+-Was ist Votum und wann wird es angewendet?

Votum gehört zu einer Gruppe von Arzneimitteln, die Angiotensin II-Rezeptor-Antagonisten genannt und zur Senkung des Blutdrucks angewendet werden.

Angiotensin II ist eine körpereigene Substanz, die die Blutgefässe verengt, dadurch den Blutfluss durch die Gefässe erschwert und zu einem Anstieg des Blutdrucks führt. Votum verhindert die Wirkung von Angiotensin II und bewirkt so eine Entspannung der Blutgefässe und eine Senkung des Blutdrucks.

Votum wird angewendet zur Behandlung des nicht organ-bedingten Bluthochdrucks, auch als essentielle Hypertonie bekannt.

Votum Filmtabletten sollen nur auf Verschreibung des Arztes/der Ärztin eingenommen werden.

+-Was sollte dazu beachtet werden?

Bluthochdruck kann die Blutgefässe verschiedener Organe, wie die des Herzens, der Nieren, des Gehirns und der Augen schädigen. In manchen Fällen kann dies zum Herzinfarkt, Herz- oder Nierenversagen, Schlaganfall oder zur Erblindung führen. Gewöhnlich zeigen sich keine Symptome des Bluthochdrucks. Es ist aber wichtig Ihren Blutdruck kontrollieren zu lassen und bei zu hohen Werten zu behandeln, um das Auftreten von späteren Schädigungen zu vermeiden.

Bluthochdruck kann mit Arzneimitteln wie Votum behandelt und unter Kontrolle gebracht werden. Ihr Arzt oder Ihre Ärztin hat Ihnen wahrscheinlich auch empfohlen, Ihre Lebensgewohnheiten zu ändern, um die Blutdrucksenkung zu unterstützen (Gewichtsreduktion, Aufgeben des Rauchens, Vermeidung von Alkohol, salzarme Ernährung). Er hat Sie möglicherweise auch ermutigt, sich regelmässig leicht körperlich zu betätigen, wie z.B. durch Gehen und Schwimmen. Es ist wichtig, diesen Anweisungen Ihres Arztes oder Ihrer Ärztin zu folgen.

Wie bei jedem blutdrucksenkenden Arzneimittel könnte ein übermässiger Blutdruckabfall bei Patienten mit Durchblutungsstörungen des Herzens oder des Gehirns zu einem Herzinfarkt oder Schlaganfall führen.

Ihr Arzt bzw. Ihre Ärztin wird deswegen Ihren Blutdruck sorgfältig überprüfen.

+-Wann darf Votum nicht eingenommen werden?

Votum darf nicht eingenommen werden, wenn

  • Sie allergisch (überempfindlich) gegenüber Olmesartanmedoxomil, dem Wirkstoff von Votum, oder einem der sonstigen Bestandteile von Votum Filmtabletten sind,
  • Sie Diabetes mellitus oder eine eingeschränkte Nierenfunktion haben und mit einem blutdrucksenkenden Arzneimittel, das Aliskiren enthält, behandelt werden,
  • Sie ab dem vierten Schwangerschaftsmonat schwanger sind (siehe «Darf Votum während einer Schwangerschaft oder in der Stillzeit eingenommen werden?»),
  • Sie schwere Leberprobleme haben, der Abfluss der Gallenflüssigkeit aus der Leber vermindert oder der Abfluss der Gallenflüssigkeit aus der Gallenblase verhindert ist (z.B. durch Gallensteine), oder wenn Sie eine Gelbsucht haben (Gelbfärbung der Haut und Augen),
  • Bei Ihnen früher anlässlich der Einnahme eines blutdrucksenkenden Arzneimittels Schwellungen im Gesicht, an Lippen und Rachen auftraten oder Sie an einer vererbten Störung des Immunsystems leiden, welche solche Schwellungen verursacht (sogenanntes Angioödem).
+-Wann ist bei der Einnahme von Votum Vorsicht geboten?

