Trobalt gehört zu einer Gruppe von Arzneimitteln, die Antiepileptika genannt werden. Trobalt erzielt seine Wirkung, indem es die Überaktivität des Gehirns, die zu epileptischen Anfällen führt, verhindert. Es wird auf Verschreibung des Arztes oder der Ärztin zusammen mit anderen Arzneimitteln verwendet zur Behandlung Erwachsener mit einer Art von Epilepsie, die einen Teil des Gehirns betrifft und partielle Anfälle genannt wird. Diesen Anfällen können Anfälle folgen, die das gesamte Gehirn betreffen (sekundäre Generalisierung).
Trobalt wird nur verschrieben, wenn andere Arzneimittel ungenügend wirksam oder unverträglich sind.
Bei bekannter Überempfindlichkeit gegenüber dem Wirkstoff Retigabin oder einem der sonstigen Bestandteile des Präparates darf Trobalt nicht eingenommen werden.
Falls Sie Nieren- oder Leberprobleme haben, muss Ihr Arzt oder Ihre Ärztin vor Einnahme von Trobalt informiert werden. Ihr Arzt oder Ihre Ärztin entscheidet sich möglicherweise dafür, Ihnen eine geringere Dosis zu verschreiben.
Bei älteren Patienten (ab 65 Jahren) wird ein Neubeginn einer Behandlung mit Trobalt nicht empfohlen, da zur Verträglichkeit und Wirksamkeit in dieser Altersgruppe nur begrenzte Daten vorliegen. Bei Patienten über 65 Jahren, die bereits vorher mit Trobalt behandelt wurden und dies gut vertragen haben, entscheidet sich der Arzt oder die Ärztin möglicherweise dafür, die Dosis zu senken.
Trobalt wird für Kinder und Jugendliche unter 18 Jahren nicht empfohlen. Die Verträglichkeit und Wirksamkeit in dieser Altersgruppe sind noch nicht bekannt.
Trobalt kann schwere Nebenwirkungen hervorrufen, beispielsweise die Unfähigkeit, Urin auszuscheiden (Harnverhalt) und psychische Störungen. Während Sie Trobalt einnehmen, müssen Sie auf bestimmte Symptome achten, um die Gefahr möglicher Nebenwirkungen zu verringern. Verfärbung von Teilen des Auges einschliesslich der Netzhaut, der Nägel, der Lippen und/oder der Haut wurden ebenfalls berichtet (siehe auch «Welche Nebenwirkungen kann Trobalt haben»).
Trobalt kann den Herzrhythmus beeinflussen. Die Wahrscheinlichkeit, dass Sie davon betroffen sind, ist höher, wenn Sie andere Arzneimittel einnehmen, wenn Sie bereits bestehende Herzprobleme haben, wenn der Kalium- oder Magnesiumspiegel Ihres Blutes niedrig ist (Hypokaliämie bzw. Hypomagnesiämie) oder wenn Sie 65 Jahre oder älter sind. Bitte informieren Sie Ihren Arzt oder Ihre Ärztin, wenn etwas davon auf Sie zutrifft oder wenn Sie ungewöhnliche Veränderungen Ihres Herzschlags feststellen (beispielsweise zu schneller oder zu langsamer Herzschlag). Es kann sein, dass Sie zusätzliche Untersuchungen (einschliesslich eines Elektrokardiogramms (EKG), ein Test, der die elektrische Leitfähigkeit der Aktivität Ihres Herzens aufzeichnet) benötigen, während Sie Trobalt einnehmen.
Eine geringe Anzahl von Patienten, die mit Antiepileptika wie Trobalt behandelt wurden, hatten Gedanken daran, sich selbst zu verletzen oder sich das Leben zu nehmen. Wenn Sie zu irgendeinem Zeitpunkt solche Gedanken haben, setzen Sie sich sofort mit Ihrem Arzt oder Ihrer Ärztin in Verbindung.
Trobalt kann die Ergebnisse einiger Labor-Tests beeinflussen. Wenn Sie einen Blut- oder Urintest benötigen, informieren Sie die Person, die den Test anordnet, darüber, dass Sie Trobalt einnehmen.
Trobalt kann den Gehalt an Digoxin (Arzneimittel zur Behandlung von Herzproblemen) im Blut erhöhen. Informieren Sie Ihren Arzt oder Ihre Ärztin, falls Sie Digoxin einnehmen.
Trobalt kann bestimmte Narkosemittel (z.B. Thiopental) beeinflussen. Wenn bei Ihnen eine Operation in Vollnarkose geplant ist, setzen Sie den Arzt oder die Ärztin frühzeitig darüber in Kenntnis, dass Sie Trobalt einnehmen.
