Transtec 35 µg/h, 52.5 µg/h oder 70 µg/h ist ein wirkstoffhaltiges Matrix-Pflaster, welches seine Wirkung durch die Haut entfaltet (transdermal). Nach dem Aufkleben gelangt der Wirkstoff Buprenorphin über die Haut in das Blut. Transtec gehört zur Gruppe der starken Schmerzmittel (Opioide), die über das Zentralnervensystem (bestimmte Nervenzellen im Rückenmark und im Gehirn) wirken. Transtec 35 µg/h, 52.5 µg/h oder 70 µg/h wird bei ungenügender Wirksamkeit von schwachen Schmerzmitteln bei mittelstarken bis starken langanhaltenden Schmerzen eingesetzt.
Transtec Pflaster dürfen nur auf spezielle Verschreibung des Arztes oder der Ärztin angewendet werden.
Beachten Sie, dass die Möglichkeit einer Abhängigkeit besteht, wenn starke Schmerzmittel wie Transtec über einen längeren Zeitraum angewendet werden.
Transtec wurde Ihnen für Ihre momentane Erkrankung verschrieben. Benutzen Sie es nicht für andere Schmerzzustände.
?Wenn Sie überempfindlich (allergisch) gegenüber dem Wirkstoff Buprenorphin oder einem der sonstigen Bestandteile von Transtec sind.
?Wenn Sie wissen oder vermuten, dass Sie von anderen starken Schmerzmitteln (Opioide) abhängig sind.
?Wenn Ihre Lungenfunktion oder Ihre Atmung stark beeinträchtigt ist.
?Wenn Sie gleichzeitig MAO-Hemmstoffe (bestimmte Arzneimittel zur Behandlung von Depressionen) einnehmen oder diese während der letzten 14 Tage vor Beginn der Behandlung mit Transtec angewendet haben.
?Transtec darf bei akutem Alkohol-Entzugssyndrom (Delirium tremens) nicht angewendet werden.
?Personen unter 18 Jahren kann Transtec nicht empfohlen werden, da für diese Altersgruppe noch keine Erfahrungen vorliegen.
?Transtec darf nicht als Ersatzmittel beim Drogenentzug eingesetzt werden.
?Während der Schwangerschaft.
?Wenn Sie an Myasthenia gravis (krankhafte Muskelschwäche) leiden.
Transtec kann Schwindel, Schläfrigkeit, verschwommenes oder Doppelsehen verursachen. Deshalb kann dieses Arzneimittel die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Dies trifft besonders zu:
?am Anfang der Behandlung
?bei einer Änderung der Dosierung
?wenn Sie von einem anderen Schmerzmittel auf Transtec umstellen
?wenn Sie Alkohol konsumieren
?wenn Sie gleichzeitig andere Arzneimittel nehmen, die dämpfend auf das Zentralnervensystem wirken können (z.B. Beruhigungs- oder Schlafmittel, Antidepressiva).
Falls Sie die oben erwähnten Symptome verspüren, sollten Sie nicht Autofahren oder Maschinen bedienen. Dies trifft auch am Ende der Behandlung mit Transtec zu: Verzichten Sie auf das Autofahren oder Bedienen von Maschinen während mindestens 24 Stunden nach der Entfernung des Pflasters.
Des Weiteren teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn eine oder mehrere der folgenden Zustände auf Sie zutreffen oder in der Vergangenheit auf Sie zutrafen:
?wenn Sie an erhöhtem Hirndruck (z.B. nach Kopfverletzungen oder einer Erkrankung des Gehirns) leiden
?wenn Sie zu Krampfanfällen im Gehirn (z.B. Epilepsie) neigen
?wenn Sie Bewusstseinsstörungen oder Anzeichen eines Schocks (z.B. kalter Schweiss) haben
?wenn Sie Probleme mit der Atmung haben
?wenn Sie an einer Funktionsstörung der Leber leiden
?wenn Sie gleichzeitig andere Arzneimittel einnehmen, die die Atmung beeinträchtigen oder dämpfend auf das Zentralnervensystem wirken können, wie z.B. Beruhigungs- oder Schlafmittel, Antidepressiva oder Alkohol.