Dieses Arzneimittel enthält Lactose (Milchzucker). Falls bei Ihnen eine Unverträglichkeit gegenüber einigen Zuckern besteht, sprechen Sie mit Ihrem Arzt oder Ihrer Ärztin bevor Sie dieses Arzneimittel einnehmen.

Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie eines der folgenden Arzneimittel zur Behandlung von hohem Blutdruck einnehmen:

  • einen ACE-Hemmer (z.B. Enalapril, Lisinopril, Ramipril), insbesondere wenn Sie Nierenprobleme aufgrund von Diabetes mellitus haben,
  • Aliskiren.

Ihr Arzt bzw. Ihre Ärztin wird gegebenenfalls Ihre Nierenfunktion, Ihren Blutdruck und die Elektrolytwerte (z.B. Kalium) in Ihrem Blut in regelmässigen Abständen überprüfen.

Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie an anderen Krankheiten, insbesondere unter folgenden gesundheitlichen Problemen leiden:

  • Nierenerkrankungen oder nach Nierentransplantation,
  • Lebererkrankungen (bei stark eingeschränkter Leberfunktion wird die Einnahme von Votum nicht empfohlen),
  • Herzerkrankungen,
  • Schweres Erbrechen, Durchfall, hochdosierte Behandlung mit harntreibenden Arzneimitteln (Diuretika) oder wenn Sie eine salzarme Diät einhalten,
  • Erhöhter Kaliumspiegel im Blut,
  • Probleme mit Ihren Nebennieren, z.B. erhöhter Aldosteronspiegel (primärer Aldosteronismus).

Verständigen Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie starken anhaltenden Durchfall bekommen und dadurch erheblich an Gewicht verlieren.

Ihr Arzt bzw. Ihre Ärztin wird Ihre Beschwerden beurteilen und entscheiden, wie Ihr Blutdruck weiter behandelt werden soll.

Wechselwirkungen mit anderen Arzneimitteln

Bitte informieren Sie Ihren Arzt oder Apotheker bzw. Ärztin oder Apothekerin, wenn Sie folgende Arzneimittel einnehmen/anwenden bzw. vor kurzem eingenommen/angewendet haben:

  • Andere blutdrucksenkende Arzneimittel, da der Effekt von Votum verstärkt werden kann und die Häufigkeit der Nebenwirkungen höher sein kann,
  • Arzneimittel, die den Kaliumspiegel im Blut beeinflussen, wie kaliumhaltige Präparate, harntreibende Arzneimittel, blutgerinnungshemmende Arzneimittel (Heparin). Werden diese Arzneimittel zur gleichen Zeit wie Votum angewendet, können die Mengen an Kalium in Ihrem Blut erhöht werden,
  • Lithium zur Behandlung von bestimmten psychiatrischen Erkrankungen. Wird Lithium zur gleichen Zeit angewendet wie Votum kann die Toxizität von Lithium erhöht werden. Wenn Sie Lithium einnehmen müssen, wird Ihr Arzt bzw. Ihre Ärztin Ihren Lithium-Blutspiegel messen,
  • Arzneimittel gegen Schmerzen und Entzündungen (Nichtsteroidale Antirheumatika/Antiphlogistika). Werden diese Arzneimittel zur gleichen Zeit wie Votum verwendet, kann das Risiko von Nierenversagen erhöht sowie die Wirkung von Votum verringert werden,
  • Bestimmte Antazida (Arzneimittel gegen Sodbrennen), da diese die Wirkung von Votum leicht verringern können.

Verkehrstüchtigkeit und das Bedienen von Maschinen

Möglicherweise fühlen Sie sich schläfrig oder schwindlig, während der Behandlung Ihres Bluthochdrucks. Dadurch können die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigt sein.

Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie

  • an anderen Krankheiten leiden,
  • Allergien haben oder
  • andere Arzneimittel (auch selbst gekaufte!) einnehmen oder äusserlich anwenden!
+-Darf Votum während einer Schwangerschaft oder in der Stillzeit eingenommen werden?

