Zonegran Kapseln enthalten Zonisamid, ein Antiepileptikum. Es wird auf Verschreibung des Arztes oder der Ärztin zur Behandlung fokaler epileptischer Anfälle mit oder ohne sekundäre Generalisierung verwendet:
Zonegran Kapseln zu 100 mg enthalten die beiden Farbstoffe Gelborange S (E110) und Allurarot AC (E129). Diese können allergische Reaktionen hervorrufen.
Sie dürfen Zonegran nicht einnehmen:
Zonegran gehört zu einer Gruppe von Arzneimitteln (Sulfonamide), die zu schwerwiegenden allergischen Reaktionen, schwerwiegenden Hautausschlägen und Erkrankungen des Blutes führen kann, die in äusserst seltenen Fällen sehr gefährlich (lebensbedrohlich) sein können.
Patientinnen und Patienten, die auf Azofarbstoffe, Acetylsalicylsäure sowie Rheuma- und Schmerzmittel (Prostaglandinhemmer) überempfindlich reagieren, sollen Zonegran Kapseln zu 100 mg nicht einnehmen.
Dieses Arzneimittel kann die Reaktionsfähigkeit, die Fahrtüchtigkeit und die Fähigkeit, Werkzeuge oder Maschinen zu bedienen, beeinträchtigen! Zonegran kann insbesondere bei höheren Dosierungen (300-500 mg/Tag) Schläfrigkeit und Müdigkeit verursachen. Über Doppeltsehen wurde häufig berichtet. Sie sollten solange kein Fahrzeug führen oder Maschinen bedienen, bis Sie genügend Erfahrung mit Zonegran gewonnen haben, um sicher zu sein, dass das Arzneimittel Ihre Leistungsfähigkeit nicht beeinträchtigt.
Wenn Sie eine ältere Patientin bzw. ein älterer Patient sind, da zur Anwendung von Zonegran bei dieser Altersgruppe nur begrenzte Informationen verfügbar sind und die Dosis Ihres Arzneimittels möglicherweise angepasst werden muss und Sie möglicherweise eher eine allergische Reaktion oder einen schwerwiegenden Hautausschlag entwickeln, wenn sie Zonegran einnehmen (siehe Welche Nebenwirkungen kann Zonegran haben?).
Wenn Sie eine Frau im gebärfähigen Alter sind. Sie müssen während der Einnahme von Zonegran und bis einen Monat nach Absetzen von Zonegran ohne Unterbrechung geeignete empfängnisverhütende Massnahmen anwenden.
Wenn Sie einen unerklärlichen Hautausschlag aufweisen, da sich dieser zu einem schwerwiegenderen Hautausschlag oder einem Abschälen der Haut entwickeln könnte. Wenden Sie sich sofort an Ihren Arzt bzw. Ihre Ärztin, da dies in sehr seltenen Fällen schwerwiegend werden kann.
Wenn Sie unter Leberproblemen leiden, da zur Anwendung von Zonegran bei dieser Patientengruppe nur begrenzte Informationen verfügbar sind und die Dosis Ihres Arzneimittels möglicherweise verändert oder langsamer gesteigert werden muss.
Wenn Sie unter Nierenproblemen leiden, da nur eingeschränkte Informationen zur Anwendung von Zonegran bei dieser Patientengruppe verfügbar sind und die Dosis Ihres Arzneimittels möglicherweise verändert oder langsamer gesteigert werden muss.
Wenn Sie plötzlich Schmerzen im Rücken oder der Magengegend haben, Schmerzen beim Wasserlassen oder Blut im Urin bemerken, da dies ein Anzeichen für Nierensteine sein kann. Zonegran kann die Bildung von Nierensteinen begünstigen. Risikofaktoren für eine Nierensteinbildung sind, neben familiärer Veranlagung, bereits früher aufgetretene Nierensteine und gesteigerte Kalzium-Ausscheidung im Harn oder die gleichzeitige Einnahme anderer Arzneimittel, welche die Nierensteinbildung fördern. Es ist sehr wichtig, während der Therapie mit Zonegran auf eine ausreichende Trinkmenge zu achten, um dieses Risiko zu vermindern.
Wenn Sie vermindertes Schwitzen bemerken; dies kann zu einem Hitzschlag führen, der eine Einweisung ins Spital notwendig machen kann. Vermindertes Schwitzen und eine Erhöhung der Körpertemperatur treten vor allem bei erhöhter Umgebungstemperatur auf. Bei hoher Umgebungstemperatur sollen sportliche Aktivitäten vermieden werden. Achten Sie darauf, ausreichend zu trinken.