Informieren Sie Ihren Arzt bzw. Ihre Ärztin oder Apotheker bzw. ihre Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
Es liegen bisher keine ausreichenden Erfahrungen über die Anwendung von Transtec an Schwangeren vor. Systematische wissenschaftliche Untersuchungen wurden nie durchgeführt. (Vgl. Kapitel "Wann darf Transtec nicht angewendet werden").
Buprenorphin hemmt die Milchbildung und geht in die Muttermilch über. Daher sollte ein Einsatz von Transtec während der Stillzeit unterbleiben.
Falls von Ihrem Arzt bzw. Ihrer Ärztin nicht anders verordnet, kleben Sie ein Transtec Pflaster zu 35 µg/h oder 52.5 µg/h oder 70 µg/h auf und wechseln Sie es regelmässig, spätestens aber nach 96 Stunden. So können Sie das Pflaster zweimal in der Woche zu zwei festen Zeitpunkten, z.B. jeden Montag morgen und jeden Donnerstag abend, wechseln. Notieren Sie sich bitte den Zeitpunkt des Pflasterwechsels, z.B. auf der Faltschachtel. Hat Ihnen Ihr Arzt bzw. Ihre Ärztin zusätzlich zum Pflaster weitere Schmerzmittel verordnet, befolgen Sie seine Anweisungen genau. Andernfalls kann es vorkommen, dass Sie von der Behandlung nicht optimal profitieren.
Bevor Sie ein Pflaster aufkleben:
?Wählen Sie an Ihrem Oberkörper eine flache, unbehaarte Hautstelle aus. (Empfohlene Bereiche können Sie den neben-stehenden Abbildungen entnehmen.) |

?Lassen Sie sich von jemandem helfen, falls Sie das Pflaster nicht selber anbringen können.
?Haare auf der ausgewählten Hautstelle sollten mit einer Schere abgeschnitten werden. Nicht rasieren!
?Vermeiden Sie Hautstellen, die gerötet oder gereizt sind oder andere Schäden (z.B. grössere Narben) aufweisen.
?Die ausgewählte Hautstelle muss trocken und sauber sein. Falls erforderlich, reinigen Sie sie mit kaltem oder lauwarmem Wasser. Benutzen Sie dabei keine Seife. Haben Sie heiss gebadet oder geduscht, warten Sie, bis Ihre Haut völlig trocken und abgekühlt ist. Tragen Sie auf die ausgewählte Hautstelle keine Körperlotion auf. Dadurch könnte das Pflaster nicht richtig kleben.
Wie Sie ein Pflaster aufkleben:
| Schritt 1 Jedes Pflaster befindet sich in einem eigenen Beutel. Öffnen Sie diesen Beutel erst unmittelbar vor der Anwendung, indem Sie ihn an einem Einschnitt aufreissen. Entnehmen Sie das Pflaster. |
| Schritt 2 Die Klebeseite des Pflasters ist mit einer silberfarbenen Schutzfolie versehen. Entfernen Sie vorsichtig die Hälfte der Folie, wobei Sie die Klebeschicht möglichst nicht berühren sollten. |
| Schritt 3 Kleben Sie das Pflaster auf die ausgewählte Hautstelle und entfernen Sie den Rest der Schutzfolie. |
| Schritt 4 Pressen Sie das Pflaster mit der flachen Hand auf Ihre Haut und zählen Sie langsam bis 30. Vergewissern Sie sich, dass das gesamte Pflaster auf der Haut klebt, auch an den Rändern. |
Wenn Sie das Pflaster tragen
Nun können Sie das Pflaster bis zu vier Tage lang auf der Haut belassen. Vorausgesetzt, Sie haben das Pflaster richtig angebracht, ist ein Ablösen sehr unwahrscheinlich. Sie können mit dem Pflaster duschen, baden oder schwimmen. Das Pflaster sollte jedoch heissem Wasser oder direkter Hitzeeinwirkung nicht ausgesetzt werden. Verzichten Sie deshalb unbedingt während der Behandlung mit Transtec auf Wärmewickel, elektrische Heizdecken, heizbare Wasserbetten, Hitzelampen, Wärmeflaschen, Sauna, heisse Whirlpool-Bäder usw. oder Solarium. Vermeiden Sie intensive Sonnenbestrahlung und sprechen Sie mit Ihrem Arzt oder Ärztin, falls Sie Fieber haben sollten.
