Zanidip ist ein Arzneimittel zur Behandlung von leichtem bis mittelschwerem, nicht organbedingtem (essenziellem) Bluthochdruck.
Der enthaltene Wirkstoff Lercanidipin gehört zur Arzneimittelgruppe der Calciumkanalblocker (vom Dihydropyridin-Typ), welche zur Behandlung von Bluthochdruck eingesetzt wird.
Zanidip darf nur auf Verschreibung des Arztes oder der Ärztin verwendet werden.
Die Wirkung von Zanidip kann durch Grapefruitsaft verstärkt werden, und dieser sollte folglich gemieden werden.
Die Wirkung gefässerweiternder blutdrucksenkender Arzneimittel kann durch Alkohol verstärkt werden, so dass dringend angeraten wird, während der Einnahme von Zanidip, den Genuss von Alkohol zu vermeiden.
Zanidip darf nicht eingenommen werden:
wenn Sie früher einmal auf den Wirkstoff Lercanidipin oder eng verwandte Arzneimittel allergisch reagiert haben (wie z.B. Amlodipin, Nicardipin, Felodipin, Isradipin, Nifedipin oder Lacidipin) oder gegen einen der sonstigen Bestandteile des Arzneimittels allergisch sind;
wenn Sie schwanger sind, gerade stillen oder im gebärfähigen Alter sind und keine sichere Verhütung anwenden;
wenn Sie bestimmte Herzleiden haben wie: medikamentös nicht vollständig behandelbare Herzmuskelschwäche (Herzinsuffizienz), Behinderung des Blutabflusses aus dem Herzen, instabile Angina pectoris (Ruheangina oder unter Belastung graduell sich verschlimmernde Angina pectoris), falls Sie vor weniger als einem Monat einen Herzinfarkt hatten;
falls Sie schwer leber- oder nierenkrank sind;
bei gleichzeitiger Einnahme von:
sogenannten starken CYP3A4-Inhibitoren (z.B. Ketoconazol, Itraconazol, Erythromycin, Troleandomycin, Ritonavir); Cyclosporin; Grapefruitsaft;
bei Kindern und Jugendlichen unter 18 Jahren.
Durch individuell auftretende unterschiedliche Reaktionen kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und Fähigkeit Werkzeuge oder Maschinen zu bedienen beeinträchtigt werden. Dies gilt in besonderem Masse bei Behandlungsbeginn, Dosiserhöhung und Arzneimittelwechsel sowie im Zusammenwirken mit Alkohol.
Zanidip darf nur unter besonderer Vorsicht eingenommen werden, wenn Sie an bestimmten Herzrhythmusstörungen (Sinusknotensyndrom) leiden und keinen Schrittmacher tragen. Ihr Arzt oder Ihre Ärztin muss davon unbedingt unterrichtet werden, damit dies bei der Verordnung von Zanidip berücksichtigt werden kann.
Wenn Sie an leichter bis mittelschwerer Leber- oder Nierenerkrankung leiden oder Dialysen durchführen lassen müssen, informieren Sie Ihren Arzt oder Ihre Ärztin darüber, damit die Dosis entsprechend abgestimmt werden kann.
Eine Tablette enthält 30 mg bzw. 60 mg Laktose (Zanidip 10 mg bzw. Zanidip 20 mg) und sollte daher nicht von Patienten eingenommen werden, die an Laktase-Mangel, Galaktosämie oder Glukose-Galaktose-Malabsorption leiden.
Wenn Sie Zanidip gleichzeitig mit Arzneimitteln wie Cimetidin (in einer Dosis von mehr als 800 mg täglich), Digoxin oder Midazolam regelmässig einnehmen, sollten Sie ärztlich überwacht werden. Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie eines dieser Arzneimittel einnehmen.
Während der Behandlung mit Zanidip ist Vorsicht geboten, wenn Sie mit einem der folgenden Arzneimittel behandelt werden: Rifampicin, Terfenadin, Astemizol, Antiarrhythmika der Klasse III, wie z.B. Amiodaron oder Chinidin. Informieren Sie Ihren Arzt oder Ihre Ärztin, wenn Sie eines dieser Arzneimittel einnehmen. Ihr Arzt oder Ihre Ärztin werden Sie beraten.
Vorsicht ist geboten, wenn Sie Mittel gegen Krampfanfälle wie z.B. Phenytoin oder Carbamazepin einnehmen, da die blutdrucksenkende Wirkung von Zanidip herabgesetzt werden kann.
Orale Antimykotika (Mittel gegen Pilzerkrankungen zum Einnehmen, z.B. Ketoconazol oder Itraconazol), Makrolid-Antibiotika (z.B. Erythromycin oder Troleandomycin) und antivirale Mittel zum Einnehmen gegen HIV (z.B. Ritonavir) können die Wirkung von Zanidip verstärken. Die gleichzeitige Anwendung sollte daher vermieden werden.
Wird Zanidip gemeinsam mit einem Arzneimittel eingenommen, das Cyclosporin enthält, kann eine Wirkungsverstärkung beider Mittel auftreten; sie sollten daher nicht zusammen eingenommen werden.
Zanidip kann zusammen mit Betablockern (Mittel gegen Bluthochdruck oder bestimmte Herzkrankheiten) sicher angewendet werden, jedoch kann eine Dosisanpassung von Zanidip erforderlich sein.
