+-Was ist Tirosint und wann wird es angewendet?

Tirosint enthält das synthetisch hergestellte Levothyroxin, welches mit dem natürlich von der Schilddrüse gebildeten Hormon völlig identisch ist.

Tirosint wird vor allem in den folgenden Fällen angewendet:

  • Zur Ergänzung des fehlenden Hormons bei verschiedenen Formen einer Schilddrüsneunterfunktion.
  • Bei Schilddrüsenüberfunktion kann Tirosint gemeinsam mit einem Mittel zur Hemmung der Schilddrüsenfunktion eingenommen werden, um eine stabilere Stoffwechsellage zu gewährleisten.
  • Zur Unterdrückung eines erneuten Tumorwachstums und zur Ergänzung fehlender Schilddrüsenhormone vor allem nach operativer Entfernung von bösartigen Tumoren (Malignomen) der Schilddrüse.
  • Zum Testen der Schilddrüsenfunktion.
  • Zur Verhütung einer erneuten Kropfbildung nach einer Kropfoperation oder zur Behandlung eines gutartigen Kropfes.
  • Zur Beseitigung der bei der Unterfunktion der Schilddrüse oft bestehenden Krankheitszeichen wie Antriebsarmut, Leistungsabfall, Müdigkeit, Kälteempfindlichkeit, Veränderung von Haut und Haaren oder Verstopfung.

Eine erfolgreiche Behandlung setzt die regelmässige Einnahme des Arzneimittels voraus.

+-Wann darf Tirosint nicht angewendet werden?

Tirosint darf nicht angewendet werden:

  • Bei Überfunktion der Schilddrüse. Ausnahme: Als Begleittherapie bei Einnahme eines Mittels zur Hemmung einer Schilddrüsenüberfunktion (nach Erreichen der normalen Stoffwechsellage).
  • Im Falle einer bekannten Überempfindlichkeit gegenüber dem Wirkstoff oder einem Hilfsstoff von Tirosint.
  • Bei Vorliegen eines frischen Herzinfarkts, schwerer Angina pectoris, Herzinsuffizienz mit gesteigerter Herzfrequenz sowie bei unbehandelten Funktionsstörungen der Nebennierenrinde.

Es gibt keinen Nachweis, dass Schilddrüsenhormone bei Übergewicht von Patienten bzw. Patientinnen mit einer normalen Schilddrüsenfunktion eine gewichtsvermindernde Wirkung haben. Deshalb, und da höhere Dosen sehr schwerwiegende Nebenwirkungen verursachen können, ist Tirosint zur Behandlung von Übergewicht nicht geeignet und darf nicht angewendet werden.

+-Wann ist bei der Anwendung von Tirosint Vorsicht geboten?

Informieren Sie unbedingt ihren Arzt oder Ihre Ärztin, wenn Sie herzkrank oder Diabetiker sind oder einen zu hohen Blutdruck haben, damit Ihr Arzt oder Ihre Ärztin die Behandlung entsprechend einstellen kann.

Bei Unterfunktion der Hirnanhangsdrüse oder wenn die Nebenniere aus anderen Gründen eine ungenügende Leistung zeigt, wird Ihnen der Arzt oder die Ärztin wahrscheinlich zusätzlich eine Behandlung mit Nebennierenrinden-Hormonen verschreiben.

