+-Was ist Vascord und wann wird es angewendet?

Vascord enthält zwei Wirkstoffe mit den Namen Olmesartanmedoxomil und Amlodipin (als Amlodipinbesilat vorliegend). Beide Substanzen dienen zur Kontrolle von zu hohem Blutdruck.

  • Olmesartanmedoxomil gehört zu einer Gruppe von Wirkstoffen, die «Angiotensin-II-Rezeptor-Antagonisten» genannt werden und den Blutdruck senken, indem sie die Blutgefässe erweitern.
  • Amlodipin gehört zu einer Gruppe von Wirkstoffen, die «Kalziumkanalblocker» genannt werden. Amlodipin verhindert, dass Kalzium in die Wand der Blutgefässe eindringt und verhindert dadurch ein Engerwerden der Gefässe, was ebenfalls den Blutdruck senkt.

Die Wirkweise beider Substanzen trägt dazu bei, eine Straffung der Blutgefässe zu hemmen, so dass die Blutgefässe sich entspannen und der Blutdruck sinkt.

Vascord wird angewendet bei der Behandlung von hohem Blutdruck (auch bekannt unter dem Namen «Hypertonie») bei Patienten, deren Blutdruck nicht ausreichend alleine durch Olmesartanmedoxomil oder Amlodipin kontrolliert werden kann. Ein hoher Blutdruck kann die Blutgefässe in den Organen schädigen, so beispielsweise im Herzen, den Nieren, Gehirn und den Augen. In manchen Fällen kann dies zu einem Herzinfarkt, zu Herz- oder Nierenfunktionsstörungen, zu einem Schlaganfall oder Blindheit führen. Bis zu diesem Zeitpunkt verursacht ein hoher Blutdruck normalerweise keine Beschwerden. Daher ist es wichtig, den Blutdruck zu kontrollieren, um diese Schäden zu verhindern.

Ein hoher Blutdruck kann durch eine Behandlung mit Medikamenten wie Vascord kontrolliert werden. Ihr Arzt bzw. Ihre Ärztin hat Ihnen möglicherweise auch einige Änderungen in der Lebensweise empfohlen, die Ihnen helfen, den Blutdruck zu senken (z.B. Gewichtsreduzierung, ein Rauchstopp, eine Reduktion der konsumierten Alkoholmenge und eine Reduktion des verwendeten Salzes in der Nahrung). Ihr Arzt bzw. Ihre Ärztin hat Sie möglicherweise auch angewiesen, regelmässig Sport zu betreiben, wie beispielsweise Wandern oder Schwimmen. Es ist sehr wichtig, den Ratschlägen Ihres Arztes bzw. Ihrer Ärztin zu folgen.

+-Wann darf Vascord nicht angewendet werden?
  • Wenn Sie allergisch (überempfindlich) sind auf Amlodipin oder andere Medikamente des Dihydropyridin-Typs,
  • Wenn Sie allergisch sind auf Olmesartanmedoxomil oder Amlodipin oder andere Bestandteile von Vascord. Wenn Sie glauben, dass Sie allergisch sind, wenden Sie sich vor einer Einnahme von Vascord an Ihren Arzt bzw. Ihre Ärztin,
  • Wenn Sie Diabetes mellitus oder eine eingeschränkte Nierenfunktion haben und mit einem blutdrucksenkenden Arzneimittel, das Aliskiren enthält, behandelt werden,
  • Wenn Sie ab dem vierten Schwangerschaftsmonat schwanger sind (siehe «Darf Vascord während einer Schwangerschaft oder in der Stillzeit eingenommen werden»),
  • Wenn Sie schwere Leberprobleme haben, wenn der Abfluss der Gallenflüssigkeit aus der Leber vermindert oder der Abfluss der Gallenflüssigkeit aus der Gallenblase verhindert ist (z.B. durch Gallensteine), oder wenn Sie eine Gelbsucht haben (Gelbfärbung der Haut und der Augen),
  • Wenn Sie einen sehr niedrigen Blutdruck haben,
  • Wenn Sie eine unzureichende Blutversorgung des Körpergewebes haben, die sich z.B. durch niedrigen Blutdruck, langsamen Puls oder schnellen Herzschlag (Schock, einschliesslich kardiogenem Schock) äussern kann. Als kardiogenen Schock bezeichnet man einen Schock, der durch schwere Herzprobleme verursacht wird,
  • Wenn der Blutfluss von Ihrem Herzen behindert ist (z.B. aufgrund einer Verengung der Aortenklappe [Aortenstenose]),
  • Wenn Sie eine verminderte Pumpleistung des Herzens nach einem Herzanfall (akuter Herzinfarkt) haben, die sich durch Kurzatmigkeit oder Schwellung von Füssen und Knöcheln bemerkbar macht.
+-Wann ist bei der Einnahme von Vascord Vorsicht geboten?

Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie eines der folgenden Arzneimittel zur Behandlung von hohem Blutdruck einnehmen:

  • einen ACE-Hemmer (z.B. Enalapril, Lisinopril, Ramipril), insbesondere wenn Sie Nierenprobleme aufgrund von Diabetes mellitus haben,
  • Aliskiren.

Ihr Arzt bzw. Ihre Ärztin wird gegebenenfalls Ihre Nierenfunktion, Ihren Blutdruck und die Elektrolytwerte (z.B. Kalium) in Ihrem Blut in regelmässigen Abständen überprüfen.

Wenn Sie eines der folgenden Gesundheitsprobleme haben, wenden Sie sich an Ihren Arzt bzw. Ihre Ärztin, bevor Sie diese Tabletten einnehmen:

  • Nierenprobleme oder Nierentransplantation,
  • Leberstörungen,
  • Herzerkrankungen oder Probleme mit Ihren Herzklappen oder dem Herzmuskel,
  • Schweres Erbrechen, Durchfall, Behandlung mit grossen Dosen von harntreibenden Arzneimitteln (Diuretika) oder wenn Sie eine salzarme Diät durchführen,
  • Erhöhte Kaliumkonzentrationen in Ihrem Blut,
  • Probleme mit Ihren Nebennierendrüsen (hormonproduzierende Drüsen auf der Oberseite der Nieren).

Verständigen Sie Ihren Arzt, wenn Sie starken anhaltenden Durchfall bekommen und dadurch erheblich an Gewicht verlieren.

Ihr Arzt wird Ihre Beschwerden beurteilen und entscheiden, wie Ihr Blutdruck weiter behandelt werden soll.

Wie bei allen blutdrucksenkenden Arzneimitteln kann ein starkes Absenken des Blutdruckes bei Patienten, die an Durchblutungsstörungen des Herzens oder des Gehirnes leiden zu einem Herzinfarkt oder Schlaganfall führen. Ihr Arzt bzw. Ihre Ärztin wird daher Ihren Blutdruck sorgfältig kontrollieren.

Bei Einnahme mit anderen Arzneimitteln

Bitte informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie aktuell oder vor kurzem eines der folgenden Arzneimittel eingenommen haben:

  • Andere blutdrucksenkende Medikamente, da die Wirkung von Vascord verstärkt werden kann und die Häufigkeit der Nebenwirkungen höher sein kann,
  • Kaliumzusätze, Salzersatzlösungen, die Kalium enthalten, harntreibende Arzneimittel (Diuretika) oder Heparin (zur Verdünnung des Blutes und Verhinderung von Blutgerinnseln). Die Anwendung dieser Medikamente zur gleichen Zeit wie Vascord kann die Konzentration von Kalium in Ihrem Blut ansteigen lassen,
  • Lithium (ein Arzneimittel zur Behandlung von Stimmungsschwankungen und bestimmten Arten von Depressionen) und Vascord zur gleichen Zeit angewendet kann die Toxizität von Lithium verstärken. Wenn Sie Lithium einnehmen, wird Ihr Arzt bzw. Ihre Ärztin die Lithiumkonzentrationen in Ihrem Blut messen,
  • Nichtsteroidale anti-inflammatorische Medikamente (NSAID, Arzneimittel zur Linderung von Schmerzen, Schwellungen und anderen Beschwerden bei Entzündungen, einschliesslich Arthritis) und Vascord zur gleichen Zeit angewendet, kann das Risiko eines Nierenversagens erhöhen. Die Wirkung von Vascord kann durch NSAID vermindert werden,
  • Bestimmte Säurehemmer (Arzneimittel gegen Magenverstimmungen und Sodbrennen), da die Wirkung von Vascord leicht beeinträchtigt werden kann,
  • Medikamente zur Behandlung von HIV/AIDS (z.B. Retrovir, Indinavir, Nelfinavir) oder zur Behandlung von Pilzinfektionen (z.B. Ketoconazol, Itraconazol),
  • Diltiazem, Verapamil (Arzneimittel zur Behandlung von Herzrhythmus-störungen und hohem Blutdruck),
  • Rifampicin, Erythromycin, Clarithromycin (Arzneimittel zur Behandlung von Tuberkulose oder anderen Infektionen),
  • Johanniskraut (Hypericum perforatum), ein pflanzliches Heilmittel bei Angststörungen und Depressionen,
  • Dantrolen (eine Infusion zur Behandlung schwerer Störungen der Körpertemperatur),
  • Simvastatin, ein Arzneimittel zur Senkung der Cholesterin- und Triglyzeridwerte (fettartige Substanzen) im Blut.