Wenn Sie viel Gewicht verloren haben oder unter 20 kg wiegen. Informieren Sie Ihren Arzt bzw. Ihre Ärztin, da das Körpergewicht möglicherweise überwacht werden muss.
Wenn Sie ungewöhnlich müde sind oder seit einigen Tagen Halsschmerzen haben oder bemerken, dass Sie leicht blaue Flecken bekommen, wenden Sie sich an Ihren Arzt bzw. Ihre Ärztin, da dies ein Anzeichen dafür sein kann, dass Sie eine Erkrankung des Blutsystems haben.
Wenn Sie Kopfschmerzen, Schläfrigkeit, Kurzatmigkeit und Appetitverlust haben, da dies ein Anzeichen von zu tiefem Bikarbonat-Blutspiegel sein könnte, der von Ihrem Arzt bzw. Ihrer Ärztin überwacht und behandelt werden muss.
Wenn Sie an Muskelschmerzen oder einem Gefühl von Muskelschwäche leiden, informieren Sie Ihren Arzt bzw. Ihre Ärztin.
Wenn Sie an Depression leiden und/oder wenn Sie Gedanken haben, sich selbst zu verletzen oder sich das Leben zu nehmen. Eine geringe Anzahl von Patienten und Patientinnen, die mit Antiepileptika wie Zonegran behandelt wurden, hatten Gedanken, sich selbst zu verletzen oder sich das Leben zu nehmen.
Wenn Sie Ihre Behandlung absetzen, befolgen Sie die Anweisungen Ihres Arztes bzw. Ihrer Ärztin bezüglich einer schrittweisen Verringerung der Zonegran-Dosis.
Bitte wenden Sie sich auch an Ihren Arzt bzw. Ihre Ärztin, wenn diese Warnhinweise früher einmal auf Sie zugetroffen haben.
Die Wirkung von Zonisamid kann durch andere Arzneimittel, die Sie einnehmen, verringert werden. Dazu gehören Arzneimittel zur Behandlung einer Epilepsie wie z.B. Phenytoin, Carbamazepin und Phenobarbital. In diesem Fall kann eine Anpassung Ihrer Zonegran-Dosis erforderlich sein.
Wenn Ihr Kind jünger als 6 Jahre ist, da die Verwendung von Zonegran für diese Altersgruppe nicht empfohlen wird.
Wenn Zonegran einem Kind gegeben wird, da für Ihr Kind ein erhöhtes Risiko für vermindertes Schwitzen; dies kann zu einem Hitzschlag, Lungenentzündung und Leberproblemen führen.
Vorbeugung vor Überhitzung/Hitzschlag und Austrocknung bei Kindern: Zonegran kann bei Ihrem Kind zu verminderter Schweissbildung und damit zur Überhitzung des Körpers führen. Wird Ihr Kind nicht behandelt, kann es zu Hirnschäden mit tödlichem Verlauf kommen. Die grösste Gefahr für Kinder besteht bei heissem Wetter.
Beachten Sie während der Behandlung des Kindes mit Zonegran bitte folgende Vorsichtsmassnahmen:
Wenn sich die Haut Ihres Kindes sehr heiss anfühlt und nur eine geringe oder gar keine Schweissabsonderung auftritt oder wenn das Kind Verwirrtheitszustände zeigt, Muskelkrämpfe oder einen beschleunigten Puls oder eine beschleunigte Atmung hat:
Körpergewicht: Sie sollten das Gewicht Ihres Kindes jeden Monat kontrollieren und sobald wie möglich Ihren Arzt aufsuchen, wenn Ihr Kind nicht genügend an Gewicht zunimmt. Die Anwendung von Zonegran bei Kindern mit Untergewicht oder geringem Appetit wird nicht empfohlen und Zonegran muss bei Kindern mit einem Körpergewicht unter 20 kg mit Vorsicht angewendet werden.
Übersäuerung des Blutes und Nierensteine: Zur Senkung dieser Risiken sollten Sie dafür sorgen, dass Ihr Kind ausreichend Wasser trinkt und keine anderen Medikamente einnimmt, die zur Nierensteinbildung führen könnten. Ihr Arzt wird den Bicarbonat-Blutspiegel Ihres Kindes und seine Nieren regelmässig kontrollieren.
Informieren Sie Ihren Arzt, Apotheker bzw. Ihre Ärztin, Apothekerin, wenn Sie an anderen Krankheiten leiden, Allergien haben oder andere Arzneimittel (auch selbstgekaufte!) einnehmen oder äusserlich anwenden.