Folgende Arzneimittel können die Wirkung von Levothyroxin verändern

  • Lipidsenker wie Colestyramin und Colestipol: Bei gleichzeitiger Einnahme von Colestyramin oder Colestipol gegen erhöhte Cholesterinspiegel (Lipidsenker) wird die Aufnahme von Schilddrüsenhormonen aus dem Darm in das Blut vermindert. Daher sollte Tirosint 4?5 Stunden vor der Colestyramin- bzw. Colestipol-Dosis eingenommen werden.
  • Eisenpräparate oder aluminiumhaltige Mittel und andere Arzneimittel gegen Magenübersäuerung und Sodbrennen (sog. Antazida oder Sucralfat) können die Aufnahme von Tirosint aus dem Darm hemmen. Sie sollten in einem zeitlichen Abstand von mindestens 2 Stunden nach Tirosint eingenommen werden.
  • Eine gleichzeitige Einnahme von Empfängnisverhütungsmitteln («Pille») kann den Bedarf an Tirosint erhöhen. Wenn Sie erstmals ein Empfängnisverhütungsmittel einnehmen oder das Mittel wechseln, informieren Sie Ihren Arzt bzw. Ärztin. Die Dosierung von Tirosint muss gegebenenfalls angepasst werden.
  • Arzneimittel gegen epileptische Anfälle (Carbamazepin, Phenytoin, Phenobarbital) und das Antibiotikum Rifampicin können die Wirkung von Tirosint verkürzen bzw. abschwächen.
  • Arzneimittel wie Amiodaron gegen Herzrhythmusstörungen oder Betablocker können die Wirkungsdauer von Tirosint verlängern.
  • Lithiumpräparate zur Behandlung von psychischen Erkrankungen und jodhaltige Arzneimittel hemmen die Ausschüttung von Schilddrüsenhormonen aus der Schilddrüse. Bei gleichzeitiger Einnahme mit Tirosint muss die Auswirkung auf die Behandlung mit Tirosint sorgfältig überwacht werden.

Levothyroxin kann die Wirkung folgender Arzneimittel verändern

  • Blutzuckersenkende Arzneimittel (Antidiabetika): Tirosint kann die blutzuckersenkende Wirkung dieser Arzneimittel abschwächen. Der Blutzuckerspiegel ist deshalb bei Diabetes mellitus, insbesondere zu Beginn der Tirosint Behandlung, regelmässig zu kontrollieren. Bei Änderung der Dosierung von Tirosint informieren Sie Ihren Arzt oder Ihre Ärztin, damit die Dosis des blutzuckersenkenden Arzneimittels entsprechend angepasst werden kann.
  • Arzneimittel zur Verminderung der Blutgerinnung: Wenn Sie solche Arzneimittel einnehmen, sind zu Beginn der Tirosint Behandlung regelmässige Kontrollen der Blutgerinnung erforderlich. Gegebenenfalls wird Ihr Arzt oder Ihre Ärztin eine Dosisreduktion des gerinnungshemmenden Arzneimittels verordnen, da Tirosint die Wirkung von gerinnungshemmenden Mitteln verstärkt.
  • Bei gleichzeitiger Einnahme von Arzneimitteln, welche die Leistungsfähigkeit des Herzmuskels verbessern (Herzglykoside), kann Tirosint deren Wirkung verstärken. Deshalb ist bei derartigen Kombinationen die Dosis des Herzmittels neu zu überprüfen. Tirosint kann den Wirkungseintritt von Arzneimitteln gegen Depressionen (trizyklische Antidepressiva) beschleunigen und den Effekt von Kreislauf-anregenden Mitteln verstärken.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an

  • anderen Krankheiten leiden,
  • Allergien haben oder
  • andere Arzneimittel (auch selbstgekaufte) einnehmen!
+-Darf Tirosint während einer Schwangerschaft oder in der Stillzeit angewendet werden?

Während der Schwangerschaft und Stillzeit sollte die Einnahme von Tirosint nicht unterbrochen werden. Jedoch wird Ihr Arzt oder Ihre Ärztin entscheiden, ob und in welcher Dosierung Sie während einer Schwangerschaft oder während einer Stillzeit Tirosint einnehmen dürfen.

+-Wie verwenden Sie Tirosint?

Die erforderliche tägliche Dosis ist individuell unterschiedlich und wird durch den Arzt oder die Ärztin anhand labordiagnostischer und klinischer Untersuchungen festgelegt. Um diese individuelle Behandlung optimal zu ermöglichen, stehen Kapseln in verschiedenen Dosierungen mit abgestuftem Gehalt von 13 bis 200 µg des Wirkstoffs zur Verfügung.