Bei Einnahme zusammen mit Nahrungsmitteln und Getränken

Personen, die Vascord einnehmen, sollten keine Grapefruit essen und keinen Grapefruitsaft trinken, weil hierdurch der Blutspiegel des Wirkstoffs Amlodipin erhöht werden kann, was möglicherweise zu einer unvorhersehbaren Verstärkung der blutdrucksenkenden Wirkung von Vascord führt.

Ältere Patienten

Wenn Sie älter als 65 Jahre sind, wird Ihr Arzt bzw. Ihre Ärztin Ihren Blutdruck regelmässig bei Erhöhung der Dosis kontrollieren und damit sichergehen, dass Ihr Blutdruck nicht zu niedrig wird.

Patienten schwarzer Hautfarbe

Wie bei ähnlichen Medikamenten kann die blutdrucksenkende Wirkung von Vascord bei Patienten schwarzer Hautfarbe leicht vermindert sein.

Verkehrstüchtigkeit und das Bedienen von Maschinen

Möglicherweise fühlen Sie sich während der Behandlung Ihres zu hohen Blutdrucks müde, oder Ihnen wird schwindelig. Deshalb sollte beim Führen von Fahrzeugen oder beim Bedienen von Maschinen speziell zu Beginn der Behandlung Vorsicht geboten sein.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie:

  • An andern Krankheiten leiden,
  • Allergien haben oder
  • Andere Arzneimittel (auch selbst gekaufte!) einnehmen!
+-Darf Vascord während einer Schwangerschaft oder in der Stillzeit angewendet werden?

Schwangerschaft

Teilen Sie Ihrem Arzt bzw. Ihre Ärztin mit, wenn Sie vermuten, schwanger zu sein (oder schwanger werden könnten). In der Regel wird Ihr Arzt bzw. Ihre Ärztin Ihnen empfehlen, Vascord vor einer Schwangerschaft bzw. sobald Sie wissen, dass Sie schwanger sind, abzusetzen, und er/sie wird Ihnen ein anderes Arzneimittel empfehlen. Die Anwendung von Vascord in den drei ersten Schwangerschaftsmonaten wird nicht empfohlen und Vascord darf nicht mehr nach dem dritten Schwangerschaftsmonat eingenommen werden, da die Einnahme von Vascord in diesem Stadium zu schweren Schädigungen Ihres ungeborenen Kindes führen kann.

Stillzeit

Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie stillen oder mit dem Stillen beginnen wollen. Vascord wird nicht zur Anwendung bei stillenden Müttern empfohlen; Ihr Arzt bzw. Ihre Ärztin kann eine andere Behandlung für Sie wählen, wenn Sie stillen wollen, vor allem, solange Ihr Kind im Neugeborenenalter ist oder wenn es eine Frühgeburt war.

Fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin um Rat, bevor Sie irgendein Arzneimittel einnehmen, wenn Sie schwanger sind oder stillen.

+-Wie verwenden Sie Vascord?

Wenden Sie Vascord immer genau nach den Anweisungen Ihres Arztes bzw. Ihrer Ärztin an. Sie sollten mit Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin nachprüfen, wenn Sie sich nicht sicher sind.

Die übliche Dosis beträgt eine Tablette Vascord pro Tag.

Die Tabletten können mit den Mahlzeiten oder nüchtern eingenommen werden. Schlucken Sie die Tablette mit etwas Wasser. Vascord darf nicht mit Grapefruitsaft eingenommen werden. Wenn möglich sollten Sie die Tagesdosis immer zur gleichen Tageszeit einnehmen, z.B. während des Frühstücks.

Kinder und Jugendliche (unter 18 Jahren)

Vascord wird nicht empfohlen zur Behandlung von Kindern und Jugendlichen unter 18 Jahren.