Alkoholkonsum während der Einnahme von Trobalt kann zu Sehstörungen (verschwommenem Sehen) führen. Daher ist besondere Vorsicht geboten, bis Sie wissen, wie sich Trobalt und Alkohol bei Ihnen auswirken.
Trobalt kann zu Schwindel oder Schläfrigkeit führen und Sehstörungen (Doppel- oder Verschwommensehen) hervorrufen. Fahren Sie nicht und bedienen Sie keine Werkzeuge oder Maschinen, bis Sie wissen, wie sich Trobalt bei Ihnen auswirkt. Sprechen Sie mit Ihrem Arzt oder Ihrer Ärztin über die Auswirkungen Ihrer Epilepsie auf die Fahrtüchtigkeit und das Bedienen von Maschinen.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte) einnehmen oder äusserlich anwenden!
Es sind keine Informationen zur Sicherheit von Trobalt bei schwangeren Frauen verfügbar. Trobalt darf während der Schwangerschaft nicht eingenommen werden, ausser wenn dies vom Arzt oder von der Ärztin ausdrücklich verschrieben wurde. Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie schwanger sind bzw. glauben, schwanger zu sein, oder wenn Sie eine Schwangerschaft planen. Brechen Sie Ihre Behandlung nicht ohne Rücksprache mit Ihrem Arzt oder Ihrer Ärztin ab.
Es ist nicht bekannt, ob die Bestandteile von Retigabin in die Muttermilch übergehen können. Daher ist das Stillen während einer Therapie mit Trobalt nicht zu empfehlen.
Falls vom Arzt oder von der Ärztin nicht anders verschrieben, ist die übliche Anfangsdosis von Trobalt 100 mg dreimal täglich (insgesamt 300 mg pro Tag). Ihr Arzt oder Ihre Ärztin passt Ihre Dosis unter Umständen über mehrere Wochen hinweg allmählich an, damit Ihre Anfälle besser kontrollierbar werden und die Nebenwirkungen auf ein Minimum beschränkt bleiben. Die Höchstdosis beträgt 400 mg dreimal täglich (insgesamt 1200 mg pro Tag). Wenn Sie Nieren- oder Leberprobleme haben, gibt Ihr Arzt oder Ihre Ärztin Ihnen möglicherweise eine niedrigere Dosis Retigabin. Wenn Sie älter als 65 Jahre sind und vorher bereits mit Trobalt behandelt wurden und dies gut vertragen haben, wird Ihr Arzt oder Ihre Ärztin möglicherweise die Dosis auf maximal 900 mg pro Tag begrenzen. Es kann einige Zeit dauern, bis die richtige Dosis von Trobalt für Sie gefunden ist.
Ändern Sie nicht von sich aus die verschriebene Dosierung!
Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Nehmen Sie die Tabletten im Ganzen ein. Die Tabletten dürfen nicht zerkaut, zerdrückt oder geteilt werden. Sie können Trobalt zu den Mahlzeiten oder unabhängig von den Mahlzeiten einnehmen.
Wenn Sie zu viele Tabletten von Trobalt eingenommen haben, ist die Wahrscheinlichkeit höher, dass bei Ihnen Nebenwirkungen oder eines der folgenden Symptome auftreten: Gefühl von Unruhe, Aggressivität, Erregbarkeit oder Änderungen des Herzrhythmus. Bitte fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin um Rat.
Wenn Sie eine oder mehrere Dosen ausgelassen haben, nehmen Sie einfach eine Dosis ein, sobald Sie sich daran erinnern. Die nächste Dosis darf frühestens drei Stunden später eingenommen werden. Nehmen Sie nicht mehr als eine Dosis gleichzeitig ein, wenn Sie eine oder mehrere Dosen vergessen haben. Wenn Sie nicht sicher sind, wie Sie vorgehen sollen, fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin.
Nehmen Sie Trobalt so lange ein, wie von Ihrem Arzt oder Ihrer Ärztin empfohlen. Beenden Sie die Behandlung erst auf Anraten Ihres Arztes oder Ihrer Ärztin. Wenn Sie die Einnahme von Trobalt plötzlich abbrechen, können Ihre Krampfanfälle erneut auftreten oder sich verschlimmern. Um die Behandlung mit Trobalt zu beenden, muss die Dosis allmählich, d.h. über einen Zeitraum von mindestens drei Wochen, verringert werden.
Die Einnahme von Trobalt kann unerwünschte Wirkungen verursachen.