Im unwahrscheinlichen Fall, dass sich das Pflaster vor dem nächsten Wechsel löst, benutzen Sie nicht dasselbe Pflaster nochmals. Kleben Sie umgehend ein neues Pflaster auf (siehe auch Wie Sie das Pflaster wechseln).
Wie Sie das Pflaster wechseln
?Nehmen Sie das alte Pflaster ab.
?Falten Sie es in der Mitte zusammen, wobei die Klebefläche innen liegen sollte.
?Werfen Sie es sorgsam weg. Es darf weder in Sicht- noch Reichweite von Kindern gelangen.
?Kleben Sie ein neues Pflaster auf eine andere gesunde und unbehaarte Hautstelle (wie oben beschrieben). Erst nach 8 Tagen (nach zwei Anwendungen) dürfen Sie die alte Hautstelle wieder bekleben.
Ihr Arzt bzw. Ihre Ärztin wird Ihnen sagen, wie lange Sie Transtec anwenden sollen. Brechen Sie nicht von sich aus die Behandlung ab, denn dann besteht die Möglichkeit, dass Ihre Schmerzen wiederkehren und Sie sich unwohl fühlen (siehe auch Auswirkungen, wenn die Behandlung mit Transtec beendet oder abgebrochen wird).
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Wenn Sie eine grössere Menge von Pflastern aufgeklebt haben als Sie sollten
entfernen Sie die überzählige Menge und sprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin.
Wenn Sie das Wechseln des Pflasters vergessen haben?
kleben Sie ein neues Pflaster auf, sobald Sie dies bemerken. Notieren Sie sich den Tag, an dem Sie das neue Pflaster aufkleben, damit Sie den Wechselrhythmus entsprechend anpassen können.
Wenn Sie den Zeitpunkt des Wechsels sehr weit überschritten haben, können Ihre Schmerzen erneut auftreten. Wenden Sie sich in diesem Fall an Ihren Arzt bzw. an Ihre Ärztin.
Sie dürfen auf keinen Fall die vergessene Dosis durch das Aufkleben der doppelten Anzahl an Pflastern nachholen.
Auswirkungen, wenn die Behandlung mit Transtec beendet oder abgebrochen wird
Wenn starke Schmerzmittel über einen längeren Zeitraum angewendet werden und die Behandlung beendet oder plötzlich abgebrochen wird, kann es zu Nachwirkungen kommen. Diese sind nach der Anwendung von Transtec jedoch sehr unwahrscheinlich. Wenn Sie sich dennoch erregt, ängstlich, nervös oder zittrig fühlen, überaktiv sind, nicht schlafen können, oder Verdauungsbeschwerden bekommen, sprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin.
Folgende Nebenwirkungen können bei der Anwendung von Transtec auftreten:
Sehr häufig ist Übelkeit. Sehr häufig kam es auch an der Anwendungsstelle zu Rötung (Erythem) und Juckreiz.
Häufig wurden unter Transtec Schwindel, Kopfschmerzen, Müdigkeit, Erbrechen, Verstopfung, Schwitzen, Wasseransammlung in den Beinen (Ödeme) und Atemnot beobachtet. Bei wiederholter Pflasteranwendung über Monate oder Jahre wurden häufig Hautveränderungen (Exantheme) beobachtet.
Gelegentlich wurde berichtet über: Abgeschlagenheit, Beruhigung in unterschiedlichem Ausmass (von leichter Müdigkeit bis zur Benommenheit), Verwirrtheitszustände, Schlafstörungen, Unruhe, Ausschlag, Mundtrockenheit, und Schwierigkeiten beim Wasserlassen und Harnverhaltung (weniger Urin als normal).
Vor allem zu Beginn der Therapie kann es gelegentlich zu hypotonen Kreislaufreaktionen (niedrigem Blutdruck), in seltenen Fällen bis zum Kollaps kommen.
Selten wurden die folgenden Nebenwirkungen beobachtet: Appetitverlust, Sodbrennen, Sinnes- und Wahrnehmungstäuschungen (z.B. Halluzinationen, Angstzustände und Albträume), Konzentrationsstörungen, Sprechstörungen, Gleichgewichtsstörungen, vermindertes sexuelles Interesse und Erektionsschwäche. Ebenfalls selten traten auf: Taubheitsgefühl, Hautprickeln oder brennende Hautirritationen, Sehstörungen, verschwommenes Sehen, Lidschwellung, anfallartiges Hitzegefühl, eingeschränkte Atemtätigkeit (Atemdepression) und Entzugserscheinungen. Selten traten an der Anwendungsstelle als Spätfolgen Überempfindlichkeitsreaktionen mit Anzeichen einer Entzündung auf. Sollten Sie davon betroffen sein, sprechen Sie mit Ihrem Arzt bzw. Ihrer Ärztin.