Wenn Sie eine grössere Menge Vascord eingenommen haben, als Sie sollten

Wenn Sie eine grössere Menge Tabletten als verordnet eingenommen haben, oder wenn ein Kind unbeabsichtigt Tabletten geschluckt hat, kann der Blutdruck zu niedrig werden, was sich in Form von Schwindelgefühl oder zu schnellem bzw. zu langsamem Puls äussern kann. Wenden Sie sich in diesen Fällen umgehend an Ihren Arzt bzw. Ihre Ärztin oder an die nächste Notaufnahme und nehmen Sie Ihr Arzneimittel oder diese Packungsbeilage mit.

Wenn Sie die Einnahme von Vascord vergessen haben

Wenn Sie eine Dosis Vascord vergessen haben, so nehmen Sie die normale Dosis wie üblich am nächsten Tag ein. Nehmen Sie keine zusätzlichen Tabletten zum Ausgleich der vergessenen Dosis ein.

Wenn Sie die Einnahme von Vascord abbrechen

Es ist wichtig, Vascord so lange einzunehmen, bis Ihr Arzt bzw. Ihre Ärztin Ihnen rät, die Behandlung abzubrechen.

Wenn Sie weitere Fragen zur Anwendung des Arzneimittels haben, fragen Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Vascord haben?

Wie alle Arzneimittel kann Vascord Nebenwirkungen haben, die aber nicht bei jedem auftreten müssen.

Die folgenden zwei Nebenwirkungen können schwerwiegend sein, auch wenn nicht viele Personen davon betroffen sind:

Allergische Reaktionen, die möglicherweise den ganzen Körper betreffen, mit Schwellung des Gesichtes, des Mundes und des Larynx (Kehlkopf), zusammen mit Juckreiz und Hautausschlag können während der Behandlung mit Vascord auftreten. Nehmen Sie in diesem Fall kein Vascord mehr ein und kontaktieren Sie sofort Ihren Arzt oder Ihr Ärztin.

Vascord kann bei dafür empfänglichen Personen oder als Folge einer allergischen Reaktion zu einem starken Abfall des Blutdruckes führen. Dies kann schweren Schwindel und Bewusstlosigkeit hervorrufen. Wenn dieser Fall eintritt, so kontaktieren Sie sofort Ihren Arzt oder Ihre Ärztin und legen Sie sich hin.

Andere mögliche Nebenwirkungen von Vascord:

Schwindel; Kopfschmerzen; Schwellung von Fussgelenken, Füssen, Beinen, Händen oder Armen; Müdigkeit.

Gelegentlich

Schwindel beim Aufstehen; Kraftlosigkeit; Kribbeln und Taubheitsgefühl in den Händen oder Füssen; Schwindelgefühl; starkes Herzklopfen; schneller Herzschlag; niedriger Blutdruck mit Symptomen wie Schwindel und Benommenheit; erschwertes Atmen; Husten, Übelkeit; Erbrechen; Verdauungsstörungen; Durchfall; Verstopfungen; trockener Mund; Schmerzen im Oberbauch; Ausschlag; Muskelkrämpfe; Schmerzen in Armen oder Beinen; Rückenschmerzen; erhöhter Harndrang; verminderte Libido; Schwierigkeiten beim Erlangen und Aufrechterhalten einer Erektion; Schwächegefühl.

Einige Veränderungen in den Blutwerten wurden ebenfalls beobachtet und schliessen unter anderem ein: Erhöhte oder erniedrigte Kaliumkonzentration im Blut; erhöhte Kreatininkonzentrationen im Blut; erhöhte Harnsäure; erhöhte Leberwerte (gamma-Glutamyltransferase).

Selten

Medikamentenüberempfindlichkeit; Ohnmacht; Gesichtsröte und Wärmegefühl im Gesicht; Ausschlag mit Quaddelbildung; Schwellungen des Gesichtes.

Weitere Nebenwirkungen, die im Zusammenhang mit dem Gebrauch von Olmesartanmedoxomil oder Amlodipin (die einzelnen Wirkstoffe, welche in Vascord enthalten sind) beobachtet wurden:

Olmesartanmedoxomil

Häufig (betrifft 1 bis 10 Behandelte von 100)

Bronchitis; Halsschmerzen; laufende oder verstopfte Nase; Husten; Bauchschmerzen; Magen-Darm-Entzündung; Durchfall; Verdauungsstörungen; Übelkeit; Schmerzen in den Gelenken und Knochen; Rückenschmerzen; Blut im Urin; Infektionen des Harntraktes; Schmerzen im Brustkorb; grippeähnliche Beschwerden; Schmerzen; Veränderungen in den Ergebnissen von Blutuntersuchungen wie erhöhte Blutfettwerte (Hypertriglyceridämie), erhöhte Harnstoff- oder Harnsäurewerte im Blut und Anstieg der Laborwertezur Untersuchung von Leber und Muskulatur.