Frauen im gebärfähigen Alter müssen während einer Zonegran-Behandlung und bis einen Monat nach Absetzen von Zonegran geeignete empfängnisverhütende Massnahmen anwenden.
Wenn Sie schwanger sind oder vermuten, schwanger zu sein, oder eine Schwangerschaft planen, informieren Sie Ihren Arzt bzw. Ihre Ärztin. Sie dürfen Zonegran während einer Schwangerschaft nur einnehmen, wenn Ihr Arzt bzw. Ihre Ärztin Sie dazu anweist. Untersuchungen haben gezeigt, dass bei Kindern von Frauen mit Antiepileptika-Behandlung ein erhöhtes Risiko für angeborene Missbildungen besteht. Sie dürfen jedoch auf keinen Fall selbstständig die Zonegran Behandlung abbrechen.
Solange Sie Zonegran einnehmen und bis einen Monat nach dem Absetzen von Zonegran dürfen Sie nicht stillen.
Nehmen Sie Zonegran immer genau nach Anweisung des Arztes bzw. der Ärztin ein. Bitte fragen Sie bei Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin nach, wenn Sie sich nicht ganz sicher sind.
Wenn Sie Zonegran alleine zur Behandlung von fokalen epileptischen Anfällen einnehmen:
Die Anfangsdosis beträgt 100 mg täglich als Einmaldosis, die Dosis kann um bis zu 100 mg alle zwei Wochen erhöht werden. Die übliche Tagesdosis beträgt 300 mg einmal pro Tag.
Wenn Sie Zonegran zusammen mit anderen Arzneimitteln zur Behandlung von fokalen epileptischen Anfällen einnehmen:
Die übliche Anfangsdosis beträgt 50 mg täglich, aufgeteilt in zwei gleiche Einzeldosen. Die Dosierung wird für Sie von Ihrem Arzt bzw. Ihrer Ärztin angepasst und kann in ein- bis zweiwöchentlichen Abständen um bis zu 100 mg erhöht werden, bis eine tägliche Gesamtdosis von 300 mg bis 500 mg erreicht ist. Einige Patienten bzw. Patientinnen sprechen möglicherweise bereits auf geringere Dosierungen an. Die Dosis kann langsamer erhöht werden, wenn Sie unter Nebenwirkungen leiden oder wenn Sie bereits ein Nierenproblem haben.
Die Anfangsdosis beträgt einmal täglich 1 mg pro Kilogramm Körpergewicht. Die Dosierung wird alle 1 bis 2 Wochen um 1 mg pro Kilogramm Körpergewicht erhöht, bis eine Dosierung von üblicherweise 8 mg pro Kilogramm Körpergewicht oder bis zu 500 mg (je nachdem, was niedriger ist) einmal pro Tag erreicht ist.
Zonegran Kapseln müssen ganz mit Wasser geschluckt werden. Kauen Sie die Kapseln nicht. Die Kapseln müssen ein- oder zweimal täglich nach Anweisung des Arztes bzw. der Ärztin eingenommen werden. Wenn Sie die Kapseln zweimal täglich einnehmen, sollte die Hälfte der täglichen Dosis morgens und die andere Hälfte abends eingenommen werden. Zonegran kann mit oder ohne Nahrung eingenommen werden.
Die Anwendung und Sicherheit von Zonegran bei Kindern und Jugendlichen unter 6 Jahren ist bisher nicht geprüft worden.
Zonegran wird zur Langzeitbehandlung eingesetzt. Verringern Sie die Dosis nicht und setzen Sie Ihre Arzneimittel nicht ab, es sei denn, Ihr Arzt bzw. Ihre Ärztin hat Sie dazu angewiesen.
Wenn Sie mehr Zonegran eingenommen haben, als Sie sollten, teilen Sie es sofort einer Betreuungsperson (Verwandter oder Freund), Ihrem Arzt oder Apotheker bzw. Ihrer Ärztin oder Apothekerin mit oder wenden Sie sich an die nächste Notfallaufnahme eines Krankenhauses. Nehmen Sie dabei Ihr Arzneimittel mit. Sie könnten möglicherweise schläfrig werden oder das Bewusstsein verlieren. Fahren Sie in dieser Zeit kein Fahrzeug.
Wenn Sie die Einnahme einer Dosis vergessen haben, nehmen Sie Ihr Arzneimittel wie gewohnt weiter. Nehmen Sie nicht die doppelte Dosis ein, wenn Sie die vorherige Einnahme vergessen haben.