Schwangerschaft

Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie vermuten, schwanger zu sein (oder schwanger werden könnten). In der Regel wird Ihr Arzt bzw. Ihre Ärztin Ihnen empfehlen, Votum vor einer Schwangerschaft bzw. sobald Sie wissen, dass Sie schwanger sind, abzusetzen, und er/sie wird Ihnen ein anderes Arzneimittel empfehlen. Die Anwendung von Votum in den drei ersten Schwangerschaftsmonaten wird nicht empfohlen und Votum darf nicht mehr nach dem dritten Schwangerschaftsmonat eingenommen werden, da die Einnahme von Votum in diesem Stadium zu schweren Schädigungen Ihres ungeborenen Kindes führen kann.

Stillzeit

Teilen Sie Ihrem Arzt bzw. Ihrer Ärztin mit, wenn Sie stillen oder mit dem Stillen beginnen wollen. Votum wird nicht zur Anwendung bei stillenden Müttern empfohlen; Ihr Arzt bzw. Ihre Ärztin kann eine andere Behandlung für Sie wählen, wenn Sie stillen wollen, vor allem, solange Ihr Kind im Neugeborenenalter ist oder wenn es eine Frühgeburt war.

Fragen Sie vor der Einnahme von allen Arzneimitteln Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin um Rat.

+-Wie verwenden Sie Votum?

Nehmen Sie dieses Arzneimittel immer genau nach der Anweisung des Arztes bzw. der Ärztin ein. Bitte fragen Sie bei Ihrem Arzt oder Apotheker bzw. bei Ihrer Ärztin oder Apothekerin nach, wenn Sie sich nicht ganz sicher sind.

Votum Filmtabletten können mit den Mahlzeiten oder unabhängig davon eingenommen werden. Die Filmtabletten sollten mit 1 Glas Wasser geschluckt werden. Sie sollten Ihre tägliche Dosis möglichst um die gleiche Tageszeit nehmen, z.B. mit dem Frühstück. Es ist wichtig, dass Sie Votum so lange einnehmen, bis Ihr Arzt bzw. Ihre Ärztin Ihnen etwas anderes sagt.

Falls vom Arzt bzw. von der Ärztin nicht anders verordnet, ist die empfohlene Anfangsdosierung 1 Filmtablette Votum 10 mg täglich. Wenn keine ausreichende Kontrolle des Blutdrucks mit einer Filmtablette Votum 10 mg erreicht wird, kann Ihr Arzt bzw. Ihre Ärztin die Dosis auf 1 Filmtablette Votum 20 mg oder Votum 40 mg täglich erhöhen oder eine Zusatztherapie verordnen.

Ältere Patienten

Wenn Sie über 65 Jahre alt sind und Ihr Arzt bzw. Ihre Ärztin entscheidet, Ihre Dosis von Olmesartanmedoxomil auf 40 mg täglich zu erhöhen, dann müssen Sie Ihren Blutdruck regelmässig von Ihrem Arzt bzw. Ihrer Ärztin überprüfen lassen, um sicherzustellen, dass Ihr Blutdruck nicht zu niedrig ist.

Kinder und Jugendliche

Votum soll nicht an Kinder und Jugendliche unter 18 Jahren verabreicht werden.

Anwendungsfehler und Überdosierung

Wenn Sie eine grössere Menge Votum eingenommen haben, als Sie sollen, oder wenn ein Kind die Filmtabletten geschluckt hat, setzen Sie sich sofort mit Ihrem Arzt bzw. Ärztin oder mit der Notaufnahme des nächsten Krankenhauses in Verbindung und nehmen Sie die Arzneimittelpackung mit.

Wenn Sie einmal die tägliche Dosis vergessen haben, nehmen Sie Ihre normale Dosis am nächsten Tag ein. Nehmen Sie ein, um die vergessene Einnahme auszugleichen.

Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.

+-Welche Nebenwirkungen kann Votum haben?

Folgende Nebenwirkungen können bei der Einnahme von Votum auftreten

Allergische Reaktionen, die möglicherweise den ganzen Körper betreffen, mit Schwellung von Gesicht, Mund und/oder Kehlkopf gemeinsam mit Juckreiz und Ausschlag können selten (bis zu 1 von 1?000 Personen) auftreten. Sollte dies auftreten, nehmen Sie keine weitere Tablette Votum ein und kontaktieren Sie umgehend Ihren Arzt bzw. Ihre Ärztin.