Verfärbung von Teilen des Auges einschliesslich der Retina (Netzhaut oder innere Augenhaut): Diese unerwünschte Wirkung tritt besonders nach längerer Einnahme von Trobalt häufig auf. Diese Verfärbung kann manchmal auch zusammen mit Verfärbungen der Nägel, der Lippen oder der Haut beobachtet werden. Ihr Arzt oder Ihre Ärztin wird Ihnen daher vor Therapiebeginn mit Trobalt eine Augenuntersuchung empfehlen. Diese Augenuntersuchung soll während der Behandlung mit Trobalt mindestens alle sechs Monate wiederholt werden. Sollten sich bei diesen Untersuchungen Auffälligkeiten ergeben, wird Ihr Arzt oder Ihre Ärztin entscheiden, ob die Behandlung mit Trobalt abgebrochen werden muss.
Probleme beim Wasserlassen: Diese treten bei Menschen, die Trobalt einnehmen, häufig auf und können dazu führen, dass kein Wasserlassen mehr möglich ist (Harnverhalt). Diese Probleme treten am wahrscheinlichsten innerhalb der ersten Monate der Behandlung mit Trobalt auf. Mögliche Symptome sind: Schmerzen beim Wasserlassen (Dysurie) oder Schwierigkeiten, mit dem Wasserlassen zu beginnen (verzögertes Wasserlassen) oder Unfähigkeit Urin auszuscheiden (Harnverhalt). Wenden Sie sich umgehend an Ihren Arzt oder Ihre Ärztin, wenn Sie eines dieser Symptome bei sich bemerken.
Psychische Störungen: Diese treten bei Menschen, die Trobalt einnehmen, häufig und mit grösster Wahrscheinlichkeit während der ersten Monate der Behandlung auf. Mögliche Symptome sind: Verwirrtheit, psychotische Erkrankungen (schwere psychische Störungen) oder Halluzinationen (Sehen oder Hören nicht vorhandener Dinge). Wenden Sie sich so schnell wie möglich an Ihren Arzt oder Ihre Ärztin, wenn Sie eines dieser Symptome bei sich bemerken. Ihr Arzt oder Ihre Ärztin entscheidet möglicherweise, dass Trobalt nicht für Sie geeignet ist.

Diese können bei bis zu 1 von 10 Personen auftreten: Schmerzen beim Wasserlassen (Dysurie), Schwierigkeiten, mit dem Wasserlassen zu beginnen (verzögertes Wasserlassen), Blut im Urin, ungewöhnlich verfärbter Urin, Verwirrtheit, psychotische Erkrankungen (schwere psychische Störungen), Halluzinationen (Sehen oder Hören von nicht vorhandenen Dingen), Desorientiertheit, Angstgefühle, Gedächtnisprobleme (Amnesie), Lese- oder Schreibschwierigkeiten, Probleme, sich richtig auszudrücken oder Schwierigkeiten mit dem Wortverständnis, Aufmerksamkeitsprobleme, mangelnde Koordination, Schwindelgefühl (Vertigo), Gleichgewichtsprobleme, Gangstörungen, Zittern, plötzliches Muskelzucken (Myoklonus), Kribbeln oder Taubheitsgefühl der Hände oder Füsse, Doppel- oder Verschwommensehen, Verstopfung, Übelkeit, Verdauungsstörungen, Mundtrockenheit, Gewichtszunahme, vermehrter Appetit, Schwellungen an Unterschenkeln und Füssen, Schwächegefühl oder allgemeines Unwohlsein, in Bluttests nachweisbare Veränderungen der Leberfunktion, blaugraue Verfärbungen der Nägel, der Lippen und/oder der Haut, Verfärbung von Teilen des Auges einschliesslich der Retina (Netzhaut oder innere Augenhaut).
Diese können bei bis zu 1 von 100 Personen auftreten: Unfähigkeit, Urin auszuscheiden (Harnverhalt), langsame oder reduzierte Muskelbewegungen, Schluckstörungen, Hautausschlag, übermässiges Schwitzen.
Wenn eine der aufgeführten Nebenwirkungen Sie beunruhigt oder einen schweren Verlauf nimmt oder wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Trobalt Tabletten sollten nicht über 25 °C, in der Originalpackung und ausserhalb der Reichweite von Kindern aufbewahrt werden.
Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.
Übrig gebliebene Trobalt Filmtabletten sollen nicht im Abwasser oder Haushaltsabfall entsorgt werden. Fragen Sie Ihren Apotheker oder Ihre Apothekerin, wie das Arzneimittel zu entsorgen ist, wenn Sie es nicht mehr benötigen. Diese Massnahme hilft die Umwelt zu schützen.