Wenn Ihr Arzt bzw. Ihre Ärztin Sie dazu anweist, die Behandlung abzusetzen, wird die Dosis von Zonegran schrittweise reduziert, um das Risiko vermehrter Krampfanfälle zu senken.
Ändern Sie nicht von sich aus die verschriebene Dosierung. Wenn Sie glauben, das Arzneimittel wirke zu schwach oder zu stark, so sprechen Sie mit Ihrem Arzt oder Apotheker bzw. mit Ihrer Ärztin oder Apothekerin.
Folgende Nebenwirkungen können bei der Einnahme von Zonegran auftreten:

Kopfschmerzen, Schnupfen, Rachenentzündung, Schlaflosigkeit, merkwürdige oder ungewöhnliche Gedanken, ein Gefühl der Ängstlichkeit oder Emotionalität, Benommenheit, Mundtrockenheit, Geschmackstörung, Ernährungsstörung, Verdauungsstörungen, Appetitverlust, Gewichtsverlust, Schwindel, Konzentrationsverlust, Müdigkeit, Übelkeit, körperliche Unruhe oder Reizbarkeit, unfreiwillige Augenbewegungen, Sehen von Doppelbildern, schwaches Sehen, Depression, schlechte Muskelkoordination (unsichere Bewegungsabläufe), Verwirrtheit, schlechtes Gedächtnis, verlangsamter Gedankengang, Magenschmerzen, Verstopfung, Durchfall (weicher Stuhlgang), Sprachstörungen, Hautausschlag, Haarausfall, Hautblutung, Missempfindung auf der Haut (Ameisenlaufen), Zittern, Grippe-ähnliche Anzeichen, Fieber und allergische Reaktionen, Nierensteine.
Zorn, Aggression, Selbsttötungsgedanken, Selbsttötungsabsicht, Erbrechen, Gallenblasenentzündungen oder Gallensteine, Harnsteine, Lungenentzündung, Harnwegsinfektionen, niedrige Kaliumspiegel im Blut und Anfälle/Krämpfe, Störungen der Nierenfunktion.
Erkrankungen des Blutes (möglicherweise blass aussehen, sich müde oder fiebrig fühlen, oder leichteres Auftreten von blauen Hautflecken), Drüsenschwellungen, Halluzinationen, Gedächtnisverlust, Koma, malignes neuroleptisches Syndrom (Unfähigkeit, sich zu bewegen, Schwitzen, Fieber, Inkontinenz), Status epilepticus (längere oder wiederholte Krampfanfälle), Atmungsstörungen, Kurzatmigkeit, Lungenentzündung, Bauchspeicheldrüsenentzündung, Leberschäden, Juckreiz, schwerwiegende Ausschläge oder ein Abschälen der Haut (gleichzeitig können Sie sich unwohl fühlen oder entwickeln Fieber), verringertes Schwitzen und Hitzeschlag, Muskelschmerzen, Muskelschwäche, erhöhter Kreatininspiegel im Blut (Kreatinin ist ein Abfallprodukt, das Ihre Nieren normalerweise ausscheiden sollten), plötzlicher unerwarteter Tod bei Epilepsie (SUDEP).
Informieren Sie Ihren Arzt bzw. Ihre Ärztin, wenn Sie unter einer dieser Nebenwirkungen leiden oder diese für Sie zu störend sind.
Folgende Nebenwirkungen treten bei Kindern und Jugendlichen häufiger auf als bei Erwachsenen: Lungenentzündung, Austrocknung, vermindertes Schwitzen, Mittelohr-, Hals- oder Stirnhöhlenentzündungen, Entzündungen der oberen Atemwege, Husten, Nasenbluten, Schnupfen, Bauchschmerzen, Erbrechen, Ausschläge, Ekzeme, Fieber, Gewichtsverlust von 10% oder mehr.
Zonegran Kapseln zu 100 mg können Überempfindlichkeitsreaktionen der Haut- und Atmungsorgane auslösen, insbesondere bei Patientinnen oder Patienten mit Asthma, Nesselfieber (chronische Urtikaria) oder mit Überempfindlichkeit auf Acetylsalicylsäure und andere Rheuma- und Schmerzmittel.
Wenn Sie Nebenwirkungen bemerken, die hier nicht beschrieben sind, sollten Sie Ihren Arzt oder Apotheker bzw. Ihre Ärztin oder Apothekerin informieren.
Arzneimittel sollen für Kinder unerreichbar aufbewahrt werden.
Bei Raumtemperatur (15-25 °C) aufbewahren.
Das Arzneimittel darf nur bis zu dem auf der Packung mit «EXP» bezeichneten Datum verwendet werden.