Selten (aber etwas häufiger bei älteren Patienten) kann Votum bei empfindlichen Personen oder als Folge einer allergischen Reaktion eine zu starke Blutdrucksenkung auslösen. Gelegentlich kann eine Benommenheit oder Ohnmacht auftreten. Sollte dies eintreten, nehmen Sie keine weitere Tablette Votum ein, verständigen Sie umgehend Ihren Arzt bzw. Ihre Ärztin und legen Sie sich flach hin.

Dies sind die anderen Nebenwirkungen, welche von Votum bekannt sind

Häufig (bis zu 1 von 10 Personen)

Schwindelgefühl; Kopfschmerz; Übelkeit; Verdauungsstörungen; Durchfall; Bauchschmerz; Magen-Darmentzündung; Müdigkeit; Halsentzündung; Schnupfen; Bronchitis; grippeähnliche Symptome; Husten; Schmerzen; Schmerzen in der Brust; Gelenkschmerzen sowie Gelenkentzündung; Rückenschmerzen; Knochenschmerz; Harnweginfektionen; geschwollene Knöchel, Füsse, Beine, Hände oder Arme; Blut im Urin.

Einige Veränderungen der Ergebnisse von Blutuntersuchungen wurden ebenfalls beobachtet; dazu gehören: Anstieg der Blutfette, Anstieg von Harnstoff oder Harnsäure im Blut; Anstieg der Leber- und Muskelfunktionswerte.

Gelegentlich (bis zu 1 von 100 Personen)

Überempfindlichkeitsreaktionen, die den ganzen Körper betreffen können und die sowohl Atemprobleme als auch einen raschen Blutdruckabfall, der sogar bis zum Ohnmachtsanfall führen kann, verursachen können (anaphylaktische Reaktionen, zum Teil schwere Reaktionen); Schwindel; Erbrechen; Schwächegefühl; Unwohlsein; Muskelschmerz; Hautausschlag; allergischer Hautausschlag; Juckreiz; Exanthem (Hautausschlag); Hauterhebungen (Quaddeln); Angina pectoris (plötzlich einsetzende Schmerzen im Brustkorb.

In Bluttests wurde eine Reduktion der Anzahl an Blutplättchen (Thrombozytopenie) beobachtet.

Selten (bis zu 1 von 1?000 Personen)

Kraftlosigkeit; Muskelkrämpfe; Niereninsuffizienz; akutes Nierenversagen. Einige Veränderungen der Ergebnisse von Blutuntersuchungen wurden ebenfalls selten beobachtet; dazu gehört: Anstieg von Kalium im Blut (Hyperkaliämie).

Sehr selten (bis zu 1 von 10?000 Personen)

Starker, anhaltender Durchfall, welcher zu Gewichtsverlust führt.

Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.

+-Was ist ferner zu beachten?

Nicht über 30 °C und vor Licht geschützt aufbewahren.

Für Kinder unzugänglich aufzubewahren.

Das Arzneimittel darf nur bis zu dem auf der Verpackung aufgedruckten Verfalldatum verwendet werden.

Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin. Diese Personen verfügen über die ausführliche Fachinformation.

Medikamente sind nicht über das Abwasser oder den Hausmüll zu entsorgen. Fragen Sie Ihren Apotheker bzw. Ihre Apothekerin, wie das Arzneimittel, welches Sie nicht mehr verwenden, zu entsorgen ist. Diese Massnahme hilft, die Umwelt zu schützen.

Votum Filmtabletten enthalten den Wirkstoff Olmesartanmedoxomil sowie Hilfsstoffe.

Votum 10 mg enthält 10 mg Olmesartanmedoxomil.

Votum 20 mg enthält 20 mg Olmesartanmedoxomil.

Votum 40 mg enthält 40 mg Olmesartanmedoxomil.