Gelegentlich (betrifft 1 bis 10 Behandelte von 1?000)

Abnahme der Anzahl der Blutplättchen, was dazu führen kann, dass Sie leichter Blutergüsse bekommen oder Wunden länger bluten als üblich.

Überempfindlichkeitsreaktionen, die den ganzen Körper betreffen können und die sowohl Atemprobleme als auch einen raschen Blutdruckabfall, der sogar bis zum Ohnmachtsanfall führen kann, verursachen können (anaphylaktische Reaktionen, zum Teil schwere Reaktionen); Angina pectoris (Schmerzen oder Druckgefühl im Brustkorb); Juckreiz; Hautausschlag; allergischer Hautausschlag mit oder ohne; Quaddeln; Schwellungen des Gesichtes; Muskelschmerzen; allgemeines Unwohlsein.

Selten (betrifft 1 bis 10 Behandelte von 10?000)

Schwellung des Gesichts, des Mundes und/oder des Kehlkopfes (Stimmapparat), angioneurotisches Ödem; akutes Nierenversagen und Nierenfunktionsstörungen; Antriebslosigkeit.

Sehr selten (bis zu 1 von 10?000 Behandelten)

Starker, anhaltender Durchfall, welcher zu Gewichtsverlust führt.

Amlodipin

Häufig (betrifft 1 bis 10 Behandelte von 100)

Bauchschmerzen; Übelkeit; Schwellung der Fussgelenke; Schläfrigkeit; Rötung und Wärmegefühl des Gesichtes.

Gelegentlich (betrifft 1 bis 10 Behandelte von 1?000)

Schlaflosigkeit; Schlafstörungen; Stimmungsschwankungen, einschliesslich Angstgefühlen; Depressionen; Reizbarkeit; Zittern; Geschmacksveränderungen; Ohnmacht; Sehstörungen, einschliesslich Doppeltsehen; Läuten im Ohr (Tinnitus), (Verschlechterung einer) Angina pectoris (Schmerzen und unangenehmes Gefühl in der Brust); laufende oder verstopfte Nase; Haarausfall; rote Punkte oder Flecken auf der Haut aufgrund kleinster Blutungen (Purpura); Juckreiz; Verfärbungen der Haut; übermässiges Schwitzen; Hautausschlag; Muskel- und Gelenkschmerzen; Schwierigkeiten beim Wasserlassen; nächtlicher Harndrang; erhöhter Harndrang; Brustvergrösserung bei Männern; Schmerzen im Brustkorb; Schmerzen; Unwohlsein; Gewichtsverlust oder ?zunahme, veränderte Stuhlgewohnheiten.

Verwirrtheit.

Sehr selten (betrifft weniger als 1 Behandelten von 10?000)

Abnahme der Anzahl der weissen Blutkörperchen, wodurch das Infektionsrisiko erhöht sein kann; Abnahme der Anzahl der Blutplättchen, was dazu führen kann, dass Sie leichter Blutergüsse bekommen oder Wunden länger bluten als üblich; Anstieg des Blutzuckerspiegels; erhöhte Muskelspannung oder erhöhter Widerstand der Muskeln gegen Bewegung (Muskelhypertonie); Kribbeln oder Taubheitsgefühl der Hände oder Füsse; Herzinfarkt oder unregelmässiger Herzschlag; Entzündung der Blutgefässe; Entzündung der Leber oder der Bauchspeicheldrüse; Entzündung der Magenschleimhaut; Zahnfleischschwellung; erhöhte Leberenzymwerte; Hepatitis; Gelbfärbung von Haut und Augen; erhöhte Lichtempfindlichkeit der Haut; allergische Reaktionen (Juckreiz, Ausschlag, Schwellung des Gesichts, des Mundes und/oder des Kehlkopfes [Stimmapparat] zusammen mit Juckreiz und Ausschlag, andere allergische Reaktionen mit Hautentzündung oder Abschälen der Haut, die manchmal sogar lebensbedrohlich sein können); Steven-Johnson-Syndrom.
