxid (E171) und Eisenoxid (E172).