Sehr selten waren Stimmungsschwankungen, Abhängigkeit, Muskelzuckungen, Geschmacksstörungen, Pupillenverengung, Ohrschmerz, rasches Atmen (Hyperventilation), Schluckauf, Brechreiz, Brustschmerz, Pusteln oder Hautbläschen.
Sehr selten kann eine schwerwiegende allergische Reaktion auf Buprenorphin auftreten z.B. Fälle von Quincke-Ödem (schmerzhafte Schwellung von Haut und Schleimhaut) bis hin zum anaphylaktischen Schock. Wenn eine der folgenden Erscheinungen bei Ihnen auftritt, muss das Pflaster entfernt werden, sofort ein Arzt bzw. eine Ärztin gerufen werden oder eine Einweisung in das nächstgelegene Spital erfolgen.
?Schwellungen an Händen, Füssen, Knöcheln, Gesicht, Lippen, Mund oder Kehle mit Schluck- oder Atembeschwerden
?Hautausschlag
?Ohnmacht
?Gelbfärbung der Haut und Augen (Gelbsucht).
Fälle von Bronchospasmus (Krampf der Bronchialmuskeln) wurden unter Buprenorphin-Behandlung ebenfalls beobachtet.
Falls Sie Nebenwirkungen bei sich feststellen, die nicht von alleine wieder abklingen oder die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Während der Behandlung mit Transtec sollten Sie keinen Alkohol zu sich nehmen. Alkohol kann die Nebenwirkungen des Pflasters verstärken und Ihnen Unwohlsein bereiten.
Steuern Sie kein Fahrzeug und bedienen Sie keine Werkzeuge oder Maschinen, wenn Sie auf Transtec mit Schwindel, Benommenheit oder Sehstörungen reagieren. Dies kann am Anfang der Behandlung, bei einer Änderung der Transtec Dosierung, bei einem Wechsel von einem anderen Schmerzmittel oder wenn Sie andere Medikamente nehmen, die auf das zentrale Nervensystem wirken, passieren. Das gleiche gilt, wenn während der Behandlung mit Transtec Sinnes- und Wahrnehmungstäuschungen (Halluzinationen) oder Verwirrtheitszustände bei Ihnen aufgetreten sind. Dies sollten Sie auch bis zu 24 Stunden nach Entfernen des Transtec Pflasters beachten.
Bewahren Sie Transtec ausserhalb der Reichweite von Kindern auf.
Das Arzneimittel darf nur bis zu dem auf dem Behälter mit "EXP" bezeichneten Datum verwendet werden und ist bei Raumtemperatur (15-25°C) zu lagern.
Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, diese Personen verfügen über eine ausführliche Fachinformation.
1 Pflaster Transtec zu 35 µg/h bzw. 52.5 µg/h bzw. 70 µg/h enthält:
Transtec 35 µg/h hat eine wirkstoffhaltige Fläche von 25 cm2 und enthält 20 mg Buprenorphin. Innerhalb von 96 Stunden werden 35 µg/h freigesetzt.
Transtec 52.5 µg/h hat eine wirkstoffhaltige Fläche von 37.5 cm2 und enthält 30 mg Buprenorphin. Innerhalb von 96 Stunden werden 52.5 µg/h freigesetzt.
Transtec 70 µg/h hat eine wirkstoffhaltige Fläche von 50 cm2 und enthält 40 mg Buprenorphin. Innerhalb von 96 Stunden werden 70 µg/h freigesetzt.

Transtec erhalten Sie in Apotheken nur gegen ärztliche Verschreibung, die nur zum einmaligen Bezug berechtigt.
Transtec untersteht dem Bundesgesetz über die Betäubungsmittel und die psychotropen Stoffe.
Es sind folgende Packungsgrössen erhältlich:
Transtec 35 µg/h: 4 und 8 Pflaster.
Transtec 52.5 µg/h: 4 und 8 Pflaster.
Transtec 70 µg/h: 4 und 8 Pflaster.