Bei gleichzeitiger Anwendung von Zanidip und Simvastatin (Mittel zur Senkung des Cholesterinspiegels) sollte darauf geachtet werden, dass beide Arzneimittel wie vorgeschrieben eingenommen werden (Zanidip morgens und Simvastatin abends).
Bezüglich der Wirkung von Grapefruitsaft oder Alkohol in Verbindung mit Zanidip siehe unter «Was sollte dazu beachtet werden».
Die Behandlung mit diesem Arzneimittel bedarf der regelmässigen ärztlichen Kontrolle.
Informieren Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen!
Wenn Sie schwanger sind oder es werden möchten, wenn Sie keine Verhütungsmethode anwenden und wenn Sie stillen, dürfen Sie Zanidip nicht einnehmen.
Wenn Sie Zanidip einnehmen und den Verdacht haben schwanger zu sein, setzen Sie sich mit Ihrem Arzt oder Ihrer Ärztin in Verbindung.
Nehmen Sie Zanidip wie von Ihrem Arzt oder Ihrer Ärztin verschrieben ein.
Die empfohlene Dosis beträgt einmal täglich 1 Filmtablette Zanidip 10 mg (gelb). Abhängig vom Ansprechen des Patienten kann die Dosis auf 2 Filmtabletten Zanidip 10 mg (gelb) bzw. 1 Filmtablette Zanidip 20 mg (rosafarben) erhöht werden. Die Dosissteigerung sollte schrittweise erfolgen, da es etwa 2 Wochen dauern kann, bis die maximale blutdrucksenkende Wirkung erreicht wird.
Nehmen Sie die Tablette immer zur gleichen Zeit, am besten morgens ein. Eine Mahlzeit mit hohem Fettanteil erhöht die Blutspiegel des Arzneimittels signifikant. Schlucken Sie die Tablette möglichst unzerkaut mit einem Glas Wasser.
Bei älteren Patienten ist eine Anpassung der täglichen Dosis nicht erforderlich. Dennoch sollte in den ersten Therapiewochen der Blutdruck wiederholt kontrolliert werden um gelegentliche Fälle von niedrigem Blutdruck zu erkennen. Bitte konsultieren Sie Ihren Arzt bzw. Ihre Ärztin beim Auftreten von Schwindel oder Herzklopfen. Im Falle einer Überdosierung, die zu übermässiger peripherischer Gefässerweiterung mit ausgeprägter Senkung des Blutdrucks führen kann, nehmen Sie bitte Kontakt mit Ihrem Arzt bzw. mit Ihrer Ärztin auf.
Wenn Sie einmal eine Einnahme vergessen haben, holen Sie Ihre normale Dosis möglichst sofort nach und nehmen Sie am nächsten Tag Zanidip wieder wie verschrieben ein. Nehmen Sie auf keinen Fall die doppelte Dosis auf einmal ein.
Die verschriebene Dosis darf nicht überschritten werden.
Falls Sie mehr als die verschriebene Dosis einnehmen oder im Falle einer Überdosierung, müssen Sie sofort ärztliche Hilfe aufsuchen und falls möglich die Tabletten und/oder die Packung mitnehmen.
Überschreiten der richtigen Dosis kann dazu führen, dass der Blutdruck zu stark absinkt und die Herzschläge unregelmässig und schneller werden. Ausserdem kann es zu Bewusstlosigkeit kommen. Da der Wirkstoff von Zanidip lange wirksam ist, ist es wichtig, dass der Zustand nach einer Dosisüberschreitung mindestens 24 Stunden beobachtet wird.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme von Zanidip auftreten:
Gelegentlich können Kopfschmerzen, Schwindel, Anschwellen der Knöchel oder Beine, erhöhter Puls, Herzklopfen oder Gesichtsrötung auftreten.
Selten treten Hautausschlag, Muskelschmerzen, Schläfrigkeit, Übelkeit, Verdauungsstörungen, Durchfall, Bauchschmerzen, Erbrechen, Angina pectoris, erhöhtes Urinvolumen, Schwäche oder Ermüdung auf.
Von den folgenden sehr seltenen Nebenwirkungen wurde berichtet:
Verdickung des Zahnfleisches, vorübergehende Erhöhungen der Serumspiegel von Leberenzymen (Transaminasen), zu niedriger Blutduck, häufigeres Wasserlassen und Brustschmerzen.
Manche Dihydropyridine können selten zu Angina pectoris Anfällen (Schmerzen in der Herzgegend mit Beengungsgefühl) führen; sehr selten kann es bei Patienten mit bestehender Angina pectoris zu einer Zunahme von Häufigkeit, Dauer und Schweregrad der Anfälle kommen. Sollten bei Ihnen ähnliche Probleme mit Zanidip auftreten, verständigen Sie bitte sofort Ihren Arzt oder Ihre Ärztin.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Zanidip sollte von Personen unter 18 Jahren nicht eingenommen werden.
Bewahren Sie Zanidip bei Raumtemperatur (15?25 °C) und in der Originalverpackung ausserhalb der Reichweite von Kindern auf.
Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
Weitere Auskünfte erteilt Ihnen Ihr Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin, die über die ausführliche Fachinformation verfügen.
Gelbe Filmtablette: 10 mg Lercanidipinhydrochlorid.
Rosafarbene Filmtablette: 20 mg Lercanidipinhydrochlorid.
Hilfsstoffe: Laktose-Monohydrat, Farbstoffe: Titandioxid (E 171), Eisen(III)-oxid (E 172) sowie andere Filmtablettenhilfsstoffe.