Die Anfangsdosis für Erwachsene beträgt in der Regel 25?50 µg täglich und wird jeweils in 2?4-wöchigen Abständen um 13?50 µg /Tag bis auf 100?200 µg /Tag bis zur Normalisierung des Stoffwechsels erhöht.

Die Funktion Ihrer Schilddrüse soll regelmässig und bei der Umstellung von einem anderen Levothyroxinpräparat durch Ihren Arzt bzw. Ihre Ärztin kontrolliert werden.

Bei älteren Patienten, Patienten mit Herzkrankheiten, schwerer Schilddrüsenunterfunktion oder anderen Stoffwechselstörungen ist die Anfangsdosis niedriger.

Bei Kindern wird die Dosis entsprechend dem Körpergewicht festgelegt. Je nach Erkrankung kann die Dosis auch höher sein.

Für Säuglinge unter 1 Jahr ist Tirosint nicht geeignet.

Halten Sie sich strikte an die vom Arzt bzw. von der Ärztin verordnete Dosis.

Eine erfolgreiche Behandlung setzt die regelmässige Einnahme des Präparates und eine optimale, auf die individuellen Bedürfnisse des Patienten abgestimmte Tagesdosis voraus. Bei weiter bestehenden Krankheitszeichen trotz mehrwöchiger Therapie sollte der Arzt oder die Ärztin aufgesucht werden, der/die gegebenenfalls über eine Erhöhung der Dosis entscheiden wird.

Bei Unterbruch oder vorzeitiger Beendigung der Behandlung kann es zum Wiederauftreten von Krankheitssymptomen kommen, deren Art von der jeweiligen Grunderkrankung abhängig ist.

Haben Sie versehentlich an einem Tag die Einnahme von Tirosint vergessen, so nehmen Sie am folgenden Tag nur Ihre normale Dosis ein.

Korrekte Art der Einnahme

Die gesamte Tagesdosis wird morgens nüchtern, mindestens ½ Stunde vor dem Frühstück unzerkaut mit etwas Flüssigkeit eingenommen.

Behandlungsdauer

Die Behandlung mit Tirosint erfolgt bei Schilddrüsenunterfunktion und Schilddrüsenoperation wegen eines bösartigen Tumors der Schilddrüse zeitlebens, bei gutartigem Kropf und zur Verhütung eines erneuten Kropfwachstums einige Monate oder Jahre bis zeitlebens.

Bei Begleittherapie zur Behandlung der Schilddrüsenüberfunktion erfolgt die Behandlung mit Tirosint entsprechend der Dauer der Behandlung mit einem hemmenden Arzneimittel.

Schilddrüsenfunktionstest: Einnahme über 14?28 Tage bis zur Durchführung des Szintigramms.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Tirosint haben?

Folgende Symptome können gelegentlich als Folge der stoffwechselsteigernden Wirkung von Tirosint, vor allem bei zu hoher Dosierung oder bei zu schneller Steigerung der Dosis zu Beginn der Behandlung auftreten:

Unruhe, übermässiger, rascher Herzschlag (Tachykardie), Herzrythmusstörungen, Schmerzen im Brustkorb (Angina pectoris), vermehrtes Schwitzen, Durchfall, Gewichtsabnahme, Zittern, Kopfschmerzen, Schlaflosigkeit.

In diesen Fällen sollten Sie Rücksprache mit dem Arzt bzw. der Ärztin nehmen. Unter Umständen wird Ihre Therapie geändert.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Tirosint sollte in der Originalverpackung, bei Raumtemperatur (15?25 °C) und vor Licht geschützt gelagert werden.

Arzneimittel ausser Reichweite von Kindern aufbewahren.

Das Arzneimittel darf nur bis zu dem auf dem Behälter mit «Exp» bezeichneten Datum verwendet werden.
